Nano-sized particles released by red blood cells reduce inflammation and fat deposition in immune cells, potentially paving the way for new methods of treating atherosclerosis. Atherosclerosis is a disease in which fat, cholesterol, and other substances build up inside artery walls. This can lead to plaque formation, which can block arteries and cause heart attacks and strokes. Immune cells play a key role in cleaning the blood, by interacting with red blood cell extracellular vesicles (RBCEVs), which are nano-sized particles released by red blood cells. Commonly referred to as "first responders" to infections, macrophages are immune cells which detect and clear pathogens and dead cells, and secrete molecules to activate other immune cells. To understand what happens to macrophages that are exposed to a high amount of RBCEVs and whether they prove to be beneficial in treating atherosclerosis, a multi-institutional team examined the mechanisms of how RBCEVs are internalised by macrophages and analysed the consequent changes. Led by Assistant Professor Minh Le from the Department of Pharmacology and the Institute for Digital Medicine (WisDM), Yong Loo Lin School of Medicine, National University of Singapore (NUS Medicine), the study demonstrated that the uptake of RBCEVs by macrophages was highly efficient, as the particles induced multiple changes in the macrophages. Following exposure to RBCEVs, the macrophages had decreased levels of proteins that promote inflammation, suggesting the potential use of RBCEVs to alleviate conditions associated with excessive inflammation. The macrophages also produced higher levels of an enzyme which protects cells against oxidative damage -- commonly observed in inflammatory and cardiovascular diseases. In addition, the RBCEVs led to higher resistance to lipid uptake in the macrophages, reducing fat deposition -- which characterises the condition of atherosclerosis. Published in the Journal of Extracellular Vesicles, one of the top Cell Biology journals, the study was conducted in collaboration with the Department of Surgery, Department of Physiology, Nanomedicine Translational Research Programme, and Cardiovascular Research Institute at NUS Medicine; the School of Mechanical and Aerospace Engineering and Lee Kong Chian School of Medicine, Nanyang Technological University (NTU); and the Institute of Molecular and Cell Biology, Agency for Science, Technology and Research (A*STAR). Asst Prof Minh Le said, "We have known for a while that RBCEVs tend to go to macrophages when they enter the body, but we did not realise some of the implications until now. The properties of RBCEVs that we have uncovered here are desirable for treating atherosclerosis and possibly other inflammatory diseases." The team hopes to leverage their improved understanding to manipulate RBCEV uptake by macrophages and thus adjust the distribution of RBCEVs to different tissues in the body. The team also plans to further explore the therapeutic potential of RBCEVs by combining their natural beneficial properties with exogenously loaded drugs designed to treat inflammatory conditions. These plans are part of the expansion of their ongoing work on developing the RBCEV platform for the treatment of various diseases including cancer, cancer-associated muscle loss, and COVID-19. WEDNESDAY, June 7, 2023 -- For years, doctors have thought that a more extensive surgery that removes a wider swath of lymph nodes was the best option for certain patients with bladder cancer. Now, a new clinical trial is upending that belief. Researchers found that the strategy — known as extended lymphadenectomy — was no better than standard surgery at prolonging patients' lives or the amount of time they lived free of a bladder cancer recurrence. What's more, it increased their risk of complications like blood clots, serious infections and death within 90 days of surgery. Experts said the long-awaited results, from a clinical trial begun over a decade ago, will likely change many surgeons' practice. At the center of the issue are patients with muscle-invasive bladder cancer — where the cancer has spread past the inner lining of the bladder and penetrated the muscle tissue of the bladder wall. That raises the chances that tumor cells have escaped to lymph nodes just outside the bladder, or beyond. In the United States, more than 83,000 people are diagnosed with bladder cancer each year, and about 25% have muscle-invasive cancer, according to the American Urological Association. Those patients typically have the whole bladder surgically removed, along with other pelvic tissues — including nearby lymph nodes that might contain tumor cells. But at many hospitals, surgeons have been doing extended lymphadenectomy procedures for years — taking out additional nodes, farther up into the abdomen. That was based on studies showing that patients' survival tended to improve when a greater number of lymph nodes was removed. The approach "has become increasingly embedded in our teaching," said lead investigator Dr. Seth Lerner, chair of urologic oncology at Baylor College of Medicine, in Houston. What's been lacking is a "gold standard" clinical trial to rigorously test whether the extended strategy is really more effective than standard pelvic node removal. Lerner and his colleagues are now reporting on just such a trial, funded by the U.S. National Institutes of Health. And the verdict is: Extended lymph node removal is not the better procedure after all. The trial involved 618 patients at 27 medical centers in the United States and Canada. All were undergoing a radical cystectomy (complete bladder removal) for muscle-invasive bladder cancer. During the procedure, the patients were randomly assigned to have either standard removal of the pelvic lymph nodes or extended node removal. Patients in the extended group had substantially more nodes removed — typically around 39, versus 24 in the comparison group. Still, they were no more likely to have their lymph nodes test positive for cancer spread. Nor did they fare any better over the next six years, the trial found. Patients in both treatment groups had similar overall survival and disease-free survival — the amount of time they lived free of a cancer recurrence. The actual survival figures were not reported. There were some numbers on harms, though. Over 90 days, more patients in the extended lymphadenectomy group died: 19, versus seven in the standard surgery group. They were also more likely to have side effects like blood clots and sepsis — a potentially life-threatening complication of certain infections. In all, 49% of patients in the extended group and 42% in the standard group had some kind of serious side effect. Lerner presented the findings Monday at the American Society of Clinical Oncology's annual meeting, in Chicago. Findings released at meetings are generally considered preliminary until they are published in a peer-reviewed journal. Why wasn't the extended strategy the better choice? Lerner said it's not clear, but he noted that muscle-invasive bladder cancer is a curable disease. And patients often not only receive surgery, but — if they're healthy enough to tolerate it — chemotherapy. More than half of the trial patients received pre-surgery chemo. Lerner said it's also possible that many patients were cured after their surgery, and taking out additional lymph nodes made no difference. The bottom line, he said, is that for patients like those in the trial, there is no reason to use the extended strategy. A bladder cancer specialist who was not involved in the trial agreed that the findings should change practice. "A lot of people thought this would be a better technique," said Dr. Scot Niglio , an oncologist at NYU Langone's Perlmutter Cancer Center, in New York City. It made sense that removing more nodes might improve patients' outlook. "But," Niglio said, "this shows that just because we can doesn't mean we should." The good news, both doctors said, is that muscle-invasive bladder cancer is treatable, and more options have become available in recent years. Patients whose cancer has a high risk of coming back, for example, may receive immunotherapy — drugs that enlist the immune system to battle the cancer. But relative to other common cancers, bladder cancer does not get much attention. "It just doesn't get talked about a lot," Niglio said. One important point about the disease, he noted, is that the primary controllable risk factor is smoking. So, it's yet another reason for smokers to quit. As for symptoms of the disease, blood in the urine is often the first. That can also be a sign of many other conditions, Lerner said, and people should see their doctor to get to the bottom of it. The rate of clinical remission was 47.9 percent in patients who received combination induction therapy with guselkumab and golimumab compared with either treatment alone (31.0 percent and 20.8 percent, respectively) at 38 weeks1 SPRING HOUSE, Pa., Oct. 10, 2022 /PRNewswire/ -- The Janssen Pharmaceutical Companies of Johnson & Johnson today announced data from an ongoing analysis of a Phase 2a clinical trial showing adults with moderately to severely active ulcerative colitis (UC) who received 12 weeks of combination induction therapy with guselkumab and golimumab, followed by a transition to guselkumab alone for maintenance, achieved a clinical remission ratea (based on the modified Mayo score [mMayo])b at week 38 of 47.9 percent, a higher rate than induction and maintenance treatment with either guselkumab alone (31.0 percent) or golimumab alone (20.8 percent).1,2 Patients had comparable rates of adverse events (AEs) across the treatment groups.1 Guselkumab alone, or the combination of guselkumab and golimumab are under clinical investigation and not approved for the treatment of adults with UC in the U.S. The VEGA study is the first randomized controlled trial to evaluate the efficacy and safety of combination therapy with an interleukin (IL)-23p19 subunit antagonist (guselkumab) and a tumor necrosis factor-alpha (TNFα) antagonist (golimumab) in UC.3,4 Detailed results were presented today as an oral presentation (OP087) at the United European Gastroenterology (UEG) Week 2022 congress taking place in-person in Vienna, Austria and virtually from October 8-11.1 "Exploring combinations of advanced therapies is an important step in continuing to innovate for the many patients living with ulcerative colitis," said Bruce E. In the VEGA study, patients with moderately to severely active UC were randomized (1:1:1) to receive combination therapy with guselkumab and golimumab, guselkumab monotherapy, or golimumab monotherapy.1 After endpoints were evaluated at week 12, patients in the monotherapy groups continued their initially assigned treatment, while patients randomized to the combination therapy group transitioned to guselkumab alone as a maintenance therapy.1 Patients remained blinded and received their respective therapies through the final efficacy assessment at week 38. Clinical remissiona,d was assessed based on the modified Mayo score (mMayo),b as well as the total Mayo score.e Symptomatic remission,f endoscopic improvement,g endoscopic remission,h histologic remission,i and composite histologic-endoscopic endpointsg,i were also assessed at week 38 in an exploratory analysis.1 Safety was evaluated through week 50 (16 weeks after the final dose of study intervention at week 34).1 Patients who received combination therapy with guselkumab and golimumab followed by maintenance treatment with guselkumab achieved clinical remissiona (mMayob) at week 38 (47.9 percent [34/71]) versus 31.0 percent (22/71) and 20.8 percent (15/72) of patients who received guselkumab or golimumab alone, respectively (nominal p=<0.05).1 Rates of clinical remissiond as defined by the total Mayo scoree at week 38 were similar to those based on the mMayo,a,b with 43.7 percent (31/71) in the combination therapy followed by guselkumab group achieving this endpoint, compared to either the guselkumab or golimumab monotherapy groups, (31.0 percent [22/71] and 22.2 percent [16/72], respectively).1 Also, patients receiving combination therapy followed by guselkumab had the following outcomes at week 38:1 Endoscopic improvementg and the composite histologic-endoscopic endpointg,i were higher in patients who received combination therapy followed by treatment with guselkumab compared to patients who received guselkumab or golimumab alone. Endoscopic remissionh and histologic remissioni were higher in patients who received combination therapy followed by treatment with guselkumab compared to patients who received golimumab alone. Patients receiving combination therapy followed by maintenance treatment with guselkumab and patients receiving treatment only with guselkumab monotherapy achieved the same rate of symptomatic remissionf of 69 percent at week 38, which was greater than the golimumab monotherapy group of 59.7 percent.1 Rates of AEs were comparable through week 50 of the study.1 Respectively, the combination therapy followed by guselkumab, guselkumab alone, and golimumab alone groups had AE rates of 63.4, 64.8, and 76.4 percent and infection rates of 31.0, 23.9, and 31.9 percent.1 All groups had the same serious AE rates of 5.6 percent and the same serious infection rates of 2.8 percent.1 Overall, 13.1 percent of patients discontinued their treatment prior to the final dose of study intervention.1 "The 38-week VEGA data suggest the potential that guselkumab and golimumab combination therapy has in patients with moderately to severely active ulcerative colitis for whom previous conventional treatments have inadequately managed their disease or improved their well-being," said Jan Wehkamp, M.D., Ph.D., Vice President, Gastroenterology Disease Area Leader, Janssen Research & Development, LLC. IMMUNE SYSTEM PROBLEMS Rarely, people using TNF blockers have developed lupus-like symptoms. Tell your doctor if you have any symptoms such as a rash on your cheeks or other parts of the body, sensitivity to the sun, new joint or muscle pain, becoming very tired, chest pain or shortness of breath, swelling of the feet, ankles, and/or legs. LIVER PROBLEMS Serious liver problems can happen in people using TNF blockers, including SIMPONI®. BLOOD PROBLEMS Low blood counts have been seen with SIMPONI®. If this occurs, your body may not make enough blood cells to help fight infections or help stop bleeding. Your doctor will check your blood counts before and during treatment. Tell your doctor if you have signs such as fever, bruising, bleeding easily, or paleness. OTHER CONSIDERATIONS TO TELL YOUR DOCTOR Tell your doctor if you are allergic to rubber or latex. The needle cover contains dry natural rubber. Tell your doctor if you are pregnant, planning to become pregnant or are breastfeeding or have a baby and were using SIMPONI® during pregnancy. Tell your baby's doctor before your baby receives any vaccine because of an increased risk of infection for up to 6 months after birth. ALLERGIC REACTIONS Allergic reactions can happen in people who use TNF-blocker medicines, including SIMPONI®. Tell your doctor if you have any symptoms of an allergic reaction while taking SIMPONI® such as hives, swollen face, breathing trouble, or chest pain. Some reactions can be serious and life-threatening. Common side effects of SIMPONI® include: upper respiratory tract infection, reaction at site of injection, and viral infections. PSORIASIS New or worse psoriasis symptoms may occur. Tell your doctor if you develop red scaly patches or raised bumps that are filled with pus.
