Electrical Stimulation for Erector Spinae Plane Catheter Insertion: A Randomized Controlled Trial

The erector spinae plane (ESP) block has been studied for analgesia in shoulder surgery as a phrenic nerve-sparing alternative. However, successful ESP catheter placement appears multifactorial, with failure mechanisms including lamination, plane collapse, or catheter overcoiling. Electrical stimulation (ES) is a common technique used in regional anesthesia to detect possible intraneural placement. ES of the erector spinae muscle complex may objectively guide proper interfascial catheter placement and improve local anesthetic spread. The primary goal of this study is to establish if ESP catheter placement with the addition of ES to ultrasound (US) guidance facilitates accurate catheter placement. This study will further characterize postoperative analgesia and the incidence of brachial plexus stimulation for patients who receive ES-assisted ESP catheter placement.

Do Taking Supplements Provide Additional Benefits in Rotator Cuff-Related Shoulder Pain?

Rotator cuff-related shoulder pain is a pathology characterized by pain and functional impairment originating from one or more rotator cuff tendons. The lifetime incidence of rotator cuff-related shoulder pain is around 67%, with an annual incidence exceeding 1%. Many patients continue to experience pain and functional loss for up to one year, and more than half of the patients report shoulder pain persisting for over three years.
The pathophysiology underlying rotator cuff-related shoulder pain continues to be a subject of ongoing research and uncertainty, with many aspects yet to be fully elucidated. The most common belief regarding its pathogenesis involves the role of inflammation. This hypothesis is supported by the accumulation of inflammatory cells in tendons, oxidative stress, and increased levels of pro-inflammatory cytokines. In tendon pathologies with inflammatory cell accumulation and increased cytokine levels, the use of antioxidants and anti-inflammatory agents in addition to conservative treatment contributes to tendon healing. Anti-oxidants and anti-inflammatories are substances capable of preventing or delaying certain cell damage.The use of anti-inflammatory and antioxidant supplements such as Vitamin C (Vit-C), Vitamin D (Vit-D), Omega-3, and Magnesium (Mg) is recommended. Despite indicating exercise as the gold standard for managing rotator cuff-related shoulder pain and the demonstrated anti-inflammatory and anti-oxidant properties of the mentioned supplements, there are still gaps in the understanding of their effectiveness in rotator cuff-related shoulder pain.
Based on these gaps, the goal of this study is to investigate the effects of supplements (Vit-C, Vit-D, Omega-3, and Mg) given in addition to exercise on patients' blood parameters (TNF-a, IL-6, and CRP levels), pain, functional status, quality of life, and patient satisfaction in individuals with rotator cuff-related shoulder pain.

Comparison of combined suprascapular and axillary nerve pulsed radiofrequency and nerve block for the treatment of primary frozen shoulder: a prospective cohort study

OBJECTIVESTo compare the effects of pulsed radiofrequency (PRF) and block of the suprascapular nerve (SSN) and axillary nerve (AN) in patients with primary frozen shoulder (FS).METHODSPatients with primary FS received PRF (Group P) or block (Group B). Shoulder pain during rest, activity and sleep was measured by a numerical rating scale (NRS), the Shoulder Pain and Disability Index (SPADI) was used to assess shoulder function and disability, and the passive range of motion (PROM) of the shoulder joint was measured by a digital inclinometer. Assessments were made at baseline and 2 weeks and 1, 3, and 6 months after the procedure.RESULTSOf the 74 patients, 63 were eventually included, and a total of 3 patients were lost to follow-up. Finally, 60 patients (30 in each group) completed the final analysis. There was a significant improvement in all outcome measures from baseline to 6 months after the procedure. Compared with those in group B, the NRS scores during activity and sleep in group P decreased more at 6 months after the procedure (p = 0.005 and 0.028). SPADI total scores were lower at 3 and 6 months after the procedure (p = 0.021 and 0.001). At different time after the procedure, most of the parameters of PROM improved more in group P than those in group B (flexion at 3 and 6 months, p = 0.042 and <0.001; abduction at 3 and 6 monthse, p = 0.001 and 0.001; extension at 3 and 6 months, p = 0.038 and 0.007, internal rotation at 6 months, p = 0.015; external rotation at 1, 3, and 6 months, p = 0.002, 0.002, and 0.001, respectively).CONCLUSIONSIn patients with primary FS who completed both manipulation under anesthesia and intra-articular injections, PRF with SSN and AN appears to provide better pain relief, better PROM recovery, and more shoulder function improvement than nerve block treatment.

Post-laparoscopic Shoulder Pain Management: A Narrative Review

PURPOSE OF REVIEWPost-laparoscopic shoulder pain (PLSP) can slow patient recovery and extend hospital stays, making its management crucial for patients undergoing laparoscopic surgery. Current consensus guidelines say little about how to prevent or manage PLSP. In this context, a multimodal approach to PLSP management that maybe extend beyond the pharmaceutical interventions currently employed. A variety of devices comprising both invasive and noninvasive approaches are available to patients, serving as adjuvants to analgesics. In this review, we explore the potential causes of PLSP. Additionally, by searching relevant databases and reviewing existing literature, we provide a comprehensive summary of current PLSP management strategies excluding analgesics.RECENT FINDINGSA total of 30 articles were reviewed. The review identified a number of different treatments for PLSP, including trendelenburg position, discharge of residual gas, pulmonary recruitment manoeuvre, low-pressure pneumoperitoneum and phrenic nerve block, among others. However, the inconsistencies in the study designs resulted in disparate conclusions. While the current studies provide valuable insights, there is a clear need for further research in this area.

Fluoroscopy-guided suprascapular and subscapular articular nerve blocks for chronic shoulder pain: A 12-week observational study

IntroductionChronic shoulder pain is a common musculoskeletal complaint. This study evaluates the effectiveness of fluoroscopy-guided suprascapular (SSN) and subscapular (SCN) articular branch blocks in managing chronic shoulder pain. The primary objective was to assess pain relief using a numerical rating scale (NRS) and functional improvement using Shoulder Pain and Disability Index (SPADI) over 12 weeks.MethodsThis prospective, single-arm observational study included 70 adults with chronic shoulder pain (≥3 months) meeting predefined criteria. All patients underwent fluoroscopy-guided SSN and SCN articular branch blocks with bupivacaine (2 ml, 0.5 %) and triamcinolone (0.5 ml, 20 mg) per site. NRS and SPADI were recorded at baseline and biweekly for 12 weeks. Secondary outcomes included range of motion (ROM) improvements and night pain resolution. Statistical analysis involved repeated measures ANOVA for normally distributed data and non-parametric tests for skewed data (p < 0.05 considered significant).ResultsAt 12 weeks, 78 % of patients achieved ≥50 % pain reduction. Mean NRS decreased from 7.6 ± 1.1 to 3.9 ± 1.1, while SPADI pain and disability scores improved by 57.1 % and 57.4 %, respectively (p < 0.001). Night pain resolved in all affected patients within two weeks. Repeated measures ANOVA confirmed significant improvements in pain and disability scores (p < 0.001). Mean lateral abduction improved by 29° (95 % CI: 22.8°-35.2°, p < 0.001).ConclusionFluoroscopy-guided SSN and SCN articular branch blocks provide significant pain relief and functional improvement in chronic shoulder pain, offering a potential alternative to intra-articular injections or surgery in select patients.

Four-year Follow-up Data on SPRINT® PNS for Low Back Pain Highlights Sustained Pain Relief Avoiding Costly and Invasive Treatments

To understand the long-term impact of treatment with SPRINT PNS, surveys were sent to participants from a prior prospective multicenter study who reported clinically meaningful reductions in pain, disability, or pain interference.
