Semantic segmentation for tooth cracks using improved DeepLabv3+ model.

Objective:
Accurate and prompt detection of cracked teeth plays a critical role for human oral health. The aim of this paper is to evaluate the performance of a tooth crack segmentation model (namely, FDB-DeepLabv3+) on optical microscopic images.
Method:
The FDB-DeepLabv3+ model proposed here improves feature learning by replacing the backbone with ResNet50. Feature pyramid network (FPN) is introduced to fuse muti-level features. Densely linked atrous spatial pyramid pooling (Dense ASPP) is applied to achieve denser pixel sampling and wider receptive field. Bottleneck attention module (BAM) is embedded to enhance local feature extraction.
Results:
Through testing on a self-made hidden cracked tooth dataset, the proposed method outperforms four classical networks (FCN, U-Net, SegNet, DeepLabv3+) on segmentation results in terms of mean pixel accuracy (MPA) and mean intersection over union (MIoU). The network achieves an increase of 11.41% in MPA and 12.14% in MIoU compared to DeepLabv3+. Ablation experiments shows that all the modifications are beneficial.
Conclusion:
An improved network is designed for segmenting tooth surface cracks with good overall performance and robustness, which may hold significant potential in computer-aided diagnosis of cracked teeth.

Assessment of Occlusal Load Strength of Glass Ionomer Cement and Composite in Class V Cavities: An In-Vitro Study.

BACKGROUND:
Glass ionomer cement (GIC) is widely used in dentistry due to its chemical adhesion to dental tissues, biocompatibility, and anti-cariogenic potential but they have relatively weak mechanical properties. Resin composites have been widely regarded as the first choice for direct restorations but their polymerization shrinkage has remained a major problem. It has the potential to cause tooth debonding. The composite interface leads to postoperative sensitivity, secondary caries, enamel cracks, and microleakage. A restorative material's capacity to withstand occlusal stresses and support the remaining tooth structure depends on this property. Although class V restorations are predominantly done with GIC, this study was done to compare the strength of composite with the same. The GIC restore glass which is commonly used was tested against restofill composite. The main objective of conducting the study was to compare the compressive strength of the composite vs GIC in cervical cavities. So the aim of the study is to assess the occlusal load strength of GIC and composite in class V cavities using the universal testing machine.
MATERIALS AND METHODS:
This study was employed as an in vitro study involving 20 natural central incisor teeth without any carious lesions. Class V cavity preparation was done and the selected teeth were divided into two groups of ten each. The cavities were filled with D Tech Restore GIC and composite restorations (restofill), respectively, polished, and then subjected to testing. An eccentric load was applied to the tooth structure using an Instron (Instron E3000 Electropuls, Instron, Norwood, United States) - Universal testing machine with a cross-head speed of 1mm per minute, and the stresses were further analyzed in the presence of an occlusal loading test using a stainless steel jig of 1mm diameter which led to the sectioning of the tooth buccolingually under the applied load.
RESULTS:
An independent t-test was used to assess the results, and it was concluded that the results were statistically significant (p<0.05) at p=0.034.
CONCLUSION:
Conclusively, the results suggested that the occlusal load strength of the composite is greater when compared to GIC.

Is use of CBCT without proper training justified in paediatric dental traumatology? An exploratory study.

BACKGROUND:
Proper skills in radiographic diagnosis are essential for optimal management of dental trauma.
AIM:
To assess diagnostic accuracy obtained by paediatric dentists using Cone Beam Computed Tomography (CBCT) without specific training and to compare this with their performance using intraoral radiographs.
METHODS:
Intraoral and CBCT images of 89 teeth, spread over twenty dental trauma cases were presented in random order to nine paediatric dentists. Diagnostic findings were compared with those of a benchmark reference. Sensitivity and specificity were calculated and compared using paired t-tests.
RESULTS:
Overall, observers' diagnostic performance was rather poor with significantly higher sensitivity when using 2D images (P = 0.017). Performance differed considerably according to the type of pathology. Using either imaging modality, sensitivity for diagnosing apical pathology and root fractures was high while the opposite was seen for inflammatory root resorption, root cracks and subluxations. Statistically significant differences between imaging modalities were seen for root fractures (P = 0.013) and apical pathology (P = 0.001), in favor of 3D, and for crown fractures (P = 0.009) in favor of 2D.
CONCLUSION:
Overall poor performance of paediatric dentists indicates that additional training in radiographic diagnosis is required. In order to justify the use of CBCT to increase diagnostic performance, proper training of the paediatric dentist is mandatory.
