Non-cardiac chest pain is a common clinical problem encountered in our practice but at present, the results of treatments are unsatisfactory. The pathogenesis remains unknown but altered motility of the esophagus and psychological factors including anxiety have been implicated as important factors. Reports of the single use of anticholinergic drugs and anxiolytics have yielded conflicting results, has been demonstrated to yield marginal or of no value. However the use of the combination therapy, especially with a double blind fashion have not been reported. On that basis, we propose to use a combination of anti-cholinergic and tranquilliser for the symptomatic treatment of non-cardiac chest pain. The aim of this study is to evaluate the efficacy of combination therapy of anti-cholinergic and anxiolytic drugs in the treatment of non-cardiac chest pain.

Validated green high‐performance liquid chromatographic methods for the determination of coformulated pharmaceuticals: A comparison with reported conventional methods

The introduction of sustainable development concepts to analytical laboratories has recently gained interest, however, most conventional high‐performance liquid chromatography methods do not consider either the effect of the used chemicals or the amount of produced waste on the environment. The aim of this work was to prove that conventional methods can be replaced by greener ones with the same analytical parameters. The suggested methods were designed so that they neither use nor produce harmful chemicals and produce minimum waste to be used in routine analysis without harming the environment. This was achieved by using green mobile phases and short run times. Four mixtures were chosen as models for this study; clidinium bromide/chlordiazepoxide hydrochloride, phenobarbitone/pipenzolate bromide, mebeverine hydrochloride/sulpiride, and chlorphenoxamine hydrochloride/caffeine/8‐chlorotheophylline either in their bulk powder or in their dosage forms. The methods were validated with respect to linearity, precision, accuracy, system suitability, and robustness. The developed methods were compared to the reported conventional high‐performance liquid chromatography methods regarding their greenness profile. The suggested methods were found to be greener and more time‐ and solvent‐saving than the reported ones; hence they can be used for routine analysis of the studied mixtures without harming the environment.

The Impact of Digestive and Colon Drugs on the Human Hormones Profile

Hormones play an important role in the digestive system. The main hormones that control digestion are gastrin, secretin, and cholecystokinin. Herein, the current study is concerned with assessing the effect of spasmo canulase and librax drugs on the human hormones profile. Blood samples were withdrawn from adult patients to measure serum FSH, E2, LH, prolactin, progesterone, DHEAS, testosterone, TSH, T3, T4, fasting insulin, and cortisol. All hormone concentrations were determined quantitatively using ELISA procedure. Intriguingly, the present study showed putative changes including thyroid and sex hormonal profiles. Eventually, we concluded that the prospective study could be important in drug dose optimization and providing new medical guidelines to avoid side effects that could harm patients.

Spasmocanulase in irritable bowel syndrome.

OBJECTIVES:

Irritable bowel syndrome is a functional gastro intestinal disorder, with various symptomatology and difficult to treat using several medications. Spasmocanulase, which has an anti-spasmodic and anti-flatulence effect and contains several ingredients, was tried in patients with irritable bowel syndrome, who had been on other medications previously without improvement.

METHODS:

At the gastroenterology out-patient clinic, Armed Forces Hospital, Riyadh, Kingdom of Saudi Arabia, 21 patients who were diagnosed with irritable bowel syndrome for more than 2 years on treatment and did not benefit from these medications, received spasmocanulase one tablet 3 times a day and followed in the gastroenterology out-patient clinic, every 6 weeks for 6 months. Their previous medications were discontinued which, had been used for different durations and in different combinations included Mebeverine, Colpermin, Normacol, Importal, Librax. The main symptoms were different types of abdominal pain, bloating, flatulence, diarrhea, constipation or both.

RESULTS:

There was improvement or disappearance of the symptoms in more than 50% of the patients who were not improved on previous medication used for more than 2 years. The overall improvement in symptoms ranged between 43-75% in these patients, when followed up in the clinic at 6 weeks and 3 months. Spasmocanulase caused improvement in abdominal pain, flatulence and bloating compared to their previous medications. Only 43% of patients with diarrhea, showed improvement on spasmocanulase.

CONCLUSION:

Although the number of patients, we studied is small, this study has shown that spasmocanulase is beneficial in improving symptoms of irritable bowel syndrome.
