The primary purpose of this study is to evaluate the overall effect of caregivers receiving a study-provided caregiver psycho-education and skills training program on the number of treatment failures (psychiatric hospitalization, psychiatric emergency room (ER) visit, crisis center visit, mobile crisis unit intervention, arrest/incarceration, and suicide or suicide attempt) in patients under their care during a 12 month period.

Concomitant use of hydroxyzine and haloperidol did not worsen delirium in patients with cancer: A multicenter, retrospective, observational study

AbstractObjectivesThere is concern that hydroxyzine exacerbates delirium, but a recent preliminary study suggested that the combination of haloperidol and hydroxyzine was effective against delirium. This study examined whether the concomitant use of hydroxyzine and haloperidol worsened delirium in patients with cancer.MethodsThis retrospective, observational study was conducted at 2 general hospitals in Japan. The medical records of patients with cancer who received haloperidol for delirium from July to December 2020 were reviewed. The treatments for delirium included haloperidol alone or haloperidol combined with hydroxyzine. The primary outcome was the duration from the first day of haloperidol administration to the resolution of delirium, defined as its absence for 2 consecutive days. The time to delirium resolution was analyzed to compare the haloperidol group and hydroxyzine combination group using the log-rank test with the Kaplan–Meier method. Secondary outcomes were (1) the total dose of antipsychotic medications, including those other than haloperidol (measured in chlorpromazine-equivalent doses), and (2) the frequencies of detrimental incidents during delirium, specifically falls and self-removal of drip infusion lines. The unpaired t-test and Fisher’s exact test were used to analyze secondary outcomes.ResultsOf 497 patients who received haloperidol, 118 (23.7%) also received hydroxyzine. No significant difference in time to delirium resolution was found between the haloperidol group and the hydroxyzine combination group (log-rank test, P = 0.631). No significant difference between groups was found in either chlorpromazine-equivalent doses or the frequency of detrimental incidents.Significance of resultsThis study showed that the concomitant use of hydroxyzine and haloperidol did not worsen delirium in patients with cancer.

AbstractThe story of the 1967 appearance of the powerful psychedelic 2,5‐dimethoxy‐4‐methylamphetamine (DOM, STP) commonly omits details and often includes hyperbole and inaccuracies. It is well known how and when the drug was first distributed to the public for free by Owsley Stanley, but the role that Alexander Shulgin played in providing that material is not as well understood. In the interest of transparency and historical accuracy, this article attempts to present an accurate account of this well‐known but inadequately detailed event. It follows DOM's development as an experimental substance believed to hold potential promise in psychotherapeutic applications through its appearance as a street drug generating bad press and a lasting bad impression among the public. One of the more interesting questions is why Shulgin would have taken such an immense risk in releasing this material to clandestine operators. While DOM was still legal it was also Dow's intellectual property, so discovery of his involvement could have jeopardized his career. The escape is especially curious as all fingers would logically first point towards Shulgin as the source. Drawing from published and unpublished sources, the authors attempt to suggest answers. DOM rapidly faded into oblivion before human pharmacodynamics and pharmacokinetics could be established. In this account, the reader is informed of the potential value that the compound played in non‐clinical molecular neuroscience, elucidating receptor specificity of new drugs, and how mistaken warnings about combining DOM with chlorpromazine led to better non‐pharmacological drug crisis response.

Predicting Clinical Improvement in Early Psychosis Using Circuit-Based Resting-State Functional Magnetic Resonance Imaging

AbstractBackground and HypothesisIdentifying biomarkers that predict treatment response in early psychosis (EP) is a priority for psychiatry research. Previous work suggests that resting-state connectivity biomarkers may have promise as predictive measures, although prior results vary considerably in direction and magnitude. Here, we evaluated the relationship between intrinsic functional connectivity of the attention, default mode, and salience resting-state networks and 12-month clinical improvement in EP.Study DesignFifty-eight individuals with EP (less than 2 years from illness onset, 35 males, average age 20 years) had baseline and follow-up clinical data and were included in the final sample. Of these, 30 EPs showed greater than 20% improvement in Brief Psychiatric Rating Scale (BPRS) total score at follow-up and were classified as “Improvers.”Study ResultsThe overall logistic regression predicting Improver status was significant (χ2 = 23.66, Nagelkerke’s R2 = 0.45, P < .001, with 85% concordance). Significant individual predictors of Improver status included higher default mode within-network connectivity, higher attention-default mode between-network connectivity, and higher attention-salience between-network connectivity. Including baseline BPRS as a predictor increased model significance and concordance to 92%, and the model was not significantly influenced by the dose of antipsychotic medication (chlorpromazine equivalents). Linear regression models predicting percent change in BPRS were also significant.ConclusionsOverall, these results suggest that resting-state functional magnetic resonance imaging connectivity may serve as a useful biomarker of clinical outcomes in recent-onset psychosis.
