529 patients performed elective LCC at Chonbuk National University Hospital between April 2014 and August 2015. Total 509 patients were enrolled by inclusion criteria.
They were randomized studied by comparing with antibiotic group (n=249, AG, cefotetan 1g, 1 dose/prophylactic) and non-antibiotic group (n=260, NAG) by table of random numbers. The clinical variables were pre and post-operatively blood tests enclude WBC, ESR, CRP, body temperatures, symptoms and imaging of chest x-ray to evaluate the infections.

Evaluation of the Disk Diffusion Test for Bacteroides fragilis Group Clinical Isolates

BackgroundBacteroides fragilis group (BFG) isolates are the most frequently isolated gram-negative anaerobic bacteria and exhibit higher levels of antimicrobial resistance than other anaerobic bacteria. Reliable susceptibility testing is needed because of reports of resistance to the most active antibiotics. Recently, the European Committee on Antimicrobial Susceptibility Testing (EUCAST) introduced disk zone diameter breakpoints. We evaluated the disk diffusion test (DDT) for susceptibility testing of BFG isolates compared with the agar dilution method.MethodsIn total, 150 BFG isolates were collected from three institutes in Korea. The agar dilution method was conducted according to the CLSI guidelines. DDT was performed following the EUCAST guideline. Fastidious anaerobe agar supplemented with 5% defibrinated horse blood was used as the culture medium. Nine antimicrobials were evaluated: penicillin, cefoxitin, cefotetan, imipenem, meropenem, piperacillin-tazobactam, clindamycin, moxifloxacin, and metronidazole.ResultsThe categorical agreement (CA) between the two methods was >90.0% for imipenem, meropenem, clindamycin, and metronidazole. However, the CA for piperacillintazobactam was low, at 83.2%. Major errors were found: 5.4% for imipenem, 7.4% for meropenem, and 12.8% for piperacillin-tazobactam. All minor errors were <10%. We propose using the area of technical uncertainty (ATU) zone-overlapping area for susceptible and resistant strains to reduce errors in the DDT. Outside the ATU, the CAs of cefoxitin, cefotetan, and piperacillin-tazobactam were >90.0%, whereas that of moxifloxacin was increased to 88.5%.ConclusionsThe DDT can be a useful alternative antimicrobial susceptibility test for BFG isolates when using the ATU zone to reduce errors.

The Bacterial Species Behind the Wound and Their Antibacterial Resistant Pattern: A Three‐Year Retrospective Study at St. Dominic Hospital, Akwatia, Ghana

ABSTRACTBackgroundWound infections are often underestimated issues that can lead to chronic illnesses, and since the introduction of antibiotics, wound complications have become less common. However, due to the increased and irrational use of these antibiotics, the resistance in the bacterial isolates has become very common. This has led to reduced treatment options, delay in wound healing, and high treatment costs. This study aimed to investigate bacterial wound infections and their antibiotic resistance at St. Dominic Hospital, Ghana.MethodsA total of 517 records of wound swab culture and susceptibility testing, and patient demographics from 2020 to 2022 were collected from the microbiology unit of St. Dominic Hospital in the Eastern Region of Ghana. The data were entered into Microsoft Excel 2019, cleaned, and exported into IBM SPSS v26 for the statistical analysis. p < 0.05 was considered statistically significant for all analyses.ResultsThe overall prevalence of bacteriological agents causing wound infection in individuals who visited the St. Dominic Hospital from 2020 to 2022 was 70.21% (363/517), with S. aureus 79/363 (21.76%) being the most abundant isolate. Out of the 79 S. aureus isolated, 40 (50.63%) and 39 (49.37%) were resistant to ampicillin and cephalexin, respectively. More than 50% of the predominant Gram‐negative isolate, K.pneumoniae, were resistant to clindamycin 45/72 (62.50%) but susceptible to levofloxacin 70/72 (97.22%), cefotetan 69/72 (95.83%), and chloramphenicol 67/72 (93.06%).ConclusionAntibacterial susceptibility patterns revealed significant resistance trends, particularly among Gram‐negative isolates, emphasizing the urgent need for prudent antibiotic use and ongoing surveillance to combat resistance.

Audiological Profile of a Rare Case with 1 kHz Notch Audiogram

A notch is defined as the frequency point at which hearing loss is greater than 15 dB when compared to one octave above and below. C3 dip or 1 kHz notch is rarely seen and not much information is known about the clinical profile of such condition. The aim of this case report is to highlight the audiological profile of a case with 1kHz notch and discuss the possible causes for the same. Case A (16 yrs) was referred with a complaint of hearing loss and speech understanding difficulty specially at school. The teen had taken multiple medications for several health related issues like malaria, appendicitis and the understanding difficulty was evident during this period. Detailed audiological evaluation revealed a significant C3 dip in the right ear and normal hearing sensitivity in the left ear. Evidences from literature suggests strong correlation between drugs like Cefotetan, cefotaime, piperacillin, ampicillin (appendicitis treatment) and chloroquine (malaria) and hearing loss. Hence, we concluded that the possible cause of 1khz is ototoxic medication.
