The radiation exposure resulting from medical imaging is a topic of some concern. Nuclear medicine provides potentially life-saving information regarding physiological processes, and is of particular value in children where the rapid and unequivocal diagnosis of pathological concerns is essential for the health of these patients. The overall objective of this investigation is to optimize pediatric patient absorbed dose by keeping it as low as possible while maintaining excellent diagnostic quality of nuclear medicine images. This is particularly important since children are at increased risk due to the enhanced radiosensitivity of their tissues and the longer time-period over which radiation effects may manifest. Current dosimetric estimations in children are based on either animal biokinetic or pharmacokinetic data from adults due to paucity of data that exists for children. This situation will be improved through the following specific aims:
Collect image-based pharmacokinetic (PK) data from patient volunteers in different age groups scheduled for routine nuclear medicine studies for Tc-99m mercaptoacetyltriglycine (MAG3), a radiopharmaceutical commonly used in pediatric nuclear medicine
Pool and analyze the data for different age groups for each radiopharmaceuticals and
Generate biokinetic models to be used in subsequent dosimetric models for the optimization of pediatric nuclear medicine procedures.
Since inadequate pharmacokinetic data currently exist in these patients, the investigators will use the data acquired in this study to establish PK models applicable to different age categories. Data on the pharmacokinetics of agents used in pediatric nuclear medicine are almost completely lacking. Internationally adopted dose coefficients (mSv/MBq) for pediatric nuclear medicine make age-dependent adjustments only for patient size and anatomical differences, while time-dependent kinetics from adult PK models are assumed due to the lack of kinetic data for children. The data obtained from this study will make it possible for the first time to determine how the PK in pediatric patients differs from adults. This will be done for Tc-99m MAG3, a radiopharmaceutical commonly used for pediatric nuclear medicine imaging. The overall hope is that results will allow the molecular imaging community to implement pediatric dose-reduction approaches that substantially improve upon current guidelines pointing to future technological advances that could yield even greater dose-reduction while simultaneously improving diagnostic image quality.

Feasibility study of radioactivity estimation of 99mTc and 123I-labeled radiopharmaceuticals using shielded syringes

This study investigates the feasibility of estimating the radioactivity of radiopharmaceuticals using shielded syringes. The radioactivities of ^99mTc-MDP, ^99mTc-HMDP, ^99mTc-ECD, ^99mTc-MAG₃, and ¹²³I-IMP were measured using a dose calibrator. Correlation coefficients and regression equations were obtained from the radioactivity in the shielded and unshielded syringes. ^99mTc-MDP was also measured for residual radioactivity after the administration. The correlation coefficients of ^99mTc-MDP, ^99mTc-HMDP, ^99mTc-ECD, ^99mTc-MAG₃, and ¹²³I-IMP were r_s = 0.9998, r_s = 0.9997, r_s = 0.9999, r_s = 0.9998, and r_s = 0.9888, respectively. The regression equations were y = 0.0364x + 0.0913, y = 0.0349x + 0.0273, y = 0.0343x - 0.0018, y = 0.0522x + 0.1215, and y = 0.0383x + 0.0058, respectively. The correlation coefficient for the residual radioactivity of ^99mTc-MDP was r_s = 0.9887 and the regression equation was y = 0.1505x + 0.0853. The radioactivity of ^99mTc- and ¹²³I-labeled radiopharmaceuticals in shielded syringes was accurately measured. It was suggested that the measuring shielded syringes could provide an estimate of the actual radioactivity.

Deconvolution of Tc-99m-Mercaptoacetyltriglycine Renograms with the Concomitant Use of a Sparse Legendre Polynomial Representation and the Moore-Penrose Pseudo-inverse

OBJECTIVESThis study aimed to introduce an improved deconvolution technique for Tc-99m-mercaptoacetyltriglycine renograms based on the combination of a sparse Legendre polynomial representation and the Moore-Penrose inversion matrix (LG). This method reduces the effect of noise on the measurement of renal retention function transit time (TT).METHODSThe stability and accuracy of the proposed method were tested using a renal database containing Monte Carlo-simulated studies and real adult patient data. Two clinical parameters, namely, split function (SF) and mean TT (meanTT), obtained with LG were compared with values calculated with the established method that combines matrix deconvolution and a three-point linear smoothing (F121) as recommended by the 2008 International Scientific Committee of Radionuclides in Nephrourology consensus on renal TT measurements.RESULTSFor simulated data, the root mean square error (RMSE) between the theoretical non-noisy renal retention curve (RRC) and the results of the deconvolution methods applied to the noisy RRC were up to two times lower with LG (p<0.001). The RMSE of the reconvoluted renogram and the theoretical one was also lower for LG (p<0.001) and showed better preservation of the original signal. The SF was neither improved nor degraded by the proposed method. For patient data, no statistically significant difference was found between the SF for the LG method compared with the database values, and the meanTT better agreed with the physician's diagnosis than the matrix or clinical software (Hermes) outputs. A visual improvement of the RRC was also observed.CONCLUSIONBy combining the sparse Legendre representation of the renogram curves and the Moore-Penrose matrix inverse techniques, we obtained improved noise reduction in the deconvoluted data, leading to better elimination of non-physiological signals -as negative values- and the avoidance of the smear effect of conventional smoothing on the vascular peak, which both influenced the meanTT measurement.

Abstract Aim Sufficient glomerular filtration rate (GFR) is mandatory in living kidney donors (LKD) before renal transplantation (RT). Guidelines recommend exogenous GFR (e. g. Tc-99m-DTPA) besides estimated GFR by plasma creatinine (eGFR). Tc-99m-MAG3 determines split function and tubular extraction rate (TER). This study evaluated a time-efficient one-day protocol for DTPA and MAG3 in potential LKD. Methods Retrospective analysis of 82 consecutive potential LKD (female, 52; age, 54 [19–73] a). DTPA GFR (Fleming formula) with blood sampling 169 (147–205) min post injection of 27 (18–59) MBq Tc-99m-DTPA. 199 (181–219) min after DTPA injection, same-day renal scintigraphy with 105 (65–150) MBq MAG3 followed (TER according to Bubeck). Sufficient GFR for RT was defined age- and sex-dependent based on British guidelines, normal TER at 70 % of age-dependent average. In 15 separate potential LKD, a blank blood sample before MAG3 injection allowed estimation of remaining DTPA counts and resulting TER error. Results Remaining Tc-99m-DTPA blood counts during sampling for MAG3 led to median TER underestimation by 12 (IQR, 8–15) ml/min/1.73 m2. Before blood count correction, 16/82 patients had reduced TER despite sufficient GFRFleming in 13/16 patients. After correction, TER remained reduced in 9/82 patients who all had sufficient GFRFleming. 10/82 patients had insufficient eGFR (CKD-EPI) but sufficient GFRFleming. Correlation coefficient for TER and GFRFleming was ρ = 0.76, for eGFR and GFRFleming ρ = 0.61, and for TER and eGFR ρ = 0.55. Conclusion A one-day protocol for Tc-99m-DTPA GFR and MAG3 dynamic imaging is feasible. If TER is calculated, the proposed method for blood count correction may be applied. However, relevance of the TER remains questionable due to frequent discordance to GFR.
