In patients with atrial fibrillation (AF) at risk of stroke, direct oral anticoagulants (DOAC) have been shown to be at least as effective as warfarin, offering advantages like reduced monitoring requirements and lower bleeding risks. Disadvantages include limited data in patients with severe chronic kidney disease and higher patient costs. In mid-2021, Region Örebro County, Sweden, decided to switch suitable patients with AF from warfarin to DOAC. Out of the 823 patients receiving warfarin therapy, 732 were identified as suitable candidates for DOAC, and 89 percent of these patients were successfully switched. Switching from warfarin to DOAC offers superiority and simplifies treatment and monitoring. However, individual needs and risk factors must be carefully considered before switching. As many patients in Sweden continue to be prescribed warfarin, this initiative could potentially model for other regions.

A new strategy for monitoring of direct oral anticoagulants in patients with cyanotic and complex congenital heart disease

Background:

Patients with congenital heart disease (CHD) often require an oral anticoagulation. Vitamin K antagonists (VKA) are the standard treatment, however, an increased hematocrit in patients with secondary erythrocytosis due to cyanosis complicates the correct measurement of the international normalized ratio. Direct oral anticoagulants (DOAC) could be an alternative, but data on their efficacy and safety in complex and cyanotic CHD patients are scarce. This study proposes a new strategy of DOAC monitoring in these patients using D-dimers and DOAC trough levels.

Methods:

This is a retrospective study including cyanotic and complex CHD patients requiring oral anticoagulation. Clinical, cardiac imaging and laboratory data were collected before and after start of DOAC. The new monitoring strategy consists of determination of D-dimers and DOAC trough levels at 1-4 weeks, 1-6 months, 6-12 months, >1 year after start of DOAC.

Results:

Eleven patients were included. For 10 patients D-dimers and DOAC trough levels were in target range. In one patient, D-dimers increased continuously after start of DOAC despite dose escalation, suggesting insufficient DOAC efficacy and finally requiring a switch to VKA. D-dimers subsequently decreased under VKA to the therapeutic range. In three patients, one thromboembolic and two minor bleeding complications occurred. No major complications were observed.

Conclusions:

We propose a new strategy of monitoring of oral anticoagulation with DOAC and report its implementation in clinical routine. Highlighting the importance of pharmacokinetic and -dynamic monitoring, this strategy could improve safety and efficacy of DOAC in cyanotic and complex CHD which, however, requires a prospective validation.

Impact of shared decision-making in Taiwanese patients with atrial fibrillation eligible for novel oral anticoagulant therapy

BACKGROUND/PURPOSE:

Shared decision-making (SDM) promotes patient awareness about medical conditions and treatments, facilitating patient involvement in care decisions. This two-stage multicenter study evaluated impacts of SDM in Taiwanese adults with atrial fibrillation (AF) eligible for novel oral anticoagulant (NOAC) therapy.

METHODS:

Participants were NOAC-naïve (part I) or dabigatran-experienced (part II). During Stage I, part I participants (n = 124) completed a semi-structured survey (understanding evaluation sections only) before and after viewing SDM materials on stroke prevention for AF. Surveys collected data on anxiety about AF, confidence in healthcare professionals, usefulness of the SDM materials, and perception of different NOACs. During Stage II, part I participants after being prescribed NOACs, and part II participants completed another survey to compare impacts of SDM.

RESULTS:

During Stage I, dabigatran was the preferred NOAC after viewing the SDM materials among 90% of part I participants. During Stage II, both part I (n = 87) and part II participants (n = 104) completed another survey. Fewer part I participants were anxious about AF (p < 0.01), and more had confidence in healthcare professionals (p < 0.01) after viewing SDM materials than before. Most part I participants (≥90%) rated the SDM materials as "very helpful". In Stage II, participants viewing SDM before initiating dabigatran had lower anxiety (part I, 43%; part II, 53%; p < 0.01) and a higher trust (part I, 92%; part II, 84%; p < 0.01).

CONCLUSION:

In conclusion, SDM reduced anxiety and improved trust in healthcare professionals among NOAC-naïve participants with AF.
