Background:

The substance use crisis continues to progress. Medication for Opioid Use Disorder (MOUD) are prescribed to reduce opioid use and related harms; however, many individuals continue to use substances while on treatment. The objective of this study was to describe the temporal and demographic trends of the agreement between self-reported and urine tested substances.

Methods:

The current study is a retrospective secondary analysis of three phases of a prospective cohort study (Pilot 2011, Genetics of opioid addiction (GENOA) 2013-2017, and Pharmacogenetics of opioid substitution treatment (POST)) 2018-2022) spanning 2011-2022. We compared the self-reported substance use data for opioids, benzodiazepines, amphetamine/methamphetamine (AMP/MET), and cocaine with urine drug results. We compared the positive predictive value (PPV), false omission rate (FOR), sensitivity, and specificity between (i) different drugs; (ii) by sex, and (iii) age group at enrollment in each phase of the study using self-reported substance use at baseline and retrospective electronic health record data on urine drug screenings collected over the same time period.

Results:

Overall, the average PPV and FOR for any drug across all phases was 80.7 % and 37.9 %, respectively. Sensitivity and specificity were highest for cocaine and lowest for benzodiazepines. We found no specific trend by sex. Lastly, we found a higher sensitivity for opioids and AMP/MET in those under 25 years of age compared to other age groups. PPV increased over time for benzodiazepines, AMP/MET and cocaine and FOR was higher during the pilot and POST phases than the GENOA phase.

Conclusion:

Our study highlights the unique challenges associated with ascertaining substance use behaviour for individuals receiving MOUD, indicating many patients will accurately report substance use while others do not. It is therefore important to consider the context of the patient, and the type of the co-substance used to select patient-centred testing as indicated. Therefore, the answer to the question of do we need urine drug screen is yes in some cases.

Methamphetamine-associated heart failure: Clinical characteristics and outcomes in a safety net population

BACKGROUND:

Methamphetamine use is increasing and is associated with development of heart failure (HF). However, clinical characteristics and outcomes have not been well-described.

OBJECTIVE:

To compare outcomes among individuals with HF with and without methamphetamine use in a safety-net setting.

METHODS:

This retrospective matched cohort study included individuals with HF with history of methamphetamine use and age, gender-, and year-matched controls without history of methamphetamine use in a municipal health system from 2001 to 2019. One thousand seven hundred seventy-one individuals with methamphetamine use and HF and 3,542 age, sex, and year-of-HF-diagnosis matched controls with HF without methamphetamine use were included. The primary outcome was all-cause mortality. Secondary outcomes included time to HF hospitalization, 30-day, 90-day, and one-year HF, and all-cause readmissions.

RESULTS:

The median age of the cohort was 52.1 years and 22.6 % were female. Black/African American was the most common racial identity (methamphetamine: 49.1 %; no methamphetamine: 33 %). There was no significant difference in mortality between groups (40% vs 36.6 %,HR 1.00,95 % CI 0.91,1.10,p = 1.00). A subset had an index HF hospitalization (n = 1,404, 26.4 %) during the study period, including 637 (35.9 %) with history of methamphetamine use and 767 (21.7 %) without history of methamphetamine use (relative risk 1.66,95 % CI 1.52-1.81,p < 0.0001). Among those who were ever hospitalized for HF, individuals with methamphetamine use had increased risk of HF and all-cause readmission at 30 days (RR 1.92,95 % CI 1.36-2.70,p < 0.001), 90 days (RR 1.69,95 % CI 1.35-2.12,p < 0.001), and one year (RR 1.61,95 % CI 1.36-1.91,p < 0.001).

CONCLUSION:

Despite having higher all-cause and HF readmission risk, individuals with methamphetamine-associated HF did not have higher mortality risk.

A prospective, historical-controlled evaluation of oliceridine for moderate or severe pain in patients with acute burn injuries (RELIEVE)

Oliceridine, a biased, selective opioid agonist, has shown a 3-fold preferential activation of the G-protein (i.e., analgesia) over β-arrestin pathway. β-arrestin activation is believed to be associated with higher adverse events, such as constipation, respiratory depression, and desensitization. There is no literature of use in patients with burn injuries. We hypothesized the use of oliceridine would provide adequate and safe analgesia after acute burn injury. Ten patients received oliceridine as their sole opioid for up to 7 days, which was compared to a random, historical cohort receiving standard of care (i.e, fentanyl, oxycodone, hydromorphone, and morphine). The historical control group was initially matched 2:1 (though 2 patients were ultimately excluded) with the oliceridine group according to age, percent total body surface area burned (TBSA), and number of operations. No patient had a history of known opioid, cocaine, or methamphetamine use, as this was an exclusion criterion for the prospectively enrolled group. Baseline numerical rating scale (NRS) was similar for both groups [9 (7.8, 10) vs 9.5 (8.8, 10); p = 0.360). Over the 7-day period, mean daily pain scores significantly decreased in both groups. However, use of oliceridine was associated with a significantly larger decrease in mean pain score [-0.74 (-1.36, -0.12), p = 0.0215]. There was no difference in average daily morphine milligram equivalents (MME) [-14.02 (-67.22, 39.19), p = 0.5939]. There were no unexpected adverse events related to oliceridine. Oliceridine demonstrated significant pain relief, which was maintained over the 7-day study period. The control group demonstrated initial relief, which was not maintained despite similar MME.

U.S. Life Spans Rebound from Pandemic Lows, Helped by Drop in Fatal Overdoses

THURSDAY, Dec. 19, 2024 -- After years of decline, U.S. life expectancy is finally on the rise again for the first time since 2019, new government research shows. This turnaround was driven in part by a decline in drug overdose deaths, which had risen dramatically as a result of the opioid epidemic, according to a second report from the U.S. Centers for Disease Control and Prevention. That includes the first reduction in OD deaths caused by powerful synthetic opioids like fentanyl and tramadol, researchers said. “The age-adjusted rate for drug overdose deaths involving synthetic opioids other than methadone decreased from 2022 to 2023, the first such decrease since the large increases that began in 2013,” concluded the research team led by Matthew Garnett , an injury epidemiologist and statistician with the CDC’s National Center for Health Statistics (NCHS). “The rate also decreased for deaths involving natural and semisynthetic opioids and heroin between 2022 and 2023,” the team added. Life expectancy at birth in 2023 was 78.4 years for the total population, an increase of 0.9 years from 77.5 in 2022, CDC researchers reported. For men, life expectancy increased 1 year, from 74.8 in 2022 to 75.8 in 2023. Meanwhile, women saw a 0.9-year increase, from 80.2 in 2022 to 81.1 in 2023. “From 2022 to 2023, age-adjusted death rates decreased for 9 of the 10 leading causes of death,” wrote the life expectancy research team led by Elizabeth Arias , a health scientist with the NCHS. Deaths due to heart disease, accidental injury, stroke, respiratory disease, Alzheimer’s disease, diabetes, kidney disease, liver disease and COVID-19 all fell significantly between 2022 and 2023, researchers found. The rate of cancer deaths also went down, but not significantly, the report says. The end of the pandemic likely contributed to the increase in life expectancy, researchers said. “The most notable change was for COVID-19, which dropped from the 4th leading cause in 2022 to the 10th leading cause in 2023,” the researchers wrote. Overall, COVID deaths decreased more than 73% between 2022 and 2023, falling from 44.5 deaths to 11.9 deaths per 100,000, researchers said. A decrease in drug OD deaths also contributed to improved life expectancy. The rate of drug overdose deaths fell from 32.6 per 100,000 in 2022 to 31.3 per 100,000 in 2023, researchers said. Drug OD deaths are considered accidental injury deaths. These results jibe with statistics released last week by the CDC, which reported that drug OD deaths had decreased 17% between July 2023 and July 2024. However, the OD death rate is still 3.5 times what it was in 2003, when 8.9 people for every 100,000 died by drug overdose. Drug OD deaths related to synthetic opioids declined by 2.2% between 2022 and 2023, results show -- the first significant decline in OD deaths related to synthetic opioids like fentanyl since 2003, researchers added. Drug overdose deaths related to natural opioids like morphine, oxycodone and hydrocodone also declined 17.1%, between 2022 and 2023, and OD deaths caused by heroin decreased 33.3%. However, OD death rates for cocaine increased by 4.9% between 2022 and 2023, and OD deaths from stimulants like methamphetamine increased by 1.9%, researchers said. Lastly, infants did not seem to benefit from the declining death rates, researchers found. “The infant mortality rate of 560.2 infant deaths per 100,000 live births in 2023 was not significantly different from the rate in 2022,” the researchers wrote. The ten leading causes of infant death in 2023 -- birth defects, low birth weight, sudden infant death syndrome, unintentional injuries, maternal complications, bacterial sepsis of newborn, cord and placental complications, respiratory distress of newborn, intrauterine hypoxia and birth asphyxia, and diseases of the circulatory system -- accounted for more than 65% of all infant deaths, researchers concluded.
