Making causal statements regarding dose-response in treatments for posttraumatic stress disorder (PTSD) and alcohol/other drug use disorders (AODs; PTSD+AOD) is difficult because (a) dosage is rarely randomized and (b) self-selected dosage can be affected by treatment assignment. In the present study, we sought to clarify causal inferences regarding treatment-by-dosage interactions in PTSD+AOD treatment using Project Harmony, an individual-patient meta-analytic data set of behavioral, pharmacological, and combination PTSD+AOD treatments ( k = 36; N = 4,046). Using propensity score weighting and moderated multilevel “net treatment difference” modeling, trauma-focused (TF) treatments, whether integrated or nonintegrated with AOD treatment, outperformed treatment as usual by greater margins on reductions in PTSD and alcohol use as dosage increased. Furthermore, appropriately treating dosage as a posttreatment covariate and moderator revealed effects for TF treatments on drug use that had not been detected in previous studies. Implications for approaches to increasing TF-treatment attendance and greater use of causal-inference methodologies with dose-response analyses are discussed.

Identifying risk factors for disease onset and progression has been a core focus in nephrology research. Mendelian Randomization (MR) has emerged as a powerful genetic epidemiological approach, utilizing genome-wide association studies (GWAS) to establish causal relationships between modifiable risk factors and kidney disease outcomes. MR uses genetic variants as instrumental variables to infer causal relationships between exposures and disease outcomes. This method leverages the natural randomization of genetic variants to balance confounders, akin to matched cohorts in observational research. The rapid increase in MR studies on kidney disease poses challenges for journals and peer reviewers, especially clinicians unfamiliar with the methodology. High-quality MR studies use strong, well-validated genetic instruments with clear biological relevance, thoroughly testing for pleiotropy and confounding factors using methods like MR-Egger. Sensitivity analyses, such as MR-PRESSO, should ensure findings remain consistent across various assumptions. Effect sizes with confidence intervals should be reported and discussed within established biological mechanisms. Additionally, limitations must be transparently addressed, with recommendations for replication in future studies, to strengthen findings. This article guides readers in understanding MR application in nephrology and identifying high-quality MR studies, helping peers avoid pitfalls while seizing new opportunities in advancing kidney disease research.

Egypt, home to over 114 million people, hosts a diverse and expanding nephrology landscape, anchored by leading academic institutions and tertiary care centers that provide services across the full spectrum of kidney disease-from acute kidney injury (AKI) and chronic kidney disease (CKD) to dialysis and transplantation. Some centers have earned international recognition for their scientific contributions and attract referrals from across the region. Notwithstanding the range of services offered, the Egyptian healthcare system-which includes insurance-based, commercial, and governmental models-faces significant obstacles, such as unequal access to care, a lack of adequate data infrastructure, and resource limitations. This narrative review, authored by a multidisciplinary team of Egyptian nephrology experts, provides a comprehensive overview of nephrology care in Egypt, highlighting current strengths, identifying key challenges, and offering future-oriented solutions to advance kidney health nationwide. It aims to serve as a roadmap for policymakers, stakeholders, clinicians, and researchers committed to improving nephrology care delivery in Egypt and shaping a more equitable, enhanced, and sustainable future.
