Fecal Microbiota Transplantation as a Strategy to Eradicate Intestinal Carriage of Resistant Organisms

This protocol will evaluate fecal microbiota transplantation (FMT) as a strategy to eradicate intestinal colonization of extended-spectrum resistant (ESC-R) Enterobacteriaceae in pediatric patients.
FMT will be performed on subjects with a history of at least one infection due to ESC-R Enterobacteriaceae.
This protocol aims to determine the feasibility, safety, tolerability, and potential efficacy of FMT in pediatric patients with a history of ESC-R Enterobacteriaceae.

Prediction model, risk factor score and ventilator-associated pneumonia: A two-stage case-control study

BACKGROUNDVentilator-associated pneumonia (VAP) is one of the most important hospital acquired infections in patients requiring mechanical ventilation (MV) in the intensive care unit, but the effective and robust predictable tools for VAP prevention were relatively lacked.METHODSThis study aimed to establish a weighted risk scoring system to examine VAP risk among a two-stage VAP case-control study, and to evaluate the diagnostic performance of risk factor score (RFS) for VAP. We constructed a prediction model by least absolute shrinkage and selection operator (LASSO), random forest (RF), and extreme gradient boosting (XGBoost) models in 363 patients and 363 controls, and weighted RFS was calculated based on significant predictors. Finally, the diagnostic performance of the RFS was testified and further validated in another 177 pairs of VAP case-control study.RESULTSLASSO, RF and XGBoost consistently revealed significant associations of length of stay before MV, MV time, surgery, tracheotomy, multiple drug resistant organism infection, C-reactive protein, PaO₂, and APACHE II score with VAP. RFS was significantly linearly associated with VAP risk [odds ratio and 95 % confidence interval = 2.699 (2.347, 3.135)], and showed good discriminations for VAP both in discovery stage [area under the curve (AUC) = 0.857] and validation stage (AUC = 0.879).CONCLUSIONSResults of this study revealed co-occurrence of multiple predictors for VAP risk. The risk factor scoring system proposed is a potentially useful predictive tool for clinical targets for VAP prevention.

Preparation Times and Estimated Costs for Vancomycin Formulations: Does the Difference Matter?

Objective Infections from methicillin-resistant Staphylococcus aureus are increasingly treated in longterm care facilities, but long-term care pharmacies face high costs in the provision of sterile vancomycin for intravenous administration. This study compares pharmaceutical costs of outsourced, compounded, and room temperature premixed vancomycin formulations in a long-term care pharmacy. Design This retrospective observational study reviewed 124 orders of vancomycin. Means for total pharmacy preparation time, pharmacist labor time, and extrapolated time over complete course of treatment were compared for three vancomycin preparations: outsourced, compounded by pharmacy, and room temperature premixed vancomycin formulations. Cost calculations were generated using ingredient costs as reported by the pharmacy and median pharmacist labor costs as published from national sources. Results Mean total preparation times and pharmacist preparation times were shortest for premixed vancomycin. Over full courses of treatment, mean pharmacy preparation time for compounded was 5 hours 3 minutes (mean of 28 treatments) and 2 hours 8 minutes for premixed (mean of 54 treatments). Data on pharmacist time in outsourced orders were not available. Total pharmacy costs were $993.94 for compounded vancomycin, $2220.34 for outsourced, and $809.36 for room temperature premixed vancomycin. Conclusion There were reduced preparation times for room temperature premixed vancomycin compared with compounded and outsourced formulations for skilled nursing facilities. As multiple drug-resistant organism infections are increasingly treated in long-term care, finding cost-effective approaches to medication provision from pharmacies is critical.

Computational Study of Antimicrobial Peptides for Promising Therapeutic Applications Against Methicillin-resistant Staphylococcus aureus

Background:Methicillin-resistant Staphylococcus aureus (MRSA) is a causative agent for multiple drug-resistant diseases and is a prime health concern. Currently, antibiotics like vancomycin, daptomycin, fluoroquinolones, linezolid, fifth-generation cephalosporin and others are available in the market for the treatment of MRSA infection.Methods:With the increasing prevalence of drug-resistant cases, researchers are actively investigating alternative strategies to combat MRSA, including the exploration of peptide therapeutics. This study employed computational methods to prospect for potential Antimicrobial Peptides (AMPs).Results:A total of One hundred and fifty antimicrobial peptides were explored based on physicochemical properties. The results showed that Clavanin B was the most appropriate candidate. Molecular Docking and Molecular Dynamics Simulation results showed the protein-peptide interaction of the MRSA target proteins, Penicillin Binding Protein 2a and Panton-Valentine Leukocidin Toxin, with the Antimicrobial Peptide Clavanin B.Conclusion:Currently, the antimicrobial peptide database highlights Clavanin B's role as an anti-HIV peptide. Moreover, this investigation proposes Clavanin B as a viable repurposed drug for treating MRSA, underscoring its potential deployment in the management of MRSA infections.
