The findings demonstrate the highest complete response rate without reinduction with more than half of responders remaining cancer-free for at least 12 months. These results highlight the potential of TAR-200 as a breakthrough for people with Bacillus Calmette-Guérin (BCG)-unresponsive, high-risk non-muscle-invasive bladder cancer (HR-NMIBC) with carcinoma in situ (CIS), with or without papillary tumors who are ineligible or refuse radical cystectomy (RC). As of March 2025, 82.4 percent of the 85 enrolled patients in the study achieved a complete response (CR) (95 percent confidence interval [CI], 72.6-89.8), meaning their cancer was undetectable following treatment. This high response rate translated into sustained disease control, with 52.9 percent of responders maintaining complete response at one year. The median duration of response (DOR) was 25.8 months (95 percent CI, 8.3-not estimable), indicating that many patients remained cancer-free for over two years without the need for reinduction therapy. At 12 months, 86.6 percent (95 percent CI, 76.6-92.6) of responders remained cystectomy-free. Importantly, the treatment was well-tolerated, with most adverse events being mild urinary symptoms. These findings show that TAR-200 offers a highly effective and durable treatment option for patients with certain types of BCG-unresponsive HR-NMIBC. Most treatment-related adverse events (TRAEs) were mild and manageable. Overall, 71 patients (83.5 percent) experienced TRAEs, the majority of which were low-grade urinary symptoms, such as bladder irritation or discomfort. Eleven patients (12.9 percent) experienced Grade 3 or higher TRAEs, and five patients (5.9 percent) reported serious TRAEs. Only three patients (3.5 percent) discontinued treatment due to TRAEs, and there were no treatment-related deaths. TAR-200 is inserted directly into the bladder by a healthcare professional in a brief outpatient procedure, without the need for anesthesia. Designed to remain in the bladder, it does not interfere with daily activities and provides sustained release of treatment throughout the day. Johnson & Johnson said its drug-device combo for bladder cancer achieved a high rate of complete responses lasting at least a year, according to the latest data cut from a registrational trial. Almost 45% of patients treated with TAR-200 achieved CR and maintained it for at least 12 months, according to results presented Saturday at the American Urological Association annual meeting in Las Vegas. The data came from the second cohort of the Phase 2b SunRISe-1 trial, which enrolled 85 people with high-risk non-muscle-invasive bladder cancer with carcinoma in situ who were unresponsive to the standard of care Bacillus Calmette-Guérin (BCG) vaccine. J&J’s results should strengthen TAR-200’s case in front of regulators after it filed an NDA in January through the Real-Time Oncology Review program, which allows the FDA to assess certain efficacy and safety data before an NDA is formally submitted. The asset has the potential to be “one of our best-selling drugs ever,” head of global oncology commercial strategy Biljana Naumovic told Endpoints News in an interview. TAR-200 delivers low doses of the decades-old chemotherapy drug gemcitabine directly to the bladder through a silicone tube or “pretzel.” J&J filed the NDA based on primary endpoint data from cohort two of SunRISe-1, which showed the treatment achieved a CRR exceeding 80%. J&J also shared data from the fourth cohort of SunRISe-1 at the confab on Saturday. That cohort enrolled 52 high-risk non-muscle-invasive bladder cancer patients with “papillary disease-only,” meaning they presented with mushroom-shaped protrusions in their bladder lining. The patients received initial treatment but their cancer returned, after which the growths were scraped or burned away. In cohort four, the goal of TAR-200 is to stop the disease from coming back, Mark Wildgust, J&J’s head of global medical affairs for oncology, said in the joint interview. And the candidate achieved a disease-free survival rate of 81.1% at nine months. Wildgust said that the best data for these patients is “somewhere between 45 to 55%” being disease free at one year with current standard of care or investigational agents. He said TAR-200’s results were “really remarkable” in that context. J&J is currently enrolling for a Phase 3 trial of the asset in papillary-disease only patients called SunRISe-5. The pharma company obtained TAR-200 and the pretzel delivery technology when it acquired Taris Biomedical back in 2019 in a deal with undisclosed sums. J&J says the technology has peak sales potential of at least $5 billion, although it hasn’t split this figure over different programs. For now, the drugmaker is focusing on readying itself for a potential launch of TAR-200, which is inserted as part of a two-minute procedure called a cystoscopy that is routinely performed by urologists. “This device has been made by urologists for urologists, and this is how we’re going to be introducing it to the US market,” Naumovic said.
