Introduction

A form of paralytic poliomyelitis affecting neurons of the MEDULLA OBLONGATA of the brain stem. Clinical features include impaired respiration, HYPERTENSION, alterations of vasomotor control, and dysphagia. Weakness and atrophy of the limbs and trunk due to spinal cord involvement is usually associated. (From Adams et al., Principles of Neurology, 6th ed, p765)

Human enteroviruses and the long road to acute flacid paralysis eradication

AbstractEnteroviruses (EVs) are a highly diverse group of viruses multiplying primarily in the gastrointestinal tract and/or the upper respiratory tract, initially distributed in two separate genera: Enterovirus and Rhinovirus, respectively. According to the similarities in genome organization and particle structure, rhinovirus species were later reclassified as also belonging to genus Enterovirus. Human EV infections are usually asymptomatic or causing mild clinical manifestations. Nevertheless, some EV infections may derive in severe neural complications, including acute flaccid paralysis (AFP) such as poliomyelitis, whose etiological agent is poliovirus, a member of the Enterovirus C species. The inactivated polio vaccine (IPV) and particularly the oral attenuated polio vaccine (OPV) have contributed to the virtual eradication of the disease. However, sustained global circulation of vaccine-derived poliovirus 2 (cVDPV2), originated from the genetic instability of OPV strain 2 and intertypic recombination between Sabin OPV strains and members of the Enterovirus C species, still causes outbreaks of AFP worldwide. In addition, humanitarian crises, in particular armed conflicts, hamper polio vaccination campaigns and facilitate the occurrence of cases. Additionally, besides poliovirus, other EV may also cause AFP, among them EV A71 or EV D68, and it is highly advisable to implement wastewater surveillance to elucidate the occurrence of not only polioviruses, but also of other EV susceptible to derive in serious neural complications, since the screening of viral RNA in cerebrospinal fluid samples in patients suffering from AFP is not a reliable diagnostic tool.

IntroductionAcute flaccid paralysis (AFP) caused by acute anterior poliomyelitis (AAP) remains endemic in some Asian and African countries, particularly Niger. Performance indicators defined by the WHO are used to evaluate the AFP surveillance system in affected countries. The objective of this study is to assess its outcomes in Niger from 1998 to 2021. The objective of this study is to assess its outcomes in Niger from 1998 to 2021.MethodologyThis is a cross-sectional study based on all secondary data from AFP case surveillance reported to the Directorate of Surveillance and Epidemic Response in Niger, over a 24-year period from 1998 to 2021.ResultsThe sample included 9,659 patients of all ages and sexes. The sex ratio was 1.23, and 92.01% of patients were under 5 years old. The Maradi region ranked first among Niger's eight regions in terms of reported cases (32.27%). It hosts a large portion of the population from neighboring Nigeria, a country also endemic for AFP. More than half of the patients (66.59%) had received 1 to 10 doses of oral polio vaccine. From 1998 to 2021, 8,419 stool samples from 9,494 cases (88.70%) were usable upon arrival at the laboratory. Confirmed and compatible poliovirus cases accounted for 0.80% and 2.35%, respectively. Of the 276 patients who presented with paralysis, 71.73% suffered from paralysis in both limbs, and in 94.35% of the cases, the paralysis had progressed within 3 days.ConclusionThis study allowed for an analysis of the performance of the active AFP surveillance system. In Niger, this surveillance is acceptable according to performance indicators, but there is still much progress to be made at both the population level and within the healthcare workforce.

Vaccine-associated paralytic poliomyelitis in oral polio vaccine recipients: disproportionality analysis using VAERS and systematic review

BACKGROUNDVaccine-associated paralytic poliomyelitis (VAPP) is a rare adverse event of oral poliovirus vaccines (OPV), particularly affecting immunodeficient individuals.RESEARCH DESIGN AND METHODSThis study aimed to (1) Assess the association between OPV and VAPP using Vaccine Adverse Event Reporting System (VAERS) database (2) Outline patient characteristics and risk factors associated with the occurrence of VAPP in OPV recipients through a systematic review of case reports and case series. A disproportionality analysis was conducted using the data from VAERS, encompassing adverse events reported from 1990 till February 2023. Additionally, we conducted a systematic review of case reports and case series using PubMed, Scopus, and Embase databases.RESULTSThe VAERS data revealed 130 VAPP reports among 1,739,903 OPV linked adverse events, with year 2010 reporting the strongest association. The systematic review of 37 studies highlighted VAPP occurrence within 2 months to 4 years post-vaccination, typically with acute flaccid paralysis. Immunodeficiency and perianal abscess emerged as major risk factors. Out of the 37 included studies, 27 showed consistent causal association of VAPP with OPV using WHO-AEFI causality assessment tool.CONCLUSIONThe study emphasized the seriousness of VAPP and highlights its association with OPV, identifying immunodeficiency as a prominent contributor to VAPP manifestation.
