Introduction

Scaly papule or warty growth, caused by five fungi, that spreads as a result of satellite lesions affecting the foot or leg. The extremity may become swollen and, at its distal portion, covered with various nodular, tumorous, verrucous lesions that resemble cauliflower. In rare instances, the disease may begin on the hand or wrist and involve the entire upper extremity. (Arnold, Odom, and James, Andrew's Diseases of the Skin, 8th ed, p362)

Retrospective Descriptive Observational Study of the Epidemiological, Clinical and Therapeutic Profile of Patients With Rare Infections of Dermatological Interest at Three Major Reference Hospitals in Costa Rica From 2019-2023

In Costa Rica there are some descriptive studies of some of these rare infectious diseases (sporotrichosis, chromomycosis and mycetomas); however, there are no recent published reports. Most of the publications on the subject date back to the previous century; from 2000 to the present year there are only publications of small series of cases, so the current behavior of these skin infections is unknown. The primary objective is to analyze the epidemiological, clinical, and therapeutic profile and determine the risk factors in patients with a diagnosis of rare infections of dermatological interest treated at Hospital México, Hospital Rafael Calderón Guardia, and Hospital San Juan de Dios during the period 2019-2023.

Posaconazole effectiveness in rare invasive fungal infections: A systematic literature review

INTRODUCTIONMucormycosis, hyalohyphomycosis, chromoblastomycosis, and fungal mycetoma are rare invasive fungal infections (IFIs) that cause significant morbidity and mortality in immunocompromised patients. Few effective treatment options are available for these IFIs.METHODSWe performed a systematic literature review of MEDLINE and Embase to identify studies published from 2005 (year of posaconazole approval) to 22 October 2022, reporting the efficacy/effectiveness of posaconazole monotherapy or combination therapy for treating mucormycosis, hyalohyphomycosis, chromoblastomycosis, and mycetoma. Positive outcomes or positive clinical outcomes were defined as reporting of a positive efficacy/effectiveness measure (i.e. no relapse, response, cure, radiological improvement, clinical/symptom improvement, or survived therapy).RESULTSOf 3207 articles identified (after removing duplicates), 533 articles (mostly case reports) were included. Positive clinical outcomes with posaconazole therapy were observed in most patients with mucormycosis (74.8%, 1197/1601), hyalohyphomycosis (58.5%, 62/106), chromoblastomycosis (90.5%, 19/21), and mycetoma (100%, 5/5). Overall survival was around 70% or greater across the IFIs examined. Positive response was higher in second-line monotherapy than first-line monotherapy in mucormycosis and chromoblastomycosis. Higher mortality was observed with combination therapy than monotherapy in mucormycosis and hyalohyphomycosis infections (except for first-line use in mucormycosis). Positive clinical outcome was 78.6% and overall survival was 78.6% in 323 coronavirus disease-associated mucormycosis infection cases.CONCLUSIONSDespite the rarity of these IFIs, substantial data have been published since posaconazole was initially approved in 2005, and the real-world case reports demonstrate that posaconazole is an effective therapeutic option alone or in combination for the treatment of these rare IFIs.

A 19-year-old male from Odisha state was diagnosed with facial chromoblastomycosis caused by Fonsecaea nubica, initially misidentified as F. pedrosoi. Sequencing of Internal transcribed spacer region of rDNA confirmed F. nubica, marking its first detection in India. This highlights the need for further research and monitoring of epidemiology of chromoblastomycosis.

The role of melanin in Fonsecaea monophora pathogenicity: adhesion, fibroblast interactions, and implications for chromoblastomycosis

AIMSFonsecaea monophora is a pathogen of chromoblastomycosis (CBM), and melanin is one of the important virulence factors of F.monophora. Despite the crucial role fibroblasts play in inflammatory diseases, the interaction between fibroblasts and F.monophora remains poorly understood. This study investigated fibroblast responses to wild-type (WT) and polyketide synthase A gene knockout (ΔpksA) strains, aiming to elucidate the role of fungal melanin in adhesion mechanisms and the interactions between fibroblasts and pathogens.MATERIALS & METHODSCoculture systems of fibroblasts with WT and ΔpksA strains of F.monophora were established. Adhesion and ectophosphatase activity assays evaluated fungal attachment. Cell scratch and Transwell migration assays assessed fibroblast motility. RT-qPCR and ELISA quantified collagen synthesis, myofibroblast differentiation, and apoptosis-related gene expression.RESULTS AND CONCLUSIONSThe WT strain exhibited greater adhesion and inhibited fibroblast migration, whereas the ΔpksA strain showed reduced adhesion and enhanced migration, correlating with melanin content and ectophosphatase activity. Both strains inhibited fibroblast-to-myofibroblast differentiation, collagen synthesis, and extracellular matrix deposition, contributing to tissue remodeling. These findings highlight melanin's role in F. monophora's pathogenicity and its disruption of fibroblast function, potentially delaying wound healing in CBM.
