Randomized Phase II Trial Evaluating Compliance With Monitoring Conducted Either by a Hospital-based Physician in Person or by a Nurse in Remote Monitoring

This is a multicenter, interventional, randomized study among adult patients recently diagnosed with a rare tumor (<12 months). The study will aim to compare compliance with the personalized post-treatment surveillance plan, established for each patient according to national guidelines, when the surveillance is conducted in person by a hospital-based physician (control arm) or remotely by a trained nurse (experimental arm).

RAISN - Robot-assisted ICG-guided Sentinel Node Biopsy in Testicular Cancer

Robot-assisted image-guided sentinel lymph node biopsy (RAISN) in testicular cancer is a novel technique that has not been widely investigated yet. This technique is promising and could be implemented as a future standard in the primary diagnostic work up of clinical stage (CS) I testicular cancer. Current staging strategies have a poor predictive accuracy for occult metastatic disease.
So far, feasibility studies used 99mTC-nanocolloid staining and laparoscopy and all patients with tumor-positive nodes received adjuvant systemic treatment. The development of a robot-assisted image-guided lymph node resection technique with indocyanine green (ICG) is potentially more precise, easier to apply and widely available. With this new diagnostic approach the management of newly diagnosed testicular cancer patients might be changed dramatically by reducing overtreatment and treatment-related toxicity with a minimally invasive robot-assisted procedure.

A Prospective Program Aiming at Improving Outcome for Young Adults With Poor-prognosis

This is a prospective multicenter, non-randomized research program that includes:
a phase IV study (for all patients) with a collection of tissue specimens of tumor,
a phase II study (for patients with primary mediastinal tumors and an unfavorable decline in tumor markers),
and a diagnostic study (for all patients, except patients with brain metastases at baseline or patients for whom any brain MRI is contra-indicated).
The main question it aims to answer is improving outcome for young adults with poor-prognosis Non Seminomatous Germ Cell Tumor (NSGCT) is to validate prospectively the efficacy and safety of a personalized treatment based on early tumor marker kinetic assessment in real life for patients with poor-prognosis NSGCT.
Participants will be followed-up according to the assessment of decline kinetics of the tumor markers at the end of a first chemotherapy cycle and according to the localisation of the primary lesion if unfavorable.
In the case of a patient with a favorable decline of the tumor markers, he will be treated by 3 additional standard chemotherapy cycles.
In the case of a patient with a testicular or peritoneal primary tumor and an unfavorable decline of the tumor markers, the patient will be treated by a dose-dense standard therapy.
The patient with a mediastinal primary tumor and an unfavorable decline of the tumor markers will be proposed to enter the phase II part of the study or to enter the dose-dense regimen like the other primary localisations. If the patient consents and is eligible for phase II part, he will undergo either an early surgery if feasible or a high-dose chemotherapy if the early surgery is not possible.

ABSTRACTObjectiveThis study investigated the perioperative and oncological outcomes of laparoscopic retroperitoneal lymph node dissection (RPLND) procedures for post‐chemotherapy patients with nonseminomatous testicular germ‐cell tumor at a single center.MethodsThis study included patients with nonseminomatous testicular cancer who underwent RPLND after chemotherapy at the Kyushu University Hospital between 2016 and 2024. The preoperative clinicopathological characteristics, perioperative outcomes, and oncological outcomes were investigated.ResultsA total of 13 patients underwent laparoscopic RPLND. Median maximum retroperitoneal tumor size at post‐chemotherapy before RPLND was 11 mm (range, 2–30 mm). RPLND template was one side and both sides in nine and four patients. Median operative time was 272 min (range, 129–490 min), and median estimated blood loss was 27 mL (range, 0–100 mL). Median time from operation to discharge was 8 days (range, 5–15 days). There was no severe perioperative and postoperative complication. Residual cancer and teratoma were detected in one and seven patients. During median follow‐up of 18.6 months (range, 1.0–95.7 months), no case presented recurrence.ConclusionLaparoscopic RPLND presented safety in perioperative outcomes and favorable oncological outcomes. Thus, it was confirmed that laparoscopic RPLND is a feasible minimally invasive procedure for selected cases.

Germ cell neoplasms of the testis are rare solid tumors predominantly in young men. Seminomas are slightly more frequent than nonseminomatous germ cell tumors. A special feature of seminomas is that they are sensitive to radiation, so that this represents an option in tumor stages with few metastases; however, the guideline recommendation is cautious due to the increased risk of secondary malignancies. In nonmetastasized tumor stages active surveillance is the primary approach to avoid overtreatment of patients. This is also the reason for primary nerve-sparing retroperitoneal lymph node dissection in cases of a low metastasis load. This concept has already been implemented in the American Urological Association (AUA) and National Comprehensive Cancer Network (NCCN) guidelines, whereas in the European Association of Urology (EAU) guidelines it is still considered to be an individual approach.

Intracranial nongerminomatous germ cell tumors (NGGCTs) require accurate diagnosis to guide intensive treatment protocols due to their poorer outcomes compared to pure germinomas.Here, we report the case of a 12-yr-old male presenting with a pineal mass and clin. features consistent with Parinaud′s syndrome.Initial serum tumor markers were within non-secreting ranges, and histol. anal. suggested a pure germinoma.However, repeat testing revealed elevated alpha-fetoprotein and human chorionic gonadotropin levels, prompting a diagnosis of NGGCT and initiation of dose-intense chemotherapy followed by radiotherapy.Whole-genome sequencing (WGS) of the tumor demonstrated whole-genome duplication and 12p gain, confirming the malignant nature of the tumor, and a pathogenic germline PTPN11 variant consistent with Noonan syndrome (NS).This diagnosis was later confirmed in the patient′s mother. The case emphasizes the importance of serial tumor marker testing in suspected non-secreting germinomas and the role of WGS in uncovering genetic predispositions.This is the first report of a GCT associated with NS, highlighting potential links between the RAS/MAPK pathway and NGGCT pathogenesis.Our findings underscore the value of integrating genomic testing into diagnostic workflows for pediatric oncol., particularly in cases with atypical presentations.

AUA Releases Amendment to the Diagnosis and Treatment of Early-Stage Testicular Cancer Guideline

BALTIMORE, Sept. 14, 2023 /PRNewswire/ -- Today the American Urological Association (AUA) released the 2023 clinical practice guideline amendment for the diagnosis and treatment of early-stage testicular cancer. Testicular cancer is the most common solid malignancy in young males and is relatively rare, with outcomes defined by specific cancer and patient-related factors. Many men with testis cancer have low-stage disease and survival rates are high with standard therapy. A priority for those patients with low-stage disease is limiting the burden of therapy and treatment-related toxicity without compromising cancer control. This guideline was reviewed through the AUA Update Literature Review (ULR) process and subsequently amended based on the availability of new literature, including: New studies included to support the role of MRI in surveillance/staging of patients with germ cell tumors. Retroperitoneal lymph node dissection added as a treatment option for patients with stage IIA or IIB seminoma with a lymph node ≤3cm. Updated text to include long-term follow up for 2 cycles of etoposide and cisplatin chemotherapy without bleomycin for patients with non-seminoma germ cell tumors who have pathological stage II disease that is not pure teratoma. New evidence added to support surveillance imaging for patients with stage I testicular cancer. Updated statement and expanded text on survivorship.
