Introduction

Common occlusive arterial disease which is caused by ATHEROSCLEROSIS. It is characterized by lesions in the innermost layer (ARTERIAL INTIMA) of arteries including the AORTA and its branches to the extremities. Risk factors include smoking, HYPERLIPIDEMIA, and HYPERTENSION.

A single-center, randomized, open, single-dose, two-period crossover, fasting and four-period repeated crossover, and postprandial administration design was used to compare the differences in absorption extent and rate between pentoxifylline sustained-release tablets (400 mg) produced by Zhejiang Proconyu Pharmaceutical Co., Ltd. and pentoxifylline sustained-release tablets (400 mg; trade name: Trental®) certified by Sanofi-Aventis Deutschland GmbH in healthy Chinese people under fasting and postprandial administration conditions, and to evaluate the safety of pentoxifylline sustained-release tablets (400 mg) produced by Zhejiang Proconyu Pharmaceutical Co., Ltd.

Therapeutic Erythrocytapheresis is Effective in Treating 2 Patients with High-Altitude Erythrocythemia

BACKGROUNDDue to environmental hypoxia on the high-altitude local residents often exhibit a compensative increase in hemoglobin concentration to maintain the body's oxygen supply. In certain people, the number of red blood cells continues to grow, resulting in high altitude polycythemia (HAPC) which is characterized by headache, disorientation, sleeplessness, and bone discomfort. HAPC is often associated with multiple complications, of which lower extremity arteriosclerosis obliterans (LEASO) is rare.METHODSHere we report two long-term Tibetan male patients living in plateau areas, who were diagnosed with HAPC complicated with LEASO. The diagnostic standards for HAPC are based on the Qinghai score, which is that the patient currently or once lived in high-altitude areas, and the results of conventional blood tests show that hemoglobin Hb > 21 g/dL in males and Hb > 19 g/dL in females. Due to severe complications caused by abnor-mally high hemoglobin levels, they underwent an individualized regimen of therapeutic erythrocytapheresis (TE).RESULTSAfter one TE treatment, both hemoglobin and hematocrit decreased significantly in 2 patients (Hb decreased from 20 g/dL and 21 g/dL to 17.6 g/dL and 18 g/dL in the two patients), alleviating the further deterioration of the disease caused by abnormal erythrocyte elevation.CONCLUSIONSTE can reduce the hemoglobin and hematocrit of HAPC patients in a timely and effective manner and play a clinical role in alleviating the disease progression of patients to a certain extent, especially for patients with complications. TE needs to be implemented in time.

CircHAT1 regulates the proliferation and phenotype switch of vascular smooth muscle cells in lower extremity arteriosclerosis obliterans through targeting SFRS1

This study aimed to decipher the mechanism of circular ribonucleic acids (circRNAs) in lower extremity arteriosclerosis obliterans (LEASO). First, bioinformatics analysis was performed for screening significantly down-regulated cardiac specific circRNA-circHAT1 in LEASO. The expression of circHAT1 in LEASO clinical samples was detected by quantitative real-time polymerase chain reaction (qRT-PCR). The protein expression of splicing factor arginine/serine-rich 1 (SFRS1), α-smooth muscle actin (α-SMA), Calponin (CNN1), cyclin D1 (CNND1) and smooth muscle myosin heavy chain 11 (SMHC) in vascular smooth muscle cells (VSMCs) was detected by Western blotting. Cell Counting Kit-8 (CCK-8), 5-ethynyl-2'-deoxyuridine (EdU) and Transwell assays were used to evaluate cell proliferation and migration, respectively. RNA immunoprecipitation (RNA-IP) and RNA pulldown verified the interaction between SFRS1 and circHAT1. By reanalyzing the dataset GSE77278, circHAT1 related to VSMC phenotype conversion was screened, and circHAT1 was found to be significantly reduced in peripheral blood mononuclear cells (PBMCs) of LEASO patients compared with healthy controls. Knockdown of circHAT1 significantly promoted the proliferation and migration of VSMC cells and decreased the expression levels of contractile markers. However, overexpression of circHAT1 induced the opposite cell phenotype and promoted the transformation of VSMCs from synthetic to contractile. Besides, overexpression of circHAT1 inhibited platelet-derived growth factor-BB (PDGF-BB)-induced phenotype switch of VSMC cells. Mechanistically, SFRS1 is a direct target of circHAT1 to mediate phenotype switch, proliferation and migration of VSMCs. Overall, circHAT1 regulates SFRS1 to inhibit the cell proliferation, migration and phenotype switch of VSMCs, suggesting that it may be a potential therapeutic target for LEASO.

Clinical efficacy evaluation and safety study of Qishen Tongmai Yin I combined with moxibustion therapy on ASO: A retrospective study

This study aimed to assess the therapeutic efficacy of moxibustion assisted therapy in combination with Qishen Tongmai Yin No. 1 on arteriosclerosis obliterans (ASO) patients. The impact on clinical symptoms, sign scores, foot dorsum temperature, foot dorsum artery blood flow, and overall clinical efficacy were analyzed. Sixty-three ASO patients from Beijing Xiaotangshan Hospital and Beijing Shijitan Hospital (January 2020–December 2021) were divided into 3 groups: a Qi Shen Tong Mai Yin I-only group, a moxibustion-only group, and a combined treatment group receiving both moxibustion and Qi Shen Tong Mai Yin No. 1. The treatment group took Qi Shen Tong Mai Yin No. 1 orally 3 times daily for 1 month. Moxibustion, employing a mild method, was applied once daily for 35 minutes over a month. Key acupoints included bilateral kidney Shu, Guan Yuan, Ming Men, Zu San Li, Wei Zhong, San Yin Jiao, and Yao Yang Guan. Posttreatment, clinical symptom scores, and dorsum foot parameters were measured, including foot temperature and dorsum foot artery blood flow. Total effective and cure rates were calculated. Pretreatment, no significant differences existed among the 3 groups (P > .05). Posttreatment, the clinical efficacy, dorsalis pedis temperature, dorsalis pedis artery blood flow, and symptom scores significantly improved in all groups (P < .05). Differences in these indicators were also significant among the 3 groups (P < .05). Moxibustion assisted therapy combined with Qishen Tongmai Yin No. 1 enhances total effective and cure rates in ASO patients, improving foot temperature, blood flow, microcirculation, and overall clinical efficacy. Additionally, this combination reduces scores for intermittent claudication, pain, skin temperature, soreness, skin color, and numbness, mitigating disease progression. Safety evaluation indicated no adverse events during treatment, demonstrating the safety of the combined therapy.
