Immediate and Prolonged Effects of Thai Foot Massage on Pain, Range of Motion and Somatosensory Reception Among Patients With Type 2 Diabetes Peripheral Neuropathy.

The goal of this clinical trial is to examine the effect of Thai-Foot Massage on type 2 diabetes peripheral neuropathy. The main question is to answer the role of massage in improving peripheral neuropathy. Thai Foot Massage will be administered by the researcher every alternate day for 15 days. Researchers will compare the intervention group with the control group (without intervention) to see the improvement in Somatosensory Reception, Range of Motion, and peripheral neuropathy-induced Pain.

Application value of strain elastography and shear wave elastography in patients with type 2 diabetic peripheral neuropathy: a prospective observational study

AbstractObjectiveTo evaluate the value of conventional ultrasound (US), strain elastography (SE), and shear wave elastography (SWE) in detecting diabetic peripheral neuropathy (DPN) of the tibial nerve (TN), and to establish a predictive model for the diagnosis of DPN.MethodsA total of 32 healthy participants, 34 diabetic patients without DPN, and 36 diabetic patients with DPN were recruited for this study. The TN at the ankle and popliteal fossa were selected for examination. US was used to measure the cross-sectional area (CSA) and perimeter of the TN. Additionally, SE was used to measure the strain ratio (SR) between the TN and the surrounding adipose tissue, and SWE was used to measure the shear wave velocity (SWV) of the TN.ResultsThe CSA, perimeter, SR, and SWV of the TN at the ankle were significantly higher in the DPN group compared to both the Non-DPN group and control group (P < .05). Similarly, the TN at the popliteal fossa showed these differences. At the ankle, the CSA, perimeter, SR, and SWV of the TN in patients without DPN were significantly higher than those in the control group (P < .05). At the popliteal fossa, the SR and SWV of the TN in patients without DPN were significantly higher than those in the control group (P < .05). However, the CSA and perimeter of the TN in patients without DPN did not show a statistically significant difference compared to the control group. The area under the curve (AUC) for the diagnosis of DPN using SWE is significantly greater than that of SE and US.ConclusionUS, SE, and SWE could be used to diagnose DPN, and they also have good diagnostic value for sub-clinical DPN. Among these methods, SWE has demonstrated superior diagnostic efficacy. Compared to examining the TN in the popliteal fossa, the ankle level offers a better site for examination.Advances in knowledgeFor diabetic peripheral neuropathy, US, SE, and SWE are all promising diagnostic methods with high clinical utility.

Metabolomics study of serum from patients with type 2 diabetes: Peripheral neuropathy could be associated with sphingosine and phospholipid molecules

AbstractAbnormal lipid metabolism is one of the risk factors for type 2 diabetes mellitus peripheral neuropathy (DPN). This study aimed to determine the differences in lipid metabolism in patients with type 2 diabetes and DPN and the possible pathogenesis caused by this difference. The participants comprised type 2 diabetes mellitus patients with DPN (N = 60) and healthy controls (N = 20). Blood samples were drawn from the participants in the morning in the fasting state, and then changes in serum lipids were explored using targeted metabolomics on the liquid chromatography‐electrospray ionization‐tandem mass spectrometry platform. Among the 1768 differentially abundant lipid metabolites, the results of orthogonal partial least squares‐discriminant analysis combined with random forest analysis showed that the levels of sphingosine (SPH) (d18:0), carnitine 22:1, lysophosphatidylethanolamine (LPE) (18:0/0:0), LPC (16:0/0:0), lysophosphatidylcholine (LPC) (18:1/0:0), LPC (0:0/18:0) and LPE (0:0/18:1) were significantly different between the two groups. Spearman correlation analysis showed that SPH (d18:0), carnitine 22:1, LPE (18:0/0:0), and LPC (0:0/18:0) levels correlated highly with the patients' electromyography results. Kyoto Encyclopedia of Genes and Genomes pathway annotation and enrichment analysis of 538 differentially abundant lipid metabolites revealed that type 2 diabetes mellitus DPN was related to glycerophospholipid metabolism and glycerol metabolism. Our results further identified the dangerous lipid metabolites associated with DPN and abnormal lipid metabolism. The influence of lipid metabolites such as SPH and phospholipid molecules on DPN development in patients with type 2 diabetes mellitus were suggested and the possible pathogenic pathways were clarified, providing new insights into the clinical risk of DPN in patients with type 2 diabetes mellitus.

Leptin deficiency leads to nerve degeneration and impairs axon remyelination by inducing Schwann cell apoptosis and demyelination in type 2 diabetic peripheral neuropathy in rats

Diabetic peripheral neuropathy, characterized by symptoms such as paresthesia, neuropathic pain, and potential lower limb amputation, poses significant clinical management challenges. Recent studies suggest that chronic hyperglycemia-induced Schwann cells (SCs) apoptosis contributes to neurodegeneration and impaired nerve regeneration, but the detailed mechanisms are still unknown. Our study investigated a mixed-sex type 2 diabetes mellitus (T2DM) rat model using leptin knockout (KO) to simulate obesity and diabetes-related conditions. Through extensive assessments, including mechanical allodynia, electrophysiology, and microcirculation analyses, along with myelin degradation studies in KO versus wild-type rats, we focused on apoptosis, autophagy, and SCs dedifferentiation in the sciatic nerve and examined nerve regeneration in KO rats. KO rats exhibited notable reductions in mechanical withdrawal force, prolonged latency, decreased compound muscle action potential (CMAP) amplitude, reduced microcirculation, myelin sheath damage, and increases in apoptosis, autophagy, and SCs dedifferentiation. Moreover, leptin KO was found to impair peripheral nerve regeneration postinjury, as indicated by reduced muscle weight, lower CMAP amplitude, extended latency, and decreased remyelination and SCs density. These findings underscore the effectiveness of the T2DM rat model in clarifying the impact of leptin KO on SCs apoptosis, dedifferentiation, and demyelination, providing valuable insights into new therapeutic avenues for treating T2DM-induced peripheral neuropathy.
