This is an open-label phase 2 study recruiting low, moderate, and high likelihood ACS patients from approximately 60 centers. Patients will be imaged with iodofiltic acid I 123 for the detection of myocardial ischemia. Readers independent of the clinical study centers will review results of imaging studies in a blinded fashion at an imaging core lab. The resulting independent reading of the images will be compared against the truth standard for ACS.

Underrecognized Utility of <sup>123</sup>I-BMIPP in CAD Diagnosis Outside of Japan

¹²³I-BMIPP (Iodine-123 labeled beta-methyl-p-iodophenyl-pentadecanoic acid) is a radiotracer that facilitates non-invasive assessment of myocardial fatty acid metabolism through single photon emission computed tomography imaging. Given that fatty acids serve as one of the primary energy sources for cardiac muscle, reduced uptake of ¹²³I-BMIPP offers valuable insights into the pathophysiology of various cardiac conditions, particularly in coronary artery disease (CAD). Despite its reported efficacy, the use of ¹²³I-BMIPP remains limited outside Japan, primarily due to regulatory and supply challenges. However, in Japan, ¹²³I-BMIPP is clinically utilized for CAD patients with various ischemic conditions as the protocol does not require stress tests or contrast iodine and has a relatively short acquisition time. This review highlights the clinical applications of ¹²³I-BMIPP across various conditions and aims to promote its broader adoption in clinical practice, both in Japan and internationally.

AbstractThe purpose of this practice recommendation is to specifically identify the critical steps involved in performing and interpreting 123I-β-methyl-iodophenyl-pentadecanoic acid (BMIPP) single-photon emission computed tomography (SPECT) and measurement of washout rate (WR) from the heart. This document will cover backgrounds, patient preparation, testing procedure, visual image interpretation, quantitation methods using planar and SPECT studies, and reporting of WR. The pitfall and some tips for the calculation of 123I-BMIPP WR are also included. The targets of global and regional WR calculation include ischemic heart disease, cardiomyopathy, heart failure, and triglyceride deposit cardiomyovasculopathy, an emerging rare heart disease.
