Efficacy and Safety of High-dose Vitamin C Combined With Traditional Chinese Medicine in the Treatment of Moderate and Severe Coronavirus Pneumonia (COVID-19)

In December 2019, coronavirus pneumonia (COVID-19) was detected in many cases in Wuhan, China. With the rapid spread of the coronavirus, cases of COVID-19 were also reported in other cities of China and other countries. COVID-19 can cause amplification of the pulmonary inflammatory responses, including the production of a large amount of free radicals and the release of inflammatory factors, in a short time after infection, leading to lung tissue damage and dysfunction, even resulting in life-threatening respiratory distress syndrome and respiratory failure. At present, there are no effective drugs targeting COVID-19. Previous studies have shown that Buzhong Yiqi Decoction has anti-bacterial, anti-viral, and anti-allergy effects. High-dose vitamin C also has anti-oxidative and anti-toxin effects, possibly exhibiting good effects in the treatment of viral infection and critical respiratory diseases. The study objectives include (1) investigate whether Buzhong Yiqi Decoction can improve the clinical symptoms of patients with mild and severe COVID-19 and accelerate recovery and to further investigate the clinical efficacy of high-dose vitamin C combined with traditional Chinese medicine in the treatment of mild and severe COVID-19; (2) to assess the safety of high-dose vitamin C combined with Chinese medicine in the treatment of mild and severe COVID-19.

Treatment of Polycythaemia Vera and Essential Thrombocythaemia: Influence on the Clot Structure

Myeloproliferative neoplasms (MPN) such as Polycythemia Vera (PV) and, Essential Thrombocythaemia (ET) are rare clonal myeloid neoplasms associated with an increased risk of both venous and arterial thrombosis. Thrombotic complications are the main determinant of morbidity and in a less extend mortality.
Routine haemostasis analysis (TP, aPTT) are usually normal and are useless to demonstrate a hypercoagulable state. However, previous evidence suggests that global coagulation tests such as thrombin generation or thromboelastometry are able to detect signs of procoagulant imbalance in MPN. Similarly, current data seems to demonstrate that fibrin clot properties (clot permeability, turbidimetry, clot lysis time) properties is altered suggesting an hypercoagulable state.
Goals of PV and ET treatments are to control blood count to reduce the risk of thrombotic events. Moreover, new drugs such as Janus Kinase Inhibitors (JAKi) were recently licensed for PV and are under investigations on clinical trial for ET. It is currently unknown if treatments that were used for ET and PV, and especially JAKi are able to modify the hypercoagulable state that is observed in those diseases, and if there is difference between drugs.
To evaluate impact of MPN treatment on prothrombotic haemostatic profile, we propose to evaluate global coagulation and fibrin clot properties in MPN, depending on the treatment.

Mechanism of interferon alpha therapy for chronic hepatitis B and potential approaches to improve its therapeutic efficacy

Hepatitis B virus (HBV) chronically infects 296 million people worldwide and causes more than 820,000 deaths annually due to cirrhosis and hepatocellular carcinoma. Current standard-of-care medications for chronic hepatitis B (CHB) include nucleos(t)ide analogue (NA) viral DNA polymerase inhibitors and pegylated interferon alpha (PEG-IFN-α). NAs can efficiently suppress viral replication and improve liver pathology, but not eliminate or inactivate HBV covalently closed circular DNA (cccDNA). CCC DNA is the most stable HBV replication intermediate that exists as a minichromosome in the nucleus of infected hepatocyte to transcribe viral RNA and support viral protein translation and genome replication. Consequentially, a finite duration of NA therapy rarely achieves a sustained off-treatment suppression of viral replication and life-long NA treatment is most likely required. On the contrary, PEG-IFN-α has the benefit of finite treatment duration and achieves HBsAg seroclearance, the indication of durable immune control of HBV replication and functional cure of CHB, in approximately 5% of treated patients. However, the low antiviral efficacy and poor tolerability limit its use. Understanding how IFN-α suppresses HBV replication and regulates antiviral immune responses will help rational optimization of IFN therapy and development of novel immune modulators to improve the rate of functional cure. This review article highlights mechanistic insight on IFN control of HBV infection and recent progress in development of novel IFN regimens, small molecule IFN mimetics and combination therapy of PEG-IFN-α with new direct-acting antivirals and therapeutic vaccines to facilitate the functional cure of CHB.

Inhibition of human cytomegalovirus replication by interferon alpha can involve multiple anti-viral factors

The shortcomings of current direct-acting anti-viral therapy against human cytomegalovirus (HCMV) has led to interest in host-directed therapy. Here we re-examine the use of interferon proteins to inhibit HCMV replication utilizing both high and low passage strains of HCMV. Pre-treatment of cells with interferon alpha (IFNα) was required for robust and prolonged inhibition of both low and high passage HCMV strains, with no obvious toxicity, and was associated with an increased anti-viral state in HCMV-infected cells. Pre-treatment of cells with IFNα led to poor expression of HCMV immediate-early proteins from both high and low passage strains, which was associated with the presence of the anti-viral factor SUMO-PML. Inhibition of HCMV replication in the presence of IFNα involving ZAP proteins was HCMV strain-dependent, wherein a high passage HCMV strain was obviously restricted by ZAP and a low passage strain was not. This suggested that strain-specific combinations of anti-viral factors were involved in inhibition of HCMV replication in the presence of IFNα. Overall, this work further supports the development of strategies involving IFNα that may be useful to inhibit HCMV replication and highlights the complexity of the anti-viral response to HCMV in the presence of IFNα.

Bulevirtide and emerging drugs for the treatment of hepatitis D

INTRODUCTION:

Hepatitis delta virus (HDV) causes acute and chronic liver disease that requires the co-infection of the Hepatitis B virus and can lead to significant morbidity and mortality. Bulevirtide is a recently introduced entry inhibitor drug that acts on the sodium taurocholate cotransporting peptide, thereby preventing viral entry to target cells in chronic HDV infection. The mainstay of chronic HDV therapy prior to bulevirtide was interferon alpha, which has an undesirable side effect profile.

AREAS COVERED:

We review bulevirtide data from recent clinical trials in Europe and the United States. Challenges to development and implementation of bulevirtide are discussed. Additionally, we review ongoing trials of emerging drugs for HDV, such as pegylated interferon lambda and lonafarnib.

EXPERT OPINION:

Bulevirtide represents a major shift in treatment for chronic HDV, for which there is significant unmet need. Trials that compared bulevirtide in combination with interferon alpha vs interferon alpha monotherapy demonstrated significant increase in virologic response. Overall, treatment with different doses of bulevirtide were comparable. Bulevirtide was generally well tolerated, and no serious adverse events occurred. Understanding the true prevalence of HDV, as well as continued studies of emerging drugs will prove valuable to the larger goal of eradication of Hepatitis D.

Takeda Puts Up $300M to Partner on Protagonist Drug for Rare Blood Disease

Takeda Pharmaceutical is stocking up its rare disease drug pipeline, reaching a deal for global rights to a Protagonist Therapeutics drug in late-stage development for the treatment of the rare blood disorder polycythemia vera. Under deal terms announced Thursday, Takeda is paying $300 million up front to share in the development of the drug, rusfertide. Newark, California-based Protagonist remains responsible for completing the ongoing Phase 3 test and submitting the drug for FDA review. If it’s approved, Takeda will lead commercialization of the drug. Polycythemia vera is an overproduction of blood cells. It’s caused by a mutation in the stem cells of the bone marrow, where red blood cells are produced. In addition to the higher red blood cell count, patients experience iron deficiency. The disorder raises the risk of cardiovascular events, such as heart attack and stroke. One common treatment for polycythemia vera is phlebotomy, frequent withdrawals of blood that decreases blood volume and reduces the levels of excess blood cells. Rusfertide is a peptide from Protagonist’s peptide technology platform. The company said the drug’s exact mechanism of action is not known, but it’s thought to regulate iron homeostasis and control the absorption, storage, and distribution of iron in the body. In an open-label Phase 2 clinical trial enrolling 70 participants, Protagonist reported that the once-weekly injectable drug controlled red blood cell levels and eliminated the need for phlebotomies, achieving the main study goal. Longer-term data presented in December during the annual meeting of the American Society of Hematology showed the drug continued to show durable effects through two-and-a-half years. A 250-patient, placebo-controlled Phase 3 study is underway with the same main goal as the mid-stage test. Protagonist expects to complete enrollment in the current quarter. In this 32-week study, rusfertide or a placebo will be added to a patient’s current polycythemia vera therapy. In an investor presentation, Protagonist said it expects to have 32-week data in the first quarter of next year, potentially setting the stage for an FDA submission in the fourth quarter of 2025. If rusfertide reaches the market, the deal calls for Takeda and Protagonist to split profits equally. Protagonist has an option to share in the marketing of the drug in the U.S. The biotech also has the right to opt-out of the profit share provision of the deal. Doing so would put Protagonist in line for opt-out payments and enhanced milestone and royalty payments. If Protagonist exercises either option, Takeda will still retain full rights to rusfertide outside of the U.S. According to a Protagonist regulatory filing, exercising the full opt-out right would put the company in line for up to $975 million in payments. For Takeda, rusfertide fits with the pharma giant’s strategy of acquiring late-stage assets in rare diseases, Julie Kim, president, U.S. business unit and U.S. country head, Takeda, said in a prepared statement. It also fits with other assets in the pharma giant’s hematology portfolio. This past November, the FDA approved Takeda’s Adzynma, a treatment for congenital thrombotic thrombocytopenic purpura, a rare inherited enzyme deficiency that leads to blood clotting problems. Protagonist does not yet have any FDA-approved products. President and CEO Dinesh Patel said the Takeda deal allows his company to focus on completing rusfertide’s Phase 3 study while leveraging Takeda’s global commercialization capabilities to immediately start pre-commercial work. “As we progress towards a fully integrated pharmaceutical company, this deal mitigates the inherent execution risks of a first-time commercial launch, optimizes the timing and enhances the scope for peak potential sales of rusfertide, while still allowing us to actively participate in the commercial experience and economics with a 50:50 profit split in the U.S. market,” Patel said. PharmaEssentia claimed the first FDA approval of a polycythemia vera therapy in 2021. This injectable drug, named Besremi, is an engineered version of interferon alpha. It’s believed to activate a signaling cascade that reduces production of red blood cells. Others developing potential treatments for the disease include Silence Therapeutics with a drug that employs RNA interference, and Ionis Pharmaceuticals, whose drug candidate is an antisense oligonucleotide.
