A Randomized, Parallel-Group, Controlled, Assessor-Blinded Clinical Trial of Hyperthermia Combined With Hydrogen Peroxide Microneedle Patch for Viral Warts

Hyperthermia treatment (hyperthermia) refers to treating diseases with temperature (39-45 ° C) beyond normal body temperature,. It has been reported that local warming at 44 ° C is able to effectively mobilize the body's immunity and clear HPV infected lesions, such as condyloma acuminatum and verruca vulgaris, etc. Significant progress has been made in the application of hyperthermia for viral skin diseases. Clinically, the addition of hydrogen peroxide solution can enhance the efficacy of hyperthermia in treating HPV infection. As a common transdermal drug delivery method, microneedles can increase drug penetration and thereby further improve treatment outcomes. Based on these findings, this study aims to explore an adjunctive approach to hyperthermia for treating viral warts to further enhance therapeutic efficacy.
This study employs a randomized, parallel-group, assessor-blinded design. Participants will be randomly assigned to three groups: hyperthermia alone, hyperthermia combined with microneedle patch (loaded with 0.9% saline), and hyperthermia combined with hydrogen peroxide microneedle patch (experimental group). An adaptive design will be adopted. The sample size is estimated at 70 participants per group, accounting for a potential 20% dropout rate. Interim analyses will be conducted during follow-up, and enrollment will be stopped when a positive result is reached for the primary efficacy endpoint (cure rate), at which point the sample size will be adjusted accordingly.

Hydrogen Peroxide and Ultraviolet Light for Disinfecting Surfaces in Intensive Care Units: A Randomized, Controlled, Cluster, Crossover Clinical Trial

Healthcare-associated infections (HAIs) remain a major problem in intensive care units (ICUs), driven by environmental contamination with multidrug-resistant organisms that persist despite routine manual cleaning. While hydrogen peroxide aerosolization and ultraviolet-C light devices have shown promise in reducing surface contamination, current evidence is inconsistent, mostly derived from single-center studies, and rarely linked to patient-centered outcomes.
The investigators will conduct this cluster-randomized, crossover trial in 12 Brazilian ICUs. Each ICU will sequentially implement three strategies: (1) usual surface disinfection; (2) usual surface disinfection followed by hydrogen peroxide aerosolization at 7.9% concentration, applied through a dedicated device inside a protective tent during terminal cleaning of patient beds; and (3) usual surface disinfection followed by automated ultraviolet-C irradiation, also applied under the same tent to shield adjacent occupied beds.
The primary outcome will be the antimicrobial utilization, measured as daily defined doses (DDD) of antimicrobials at the ICU level per 100 patient-days, with secondary outcomes including HAI incidence rate, environmental contamination with multidrug-resistant organisms, specific HAIs incidence rate (associated-ventilator pneumonia, central-line associated bloodstream infection, and catheter-associated urinary tract infection), and ICU length of stay costs.

Comparative efficacy of preprocedural mouthwashes in reducing microbial aerosols during dental procedures: A network meta-analysis

BACKGROUND:

This study aimed to evaluate and rank the efficacy of common preprocedural mouthwashes in reducing microbial aerosols during dental procedures via network meta-analysis (NMA) and provide evidence-based guidance for clinical infection control.

METHODS:

We systematically searched eight electronic databases from inception to February 2025. Randomized controlled trials (RCTs) assessing mouthwashes for reducing microbial aerosols during dental procedures were included. A Bayesian NMA with a random-effects model was performed. Risk of bias was assessed using the Cochrane tool, and the certainty of evidence was evaluated using the CINeMA framework. Surface under the cumulative ranking curve (SUCRA) and standardized mean difference (SMD) with 95% confidence interval (CI) were calculated to quantify intervention effects.

RESULTS:

Twenty-five RCTs involving 1111 participants and eight interventions were included. Compared with the control group, chlorhexidine (CHX) showed the largest effect (SMD = -7.10, 95% CI [-9.44, -4.73]), followed by herbal mouthwash (SMD = -5.16, 95% CI [-8.04, -2.17]) and povidone-iodine (PVP-I) (SMD = -4.57, 95% CI [-8.79, -0.21]). Efficacy ranking based on SUCRA values was as follows: CHX (0.90), cetylpyridinium chloride (CPC) (0.68), herbal mouthwash (0.61), PVP-I (0.53), essential oil mouthwash (0.44), normal saline (0.42), hydrogen peroxide (0.39), and control (0.03).

CONCLUSIONS:

CHX appears to be the most effective mouthwash for reducing microbial aerosols, with CPC and herbal mouthwashes as viable alternatives, particularly for patients with CHX intolerance or mucosal sensitivity. However, evidence for CPC is limited, and the overall certainty is low. Further well-designed RCTs are needed, especially to evaluate efficacy against viral aerosols.

BACKGROUND:

Endometritis is a major cause of infertility in intensively farmed donkeys. Streptococcus equi subsp. zooepidemicus (SEZ) is a predominant pathogen that readily forms biofilms in equine endometritis, conferring resistance to conventional treatments.

AIMS/OBJECTIVES:

This study aimed to characterize the biofilm dynamics of SEZ isolates obtained from donkeys and to evaluate potential anti-biofilm strategies with different drug combinations.

METHODS:

Uterine lavage samples were collected from 30 Dezhou Black donkeys with clinical endometritis. Bacterial cultures were performed on blood agar and MacConkey agar. SEZ isolates were identified by 16S rDNA PCR sequencing. Antimicrobial sensitivity test of SEZ isolates was performed with broth microdilution methods. Multilocus sequence typing (MLST) was used for strain characterization, and in vitro biofilm dynamics were assessed using crystal violet assays. Six agents-Tris-EDTA, hydrogen peroxide, DMSO, penicillin potassium, ceftiofur, and ciprofloxacin-were tested individually or in combination for their inhibitory effects on SEZ biofilms.

RESULTS:

Thirteen SEZ isolates were obtained from a total of 31 bacterial isolates (41.9%), representing four novel MLST types: STnovel, ST554, ST555 and ST556. Rapid formation of biofilm was observed at 12-24 h. Strain-dependent inhibitory patterns were observed in SEZ biofilms. Hydrogen peroxide showed the strongest inhibitory effect on STnovel biofilms (73.73 ± 1.48%), while ceftiofur-Tris-EDTA, ciprofloxacin-DMSO and penicillin-DMSO combinations exhibited the highest inhibition against ST554 (66.52 ± 3.66%), ST555 (82.60 ± 2.97%) and ST556 (83.35 ± 1.19%) biofilms.

CONCLUSION:

Strain-dependent inhibitory patterns were observed in SEZ biofilms. Antibiotic-non-antibiotic combinations showed promising effects in vitro, highlighting new therapeutic avenues for controlling SEZ biofilm-associated infections in donkeys.

The fruit of Lanxangia tsao - ko (LT), a widely used spice, is highly perishable during storage and transportation making it essential to develop effective preservation methods. This study employed multi-omics techniques to evaluate the effects of three different treatments on the quality of LT during short-term storage. Results demonstrated that, compared to the tap water control group (TW), the hot water treatment group (HW), carboxymethyl chitosan treatment group (CMC), and hydrogen peroxide‑silver ion solution treatment group (SP) significantly suppressed the increase in color change intensity (ΔE), browning index (BI), weight loss rate (W%), and rot rate (R%) of LT. Furthermore, these treatments promoted the accumulation of beneficial flavor substances, while reducing the level of unfavorable ones. Both CMC and SP were effective in inhibiting the increase of LT's respiration intensity and the relative abundance of pathogenic microorganisms like Rhizopus microsporus and Rhizopus arrhizus. However, HW led to an abnormal decrease in respiration intensity and increases the relative abundance of certain pathogenic microorganisms. In summary, although the temperature setting in the hot water treatment group was not ideal, hot water treatment still significantly inhibited the spoilage and discoloration phenomenon. Both CMC and SP can effectively extend the storage and transportation period of LT in all aspects. This study provides a new idea for the preservation of LT.
