This study capitalizes on the emerging technology of 19F MRI, using conventional 'thermally' polarized perfluorinated gas (perfluoropropane, or PFP) mixed with oxygen and studied with magnetic resonance imaging (MRI) to visualize ventilation. This technique has not been studied in children.
Children and adolescents (6-17 years old) with cystic fibrosis (CF) who have normal spirometry will undergo 19F MRI with the inhalation of an inert contrast gas to study ventilation. Comparisons will be made to a cohort of healthy children (6-17 years old) who will perform the same measures. The primary outcome measure is the feasibility of conducting these studies in the pediatric population. Parallel performance of multiple breath nitrogen washout (MBW) and spirometry will be used to compare the sensitivity of these outcomes to the presence of mild lung disease in these children. Finally, the investigators will compare data obtained during standard breath holds with a novel "free-breathing" technique that will eliminate the need for breath holds during MRI acquisition.

Invoking Subharmonics and Subharmonic Aided Pressure Estimation (SHAPE) for Identifying Portal Hypertension

This clinical trial tests the how well an ultrasound with subharmonic imaging and the subharmonic aided pressure estimation (SHAPE) technique works in identifying portal hypertension (PH). An ultrasound takes pictures of the inside of the body by bouncing sound waves off organs. PH is high blood pressure in the vein that carries blood to the liver from the stomach, small and large intestines, spleen, pancreas, and gallbladder. The complications associated with PH are clear only after severe liver dysfunction or liver cirrhosis develops and are accompanied by relatively high mortality rates (20-70% mortality within 2 years). Thus, identifying PH earlier is beneficial. The hepatic venous pressure gradient (HVPG) obtained using an invasive catheterization procedure remains the standard for assessing PH. However, using this invasive procedure to assess PH prevents frequent pressure monitoring. Thus, a noninvasive technique to estimate PH is beneficial not only for diagnosis but also for monitoring treatment and disease progression. The SHAPE technique is a noninvasive ultrasound-based imaging technique that can estimate pressure with an ultrasound contrast agent. A noninvasive technique using an ultrasound with subharmonic imaging and the SHAPE technique may work in identifying PH.

Illuminating the Safety of Ultrasound Contrast Agents

Echocardiography is an essential element for accurate and timely diagnosis of cardiac pathology. In this context, ultrasound enhancing agents (UEAs), are often essential for visualizing endocardial borders,¹ evaluating myocardial perfusion, characterizing intracardiac masses, and other applications. Although UEAs have been shown to be safe, rare serious adverse events (SAEs) have been reported. It is in this context that De La Fuente Gonzalez et al performed a systematic review evaluating randomized and observational data comparing the safety of UEA brands from 2018-2023. The authors conclude that "Optison reduces risk of serious adverse events vs. Definity/Lumason." However, the data presented do not support such a conclusion, as will be discussed in this editorial. What can be concluded is that UEAs as a class are extremely safe with very rare SAEs. The use of UEAs during echocardiography are encouraged per guideline-directed indications based upon evidence-based reduction of misdiagnosis and duplicative testing.

Effect of the Chemical Structure of a Self-Assembled Monolayer on the Gas-Sensing Behavior of SnO₂ Nanowires

In this study, detailed investigations of the selective sensing capability of semiconducting metal oxide (SMO)-based gas sensors with self-assembled monolayer (SAM) functionalization were conducted. The selective gas-sensing behavior was improved by employing a simple and straightforward postmodification technique using functional SAM molecules. The chemical structure of the SAM molecules promoted interaction between the gas and SAM molecules, providing a gas selective sensing of SnO₂ nanowires (NWs). In addition, a bundle of SnO₂ NWs provided a large surface area that could act as a sensing site. SAM functionalization was confirmed by infrared spectroscopy and thermogravimetric analysis, and the selective gas-sensing behaviors were investigated under different sensing conditions. With variations in the chemical structures of the SAM molecules, the selective gas-sensing behaviors also changed as the corresponding intermolecular interaction forces were different. This integration of selective gas sensing and passivation of a sensing surface provides a straightforward approach for the preferred gas sensing of SnO₂ NWs. Furthermore, owing to the universal binding characteristics of SAM molecules to the metal oxide surface, this approach can be expanded to other SMO-based gas-sensing platforms.

Porous crystals bind fluorine-containing greenhouse gases

Fluorine-containing gases -- including so-called per- or polyfluorinated hydrocarbons, or PFCs -- have a significant share in global warming. Researchers recently developed new crystalline materials that can selectively adsorb the molecules of such carbon-fluorine bonds. They hope that these porous crystals may be useful for targeted binding and recovery of PFCs. Emissions of greenhouse gases contribute significantly to global warming. Not only carbon dioxide (CO2) but also fluorine-containing gases -- including so-called per- or polyfluorinated hydrocarbons, or PFCs -- have a significant share in this development. Researchers at the Institute of Organic Chemistry of Heidelberg University led by Prof. Dr Michael Mastalerz recently developed new crystalline materials that can selectively adsorb the molecules of such carbon-fluorine bonds. The Heidelberg researchers hope that these porous crystals may be useful for targeted binding and recovery of PFCs. Polyfluorinated carbons are organic compounds of various lengths in which the hydrogen atoms of alkanes are partly or fully replaced by fluorine atoms. These atoms are chemically highly stable. They are not ubiquitous in nature and are used mainly for etching processes in the semiconductor industry, in eye surgery, and in medical diagnostics as contrast enhancers for certain ultrasound examinations. "Unlike CO2, which is integrated in natural material cycles, PFCs accumulate in the atmosphere and stay there for several thousands of years before breaking down," stresses Prof. Mastalerz. Compared to carbon dioxide, PFCs thus have a much greater global warming potential -- the impact of one PFC molecule is virtually equal to 5,000 to 10,000 CO2 molecules. According to the researcher, that makes polyfluorinated hydrocarbons a permanent problem that is not only contributing to global warming now but accelerating it as well. With his research group at the Institute of Organic Chemistry of Heidelberg University, Prof. Mastalerz has developed a new type of crystalline material that can adsorb polyfluorinated hydrocarbons highly selectively, i.e., binding them to its interior surface. The porous crystals are based on shape-persistent organic cage compounds that carry fluorine-containing side chains on the interconnected struts. These side chains react according to the "like attracts like" principle via fluorine-fluorine interactions with the PFC molecules, ensuring they are deposited on the inner surface of the material. In their experiments, the Heidelberg researchers proved that the crystals they developed bind certain fluorine-containing gases such as octafluoropropane or octafluorocyclobutane approximately 1,500 to 4,000 times more strongly than dinitrogen, the main component of air. According to Prof. Mastalerz, these numbers represent extraordinarily high selectivities to bind such PFCs. Currently Prof. Mastalerz and his team are working on further increasing the selectivity of the crystals and transferring the process to other fluorinated gases, such as those used in medical anaesthesia. "I see enormous potential for development in this area," emphasises the researcher. He hopes that the adsorbent can be used for recovery of polyfluorinated hydrocarbons at their point of use. Adverse reactions (headache, dysgeusia and fatigue) were reported in ≤ 2% of patients within the studies. In addition, a delayed hypersensitivity reaction was reported in one patient (0.2%) with a history of allergic reaction.
