Randomized clinical trial comparing the efficacy and safety of oxfendazole at 20 mg/kg per dose in one and two dose regimens with a two-dose regimen of triclabendazole at 10 mg/kg in an endemic region of the highlands of Peru. Children and adults with fascioliasis in rural communities will be screened for inclusion and exclusion criteria and a total of 336 subjects (112 per study arm) with chronic Fasciola infection will be enrolled and assigned randomly to the study arms in a 1:1:1 ratio. The primary efficacy (cure and egg reduction) endpoints will be assessed on day 7 and 30 post treatment. The secondary safety endpoint visits will be performed in days 0, 3, 7 and 30 post-treatment and Population Pharmacokinetics (PK) modeling studies will be performed in the first 24 hours after the first dose of oxfendazole.

A Double-blind Multicenter, Randomized Controlled Trial of Single and Multiple Dose Regimens of Oxfendazole for Mild (One or Two Lesions) Parenchymal Brain Cysticercosis, With an Open Comparison Group

The goal of this clinical trial is to compare a single and multiple dose regimens of oxfendazole with the standard treatment in patients with mild (one or two lesions) parenchymal brain cisticercosis. The main question it aims to answer is if OXF will enhance clearance of brain parasites and therefore provide greater cysticidal efficacy, with the potential to provide a single-dose therapy for this type of NCC.
The study cohort will also allow us to identify early imaging markers that predict lesion resolution, as well as factors associated with residual calcification or focal gliosis after lesion resolution. This study will also provide additional information on the safety of the study interventions.

An Assessor Blind, Randomized, Comparative Study of the Efficacy of Different Doses of Oxfendazole Compared to Single Dose Albendazole in the Treatment of Trichuris Trichiura Infection in Adults

The main objective of this study is to provide data on the efficacy profile of different doses of oxfendazole when used in Trichuris trichiura infection. The drug will be also be examined for efficacy against other common nematodes encountered in man (Ascaris lumbricoides, Ancylostoma duodenale, Necator americanus). The study will also provide data on the safety and tolerability of the oxfendazole in patients.

Efficacy of fenbendazole against gastrointestinal nematodes in naturally infected goats in Maputo Province, Mozambique using in vivo, in vitro and molecular assessment

Anthelmintic resistance occurs worldwide in strongyles of ruminants but data from low-income countries are sparse and rarely apply most up to date methods, while effects of management practices in these countries are poorly documented. In Mozambique, benzimidazole resistance has been previously reported; the present study followed this up in detail, applying in vivo faecal egg count (FEC) reduction test (FECRT), in vitro egg hatch test (EHT) and molecular deep amplicon sequencing approaches targeting the internal transcribed spacer 2 (ITS-2, nemabiome) and the isotype 1 β-tubulin gene to determine the resistance status on farms and the strongyle species involved. Adult Landim goats (433) from six semi-intensive and ten extensive farms (22-30 animals/farm) from Maputo Province were visited April 2021 to February 2022. Fenbendazole (5 mg/kg bw, Panacur®) was administered orally and FEC determined using Mini-FLOTAC. The eggCounts package was used to calculate FECRs with 90% confidence intervals from paired day 0 and 14 data. In vivo and in vitro tests detected AR on 5/16 (31%) farms. This included 1/10 extensive and 4/6 semi-intensive farms. The odds of finding resistant strongyles on a semi-intensive commercial farm was 40-fold higher than on an extensive farm (p = 0.016, logistic regression). A strong, negative correlation was observed between FECRT and EHT EC₅₀ values (Pearson's R = -0.83, P = 0.001; Cohen's κ coefficient 1.0). Nemabiome data showed that Haemonchus contortus, Trichostrongylus colubriformis and unclassified Oesophagostomum closely related to Oesophagostomum columbianum were most abundant before treatment and in particular H. contortus frequencies increased after treatment. Benzimidazole resistance associated polymorphisms were detected in H. contortus and T. colubriformis. Moreover, there were hints that resistance alleles were present in Trichostrongylus axei and Teladorsagia circumcincta. Farmers should regularly test the efficacy of anthelmintics used and consider more sustainable worm control approaches to reduce selection for resistance.

Spatial occurrence of emerging contaminants in rivers and wastewater. Analysis of environmental and human risks

AbstractThis study assesses the occurrence of emerging contaminants (ECs) from agricultural and livestock production activities along the Salado River (Santa Fe province, Argentina). Of the 23 ECs studied, 8 were detected and quantified in river and wastewater samples, including ciprofloxacin, enrofloxacin, chlorpyrifos-methyl, albendazole, fenbendazole, levamisole, diazepam, and thiamethoxam. In river samples, the highest concentrations corresponded to ciprofloxacin, chlorpyrifos-methyl, and enrofloxacin. In wastewater samples, albendazole, fenbendazole, ciprofloxacin, enrofloxacin, and thiamethoxam were found. The detection frequency ranged from 4.2% to 54.2% in river samples and from 11.1% to 22.2% in wastewater samples. The spatial distribution of contaminants showed different concentrations, with higher levels often found near urban and agricultural areas, suggesting anthropogenic sources. Ecological risk assessments for different organisms were conducted. Ciprofloxacin posed the highest risk, especially affecting bacteria, cyanobacteria, and algae. Diazepam also was found to pose significant risks to algae, crustaceans, and fishes. Chlorpyrifos-methyl was identified as highly hazardous to multiple living organisms, which is in agreement with the fish mortality that occurred at the sampling sites. The risk assessment in humans showed differences among children, adolescents, and adults, with infants (6 months to 1 year) being at a higher risk than adults. Children may be at a higher daily intake of contaminants than adults, raising concerns about the long-term effects of exposure. This work underscores the critical need for monitoring and regulating ECs in aquatic environments. Further studies are necessary to fully understand their impact and to develop effective strategies for mitigating their presence in water systems.

Survey on canine hookworms: Knowledge, perceptions and practices among U.S. veterinarians

Canine hookworms (Ancylostoma caninum) are highly prevalent in pet dogs in the United States of America (U.S.). They pose a significant health risk to infected dogs and are also a potential risk to humans as zoonotic agents. Veterinarians play a crucial role in managing this threat through the diagnosis and treatment of infections in pet dogs. However, the rising incidence of anthelmintic resistance in U.S. hookworm populations necessitates increased awareness among veterinarians. To determine U.S. veterinarian's knowledge, perceptions and practices about canine hookworms, a cross-sectional online survey was administered between February and December 2022. 208 veterinarians from 43 states participated in the study. Only 65 % of veterinarians reported awareness of hookworm prevalence. Hookworm infected dogs were encountered more frequently in the South than in Western states. Veterinarians perceived that hookworms were present in all breeds without breed predilection followed by mixed breeds, pit bulls and hound breeds. Most respondents reported that infections were rarely symptomatic, highlighting the importance of diagnostic testing. In-clinic diagnostic modalities were most preferred. Fenbendazole and pyrantel pamoate were the most commonly used anthelmintics. Most veterinarians followed up on infected dogs but retesting time after treatment varied. Awareness of multi-drug resistant hookworms was high but encountering persistent hookworm cases was lower. Veterinarians frequently educated clients about hookworms and prevention, but less about the zoonotic potential. This study provides insights for improving awareness, diagnostics, treatment protocols, and client education regarding canine hookworms in the U.S.. Overall, veterinarians play an active role in hookworm prevention and control.
