Abstract:

Purpose::

A multicenter, randomized, open-label, phase II study (HERIZON; NCT02795988) was conducted to evaluate the clinical and immunologic efficacy of HER-Vaxx (IMU-131), a B-cell, peptide-based vaccine targeting HER2 overexpressed in 6% to 30% of gastroesophageal adenocarcinomas (GEA).

Patients and Methods::

Patients (n = 36) with GEA were treated with standard-of-care chemotherapy (n = 17) or HER-Vaxx plus chemotherapy (n = 19), using the recommended phase 2 dose for the vaccine. Overall survival (OS; primary endpoint), safety, progression-free survival (PFS), clinical response (secondary endpoints), and vaccine-induced HER2-specific antibody levels in serum and correlation with tumor response rates (exploratory endpoints) were investigated.

Results::

A 40% OS benefit [HR, 0.60; median OS, 13.9 months; 80% confidence interval (CI), 7.52–14.32] for patients treated with HER-Vaxx plus chemotherapy compared with OS of 8.31 months (80% CI, 6.01–9.59) in patients that received chemotherapy alone. A 20% PFS difference was obtained for the vaccination arm (HR, 0.80; 80% CI, 0.47, 1.38). No additional toxicity due to HER-Vaxx was observed. The vaccine-induced high levels of HER2-specific total IgG and IgG1 antibodies (P < 0.001 vs. controls) that significantly correlated with tumor reduction (IgG, P = 0.001; IgG1, P = 0.016), had a significant capacity in inhibiting phosphorylation of the intracellular HER2-signaling pathways, mediated antibody-dependent cellular cytotoxicity, and decreased immunosuppressive FOXP3+ regulatory T cells.

Conclusions::

HER-Vaxx plus standard chemotherapy exhibits an excellent safety profile and improves OS. Furthermore, vaccine-induced immune response was significantly associated with reduced tumor size compared with standard-of-care chemotherapy. The presented vaccination approach may substitute for treatment with trastuzumab, upon unavailability or toxicity, based on further evidence of equivalent treatment efficacy.

Abstract:

Purpose::

HER2/neu is overexpressed in up to 30% of gastroesophageal adenocarcinomas (GEA) and linked to poor prognosis. Recombinant mAbs to treat HER2/neu-overexpressing cancers are effective with limitations, including resistance and toxicity. Therefore, we developed a therapeutic B-cell epitope vaccine (IMU-131/HER-Vaxx) consisting of three fused B-cell epitopes from the HER2/neu extracellular domain coupled to CRM197 and adjuvanted with Montanide. This phase Ib study aimed to evaluate the optimal/safe dose leading to immunogenicity and clinical responses (https//clinicaltrials.gov/ct2/show/NCT02795988).

Patients and Methods::

A total of 14 patients with HER2/neu-overexpressing GEA were enrolled, and dose escalation (10, 30, 50 μg) was performed in three cohorts (C). Immunogenicity was evaluated by HER2-specific Abs and cellular responses, clinical responses by CT scans according to RECIST version 1.1.

Results::

IMU-131 was safe without vaccine-related significant local/systemic reactions or serious adverse events. A total of 11 of 14 patients were evaluable for changes in tumor size and vaccine-specific immune responses. One patient showed complete, 5 partial responses, and 4 stable diseases as their best response. HER2-specific IgG levels were dose dependent. In contrast to patients in C1 and C2, all patients in C3 mounted substantial HER2-specific Ab levels. In addition, cellular vaccine responses, such as Th1-biased cytokine ratios and reduced regulatory T cell numbers, were generated. Progression-free survival was prolonged in C3, correlating with the vaccine-specific humoral and cellular responses.

Conclusions::

IMU-131 was well tolerated and safe. The induced HER2-specific Abs and cellular responses were dose dependent and correlated with clinical responses. The highest dose (50 μg) was recommended for further evaluation in a phase II trial, with chemotherapy + IMU-131 or chemotherapy alone, which is currently ongoing.

Abstract Background HER2/neu is overexpressed in 15% - 25% of gastric cancers. IMU-131 is a B-cell peptide vaccine composed of 3 epitopes of the extracellular domain of HER2/neu. Antibodies against IMU-131 peptides elicit antitumor activity in vitro and a phase I study demonstrated safety and immunogenicity in Her-2 +/ ++ breast cancer patients. Methods IMU-131 was given to patients with HER2/neu positive gastric cancer in an open-label phase 1b dose escalation trial with 14 Asian and Eastern European sites. Each patient received IMU-131 on Days 0, 14, and 35, with cisplatin and 5-fluorouracil or capecitabine every 21 days. Patients remained on study until disease progression. Ongoing patients received boosters at D98 then every 84 days. Results 14 patients were enrolled with 10 HER2 overexpressing (7 x HER2 +++, 3 x HER2 ++ FISH positive) and 4 HER2 ++ expressing tumors. Mean age was 57 yo (range of 21 - 79) with ECOG scores of 0 or 1 in 7 patients each. There were 9 Asian and 5 Caucasian patients with 5 females and 9 males. Dose levels were 10, 30 and 50 µg with 3, 6, and 5 patients respectively. 11 patients received all 3 doses and 3 patients received only 2 doses due to disease progression. P467 and HER2 antibodies were generated at all dose levels with patients dosed at 50µg responding to the vaccine with equally high antibody levels. There were no DLTs or SAEs related to IMU-131. Of the 14 patients dosed 11 were evaluable for tumor progression at day 56 and later. Of those patients, the best response was 1 CR, 5 PR, 4 SD and 1 PD. Two patients, both dosed at 50µg IMU-131, remain on study after D266, with one patient’s tumor reduced by approximately 80% from 177mm at baseline to 37mm at D266. Conclusions Safety data indicate that IMU-131 is well tolerated with no significant local or systemic reactions. There were no dose-limiting toxicities observed, no significant injection site reactions and no IMU-131 related SAEs. The 50µg dose produced the most consistent P467 and HER-2 specific antibodies compared to 10 and 30µg doses with preliminary response data demonstrating 50µg of IMU-131 is associated with tumor size reduction. Merck’s Keytruda Adds Advanced Endometrial Carcinoma to Winning Roster

(Courtesy of Kena Betancur/Getty Images) Merck ’s Keytruda (pembrolizumab) has become a Food and Drug Administration (FDA) -approved treatment for yet another indication- unresectable or metastatic advanced endometrial carcinoma that is microsatellite instability-high or mismatch repair (MSI-H or dMMR). This announcement adds another win to Merck’s trophy case, as the biologic has already impressed the biopharmaceutical industry with an extensive array of indications, including for three other gynecological cancers. Keytruda is an anti-programmed death receptor therapy that utilizes monoclonal antibodies to block interactions between PD-1 and ligands. This interaction typically results suppresses T lymphocyte activation, allowing the proliferation of tumor cells. Patients must have attempted systemic therapy treatments without success and cannot be candidates for curative surgery or radiation. About the Trial of Keytruda for Endometrial Carcinoma The KEYNOTE-158 Phase II trial ( NCT02628067 ) is a non-randomized, open-label trial that spans 27 study locations and involves various cohorts to evaluate Keytruda intervention of a multitude of advanced solid tumors. The cohort of endometrial carcinoma patients is comprised of 90 patients aged between 35 and 56 and resulted in a 46% treatment response rate. The treatment regimen included an intravenous injection every three weeks for up to two years, so long as the patient did not display signs of disease progression or toxicity. Patients underwent an average of 8.3 months of treatment before reaching either of these points. For 68% of the patients who responded to treatment, the positive physiological response continued for at least 12 months, with a smaller cohort experiencing 24 months of response. The classification of endometrial carcinoma as MSI-H or dMMR accounts for 30% of cases. Previously, combination therapy with lenvatinib was approved, indicated for advanced endometrial carcinoma that is not categorized as MSI-H or dMMR. Over 65,000 women are diagnosed with endometrial carcinoma, and 12,500 fatalities are attributed to the disease annually. Endometrial carcinoma is the most prevalent of uterine cancers. Along with endometrial carcinoma, Keytruda has been approved for treatment of non-small cell lung cancer, unresectable or metastatic melanoma, recurrent head and neck squamous cell carcinoma, Hodgkin lymphoma, B-cell lymphoma, urothelial carcinoma, non-muscle-invasive bladder cancer and microsatellite instability-high or mismatch repair deficient cancer. Additionally, ongoing clinical trials are investigating Keytruda indicated for ovarian cancer, triple-negative breast cancer, metastatic castration-resistant prostate cancer , advanced-stage lung cancer, renal cell carcinoma and solid tumors with certain biomarkers. The biologic will likely continue to impress, as it is involved in over 1,700 clinical trials, evaluating how broad the treatment can reach. Humanized monoclonal antibody treatments such as Keytruda have been heavily represented in the news since the onset of the COVID-19 pandemic, as the immunotherapeutic mechanism works to heighten the body’s immune response. Other than using it to treat endometrial carcinoma, a Phase II collaboration with Imugene was also announced last week that will investigate the safety and efficacy of Keytruda in combination with Imugene’s HER-Vaxx., an immunotherapeutic B-cell activator. The collaboration, nextHERIZON, seeks to gather data on how patients with metastatic HER-2/neu over-expressing of gastroesophageal junction adenocarcinomas respond to the combination therapy. Keytruda has been granted Orphan Drug Designation for research into rare cancers, in exchange for market exclusivity that aids in Merck’s financial successes.
