Here, we report on the first patient (78-yr-old man) with a metastatic, hormone-active (carcinoid syndrome) ileal neuroendocrine tumor (G1, Ki-67, < 3%), who received a test infusion of 1 GBq [¹⁶¹Tb]Tb-DOTA-LM3 in an ongoing prospective Phase 0 study.So far, the patient received long-acting octreotide, which was stopped 2 mo before [¹⁶¹Tb]Tb-DOTA-LM3 infusion.The ¹⁶¹Tb emits conversion electrons and high quantities of Auger electrons (1213%) with a high linear energy transfer over a short distance (< 40 keV/μm), while Somatostatin receptor subtype 2 antagonists such as DOTA-LM3 bind to many more binding sites, which leads to a much higher tumor accumulation compared to somatostatin receptor subtype 2 agonists.The case presented shows the potential of [¹⁶¹Tb] Tb-DOTA-LM3 as a promising alternative to the current standard peptide receptor radionuclide therapy with [¹⁷⁷Lu]Lu-DOTATOC/[ ¹⁷⁷Lu]Lu-DOTATATE (Lutathera) for patients with metastatic gastroenteropancreatic neuroendocrine tumors.

Neuroendocrine neoplasms of the gallbladder: A single institute analysis of outcomes and prognostic factors

AbstractIntroductionNeuroendocrine neoplasms (NENs) are classified as neuroendocrine tumors (NETs), neuroendocrine carcinomas (NECs), and mixed neuroendocrine and nonneuroendocrine neoplasms (MiNENs) according to World Health Organization classification. We present our experience of NENs of the gallbladder (GB) from a high‐volume cancer hospital.Materials and MethodsThe present study is a retrospective analysis of all patients with GB NENs who presented between January 2015 and June 2023. The patient details and treatment received with follow‐up were noted. The primary endpoint was overall survival (OS).ResultsA total of 147 patients were included in the study. The median age was 52 (27–81) years. There was a female predominance (70.7%). NEC was the most common subtype (84.4%) followed by MiNEN (12.9%) and NET (2.7%). The most common stage at presentation was metastatic (70.7%) followed by locally advanced (21.8%), and early disease (7.5%). The median follow‐up was 9.92 (1.77–76.06) months. Median OS was 6.14 (3.93–8.35) months. Median OS in patients who received multimodality treatment was 20.20 (17.99–22.41) months versus 4.00 (2.91–5.10) months in those who did not receive it.ConclusionGB NENs are rare, but aggressive tumors with NEC being the most common type. Multimodality treatment yields favorable outcomes. However, the development of better systemic therapy is needed to help improve survival further.

Peptide radionuclide radiation therapy with Lutathera in multirecurrent nonanaplastic meningiomas: antitumoral activity study by growth rate analysis

PURPOSESeveral retrospective studies and meta-analyses of Peptide Radionuclide Radiation Therapy in meningiomas suggest six-month progression-free survival improvement for WHO grade 1 and 2 meningiomas. In the present study, we aimed to evaluate the impact of such treatment on three-dimensional volume growth rate (3DVGR) in nonanaplastic meningiomas.METHODSThe authors performed a retrospective study including eight patients treated with Lutathera®. Millimetric 3D T1-weighted with gadolinium enhancement magnetic resonance imaging sequences were requested for volume measurement. Then, tumor growth rate was classified following a previously described 3DVGR classification (Graillon et al.).RESULTSPatients harbored seven WHO grade 2 meningiomas and one aggressive WHO grade 1. All patients, except one, underwent four treatment cycles. 3DVGR significantly decreased at 3, 6, and 12 months after treatment initiation analyzing each lesion separately. Mean and median 3DVGR from all patients were respectively at 29.5% and 44.5%/6 months before treatment initiation, then at 16.5% and 25%/6 months at three months post-treatment initiation, 9.5% and 4.5%/6 months after 6 months, as well as 9.5% and 10.5%/6 months after 12 months. At 3, 6, and 12 months after treatment initiation, 4/8, 6/7, and 5/6 patients were class 2 (stabilization or severe 3DVGR slowdown), respectively. No patient was class 1 at 6 and 12 months, suggesting a lack of drug response.CONCLUSIONIn nonanaplastic meningiomas, Lutathera®'s antitumoral activity appeared delayed and more likely observed at six months, while no major response was observed under treatment. Moreover, its antitumoral activity persisted for 12-18 months following treatment initiation.
