Non-inferiority, Phase IV, Open-label, Randomized Controlled Trial of Vesical Imaging- Reporting and Data System (VI-RADS) Followed by Primary Photodynamic Trans-urethral Resection of Bladder Tumours (PDD-TURBT) Versus Conventional White-light TURBT Plus Repeated-TURBT (Re-TURBT) in Non-Muscle Invasive Bladder Cancers (NMIBCs) Candidate for Second Look and Resection

Background: In European Association of Urology (EAU) Guidelines, the vast majority of non-muscle-invasive bladder cancers (NMIBCs) undergo a primary transurethral resection of the bladder tumor (TURBT) followed by a repeat TURBT (Re-TURBT). The Re-TURBT is recommended due to the possibility of residual bladder cancer but is unnecessary in many cases by constituting overtreatment. Currently, no diagnostic strategy or predictive tools have been implemented to further stratify who does or does not benefit from Re-TURBT. Recently, an MRI-based Vesical Imaging Reporting and Data System (VI-RADS) score has been developed to stage as to the preoperative probability of muscle invasion, which could potentially exclude those who do not require a Re-TURBT when a primary high-quality resection is delivered. As such, performing TURBT with standard white light (WL) cystoscopy is known to miss many bladder tumours, which may be poorly visible, and a technique known as with photodynamic diagnosis (PDD) results in lower residual tumor and lower early intravesical recurrence rates. PDD is performed using violet light to improve the detection of these lesions not easily visible with WL cystoscopy.
Methods/Aims: The investigators propose an Italian, single-center, phase IV, open-label, non-inferiority, randomized controlled trial, in which participants (n=112) who had already received a mpMRI/VI-RADS score, are randomized to receive PDD-TURBT, no Re-TURBT versus standard of care represented by conventional WL-TURBT followed by WL-Re-TURBT. The primary outcome is proportions of early recurrence in the urinary bladder. Secondary outcomes will include proportions of late BCa recurrence, late disease-free interval, time to progression to MIBC, patient's quality of life assessment, and cost-analysis.
Perspective: The CUT-less trial aims to respond to this unmet need through a non-inferiority randomized clinical study potentially shaping the perspective for a paradigm shift towards a more personalized, socially, and economically sustainable updated NMIBC therapeutic pathway.
Implications: The current clinical trial proposal is aiming to achieve a paradigm shift in the oncological and socio-economical management of urothelial malignancies of the urinary bladder. Our first concern is indeed to guarantee a safe and ground-breaking strategy to manage the pathway of such patients in order to guarantee the non-inferior oncologic safety (and possibly superiority) when compared to the current standard of care.
Additionally, if our hypotheses are confirmed, the investigators will be able to significantly relieve these patients from the oncologic burden of an already invasive and arduous bladder cancer care path. Finally, safely avoiding an unnecessary, expensive surgical procedure will bring significant social and economic benefits to the EU healthcare system and possibly worldwide.

Outcomes of High-risk Non-muscle Invasive Bladder Cancer Treated With Blue Light Resection

Comparing white-light cystoscopy (WLC) and blue-light cystoscopy (BLC) in TURBT for high risk (HR) non-muscle invasive bladder cancer (NMIBC) patients is crucial to determine the most effective method for reducing residual disease burden and improving recurrence-free survival. Enhanced visualization with BLC may lead to more accurate resections, potentially decreasing recurrence rates and improving long-term outcomes for bladder cancer patients. Patients will be randomized to either WLC TURBT or BLC TURBT, and outcomes will be measured using standard-of-care testing with cystoscopy and cytology, along with minimal residual disease (MRD) burden evaluation using urine next-generation sequencing.

Bibliometric analysis of photodynamic research in bladder cancer: Trends and future directions

BACKGROUNDRecent years have seen the use of photodynamic technologies concerning the detection and therapy of bladder cancer (BC) due to their rapid development and well-established therapeutic impact. However, a thorough analysis and bibliometric assessment of photodynamic technologies publishing trends in BC has not been completed yet.METHODSRetrieving bibliographies from the Web of Science Core Collection limited the publication date to December 31, 2023, from January 1, 2004. We used VOSviewer (Version 1.6.19) and CiteSpace (Version 6.4 R1) for both statistical and visualization analysis.RESULTSWe selected a total of 870 documents for analysis. The yearly publication findings show notable upward patterns over the last two decades. The Kochi Medical School in Japan was the most productive school, while the USA was the most productive nation. Japanese researcher Inoue Keiji published the highest number of photodynamic -related articles in BC. The most quoted and prolific journals were the Photodynamic Therapy and Photodiagnosis. According to the keyword analysis, the terms "cystoscopy," "carcinoma in situ," "drug delivery," "follow-up," "hexaminolevulinate," and "impact" are all relatively recent and hot field.CONCLUSIONSOur investigation produced a bibliometric outcome for the field, potentially opening up new research opportunities. We suggest that future research concentrate on in-situ carcinoma identification, photosensitizer invention, medication delivery enhancement, and photodynamic technology follow-up in BC.

Long-term outcome of photodynamic therapy with hexyl aminolevulinate, 5-aminolevulinic acid nanoemulsion and methyl aminolevulinate for low-risk Basal Cell Carcinomas

BACKGROUNDNon-surgical treatments are cost-effective options for low-risk basal cell carcinomas (BCCs) i.e. superficial or small nodular BCCs located outside the high-risk locations. Hexyl aminolevulinate (HAL) and 5-aminolevulinic acid nanoemulsion (BF-200 ALA) with enhanced penetration depth enables the use of lower concentrations compared to methylaminolevulinate (MAL) in photodynamic therapy (PDT). We have previously reported comparable short-term efficacies for MAL 16 %, BF-200 ALA 7.8 % and HAL 2 % in PDT of low-risk BCC, and here we report the long-term results.OBJECTIVESThe goal of this trial was to compare long-term outcomes of HAL and BF-200 ALA, compared to MAL in PDT of low-risk BCCs.METHODSNinety-eight histologically verified low-risk BCCs on the trunk or extremities were included and randomized into three arms to receive PDT in two sessions with MAL, BF-200 ALA or HAL. A blinded dermatologist assessed the response, cosmetic outcome, and obtained biopsies for histological verification at three months, one year and five years. Histologically verified non-responsive lesions were excised. Patients' satisfaction with the treatment was also queried.RESULTSAccording to intention-to-treat (ITT) analyses, the cumulative response rate at one year was 90.6 % for MAL, 81.3 % for BF-200 ALA, and 75.8 % for HAL, and correspondingly at five years 71.9 %, 54.6 % and 60.6 %. There were no statistically significant differences between interventions and comparator. The overall cumulative response rate for PDT was 82.5 % at one year and 62.2 % at five, and 48.6 % of the treatment failures were recorded at five years. The recurrent lesions were excised as second line treatment. No aggressive subtypes were reported, with only superficial or nodular growth in the final histopathological report. There were no significant differences in cosmetic outcome or patient satisfaction.CONCLUSIONSThis trial shows that HAL has potential for dermatological PDT. However, the long-term efficacy of PDT in the treatment of low-risk BCCs remains rather low.

Bladder Cancer presentations at AUA2025: Blue Light Cystoscopy improves risk stratification and informed decision making

OSLO, Norway, April 29, 2025 /PRNewswire/ -- Photocure ASA (OSE: PHO) , the Bladder Cancer Company, announces four abstract presentations at the AUA 2025, highlighting the benefits of Blue Light Cystoscopy (BLC®), notably its impact on management of the disease, improved risk stratification and therefore the ability of the BLC procedure to help urologists and patients make well-informed decisions. The American Urological Association Annual Congress 2025 was held April 26-28, at the Venetian Convention & Expo Center in Las Vegas, NV, USA. Three abstracts were presented from Photocure's U.S. Blue Light Cystoscopy with Cysview Registry, a large multicenter bladder cancer patient registry of real-world data, established by Photocure in 2014 and projected to enroll 4,400 patients. In addition, the study protocol of a randomized controlled non-inferiority trial comparing a multidisciplinary approach including PDD-guided primary TURBT to reduce the patients' burden of second resection including a total of 327 patients has been presented. This investigator-initiated trial is supported by Photocure. The abstract sessions on Saturday, April 26: "Upstaging and Risk Migration with BLC for NMIBC: Results from a prospective multicenter registry" by Alireza Ghoreifi, Duke University The study looked at 2,854 NMIBC* patients from the US Blue Light Cystoscopy with Cysview Registry. A total of 201 (7%) patients had at least one malignant lesion detected exclusively by BLC while having a negative WLC. These lesions (335 in total) included carcinoma in-situ (CIS) (145; 43%), low-grade Ta (53; 16%), high-grade Ta (95; 28%), high-grade T1 (37; 11%), and high-grade T2 (5; 1%). As a result of BLC-enhanced detection, the rate of upgrading or upstaging to a more advanced tumor using BLC was 9.3%. The authors concluded that resulting changes in grade/stage could impact patient management, such as the appropriate administration of intravesical therapy, duration of therapy, and when to perform radical cystectomy. The results are expected to form the basis for further studies on how Blue Light Cystoscopy can support precision diagnostics and improve patient management in NMIBC. The authors developed predictive models for recurrence and progression in patients treated with BLC using 1109 patients. Median age at diagnosis was 71 years, and 198 (18%) patients were female. Tumor stage was Ta in 658 (60%), T1 in 241 (22%), and pure CIS in 210 (19%) patients. 759 (71%) patients had high-grade tumors, and 324 (29%) patients had multifocal disease. Median follow-up for relapse-free survival (RFS) and progression-free survival (PFS) was 18, and 24 months, respectively, during which time 360 recurrence and 79 progression events occurred. Results showed a greater number of tumors (unit HR 1.09), and recurrent tumor status (HR 1.32) were associated with increased risk of recurrence, while pure CIS (HR 0.69) and receipt of perioperative intravesical chemotherapy (HR 0.76) were associated with decreased risk of recurrence. In contrast, higher tumor stage (HR 3.88 for T1; HR 3.69 for T1+CIS) and lymphovascular invasion (HR 3.88) were associated with increased risks of progression. Median follow-up for RFS and PFS was 18 and 24 months, respectively. The discrimination of the EORTC model was c=0.59 for RFS and c=0.67 for PFS, while for the CUETO model it was c=0.53 for RFS and c=0.72 for PFS. Thus, overall, in a contemporary cohort of patients undergoing BLC-TURBT, the EORTC and CUETO models demonstrated poor performance in predicting RFS and PFS. Among protocol-eligible patients, the pathological complete response (pCR) rates were 41% for the combination arm versus 20% for monotherapy. The combination was particularly effective in patients with low PD-L1 expression, where it achieved a pCR rate of 42% compared to just 11% with monotherapy. Meanwhile, APL-2401 — a dual FGFR2/3 small molecule inhibitor — has entered IND-enabling studies, with its findings selected for a poster presentation at AACR 2025. In addition, the investigational new drug (IND) application for APL-1202 in the treatment of free-living amoebae (FLA) infections was approved by the NMPA in June 2024. Clinical data presentations at the European Association of Urology (EAU) 2025 congress highlight the benefits of using Blue Light Cystoscopy in bladder cancer

OSLO, Norway, March 24, 2025 /PRNewswire/ -- Photocure ASA (OSE: PHO), the Bladder Cancer Company, announces two abstract presentations at the 2025 European Association of Urology congress (EAU) in Madrid, Spain, highlighting the benefits of Blue Light Cystoscopy (BLC®), notably its impact on recurrence risk and ability to help urologists make the best bladder cancer management choices for their patients. The EAU annual meeting is one of the largest international meetings in the urology calendar, showcasing the latest and most relevant clinical and scientific advancements in this area of patient care. This year's event was held on March 21-24, 2025, and attracted urologists from all over the world. Photocure participated with its Hexvix® product designed for better detection and resection of bladder tumors under the theme that "Seeing is believing". As in the past two years, Photocure will make 2025 EAU bladder cancer session highlights available to healthcare professionals after the event, by means of video interviews with the presenters of these sessions at the Photocure booth B28. This successful initiative is once again supported by two of the leading names in Bladder Cancer in Europe, Prof. M. Rouprêt, APHP, Sorbonne University Paris, France and Prof. P. Gontero, Division of Urology, University of Studies of Torino, Italy. In particular, two notable abstract presentations were: The impact of PDD on recurrence and progression in BCG-treated NMIBC* patients: a nationwide follow-up study (Abstract session 39: Sunday, March 23rd, 5:15pm CET – NMIBC: Advances in diagnosis and follow-up) This nationwide cohort study investigates the association of PDD on recurrence and progression risk in BCG-treated NMIBC patients, using Danish registry data from 2009-2022 assessing patients treated with BCG for primary NMIBC. In the study, 4318 patients from the Danish registry were identified with a first-time NMIBC diagnosis that were treated with BCG. Of the study subjects, 2388 (55%) of patients were diagnosed with the assistance of PDD. Basic demographics, stage and grade was comparable between the two groups. There were no differences in BCG-response across exposure groups. Age adjusted relative risk of recurrence was 0.88 (0.79 – 0.97) and 0.97 (0.89 – 1.05) at 1 and 5 years, respectively for the PDD-group compared to the non PDD-group. Age adjusted relative risk for progression was 0.93 (0.73 – 1.19) and 1.01 (0.84 – 1.21) at 1 and 5 years, respectively for the PDD-group. The authors conclude that, since the BCG-response across exposures were equal, the reduced risk of recurrence at 1 year was likely due to the detection effect of PDD: "The present study did not support the hypothesis of PDD modulating the BCG-response in NMIBC patients, suggesting a different mechanism for the impact of PDD on recurrence and progression in BCG-treated NMIBC patients." Read the abstract: Trans Urethral Laser Ablation of bladder tumors under local anesthesia: A prospective study of pain and recurrences (Abstract session 50: Monday, March 24th, 12:30pm CET – NMIBC: Innovations and outcomes 2) This study investigated the short- and long-term oncology safety regarding recurrence and progression as well as perioperative pain after office-based Transurethral Laser Ablation (TULA) or fulguration by PDD-assisted flexible cystoscopy. This prospective observational study enrolled eligible patients from August 2017 to June 2022 at Oslo University Hospital. 329 patients were enrolled, with a median age of 74 years. The median NRS (numeric rating scale) for perioperative pain was 4 (IQR: 3-5) for both the laser and fulguration group. The 12-month relapse-free survival (RFS) was 91% overall, 93% for fulguration, and 89% for laser ablation. The 12-month PFS was 94% for the laser group and 97% for the fulguration group. The authors conclude that office-based TULA under local anesthesia using PDD-assisted flexible cystoscopy is associated with low perioperative pain and does not compromise short- or long-term RFS or PFS. Therefore, it is a feasible option regarding perioperative pain and oncological safety.
