The aim of the study is to investigate whether patients with moderate-to-severe VKC and keratitis will benefit from the addition of Diquafosol ophthalmic solution in terms of symptoms and signs of VKC, tear film metrics, ocular surface inflammation and quality of life. Potential beneficial effect of Diquafosol treatment on top of current standard use of topical immunosuppressant therapy in VKC would be studied.
Subject at least aged 6 and above with clinical diagnosis of vernal keratoconjunctivitis would be invited to join this study and perform the following assessments:
1. best corrected visual acuity,
2. slit lamp biomicroscope examination,
3. tear meniscus height and non-invasive keratographic tear breakup time(NIKBUT) measurement,
4. cornea fluorescein staining (CFS) and
5. Schirmer's I test. Doctor will prescribe eyedrop, 3% Diquafosol ophthalmic solution and 0.1% Cyclosporin A cationic ophthalmic emulsion or 0.1% Cyclosporin A cationic ophthalmic emulsion at the end of examination. Subjects will be followed up at 4 weeks, 8 weeks, 16 weeks after treatment.
All participants will be required to answer a standardised questionnaire relating to severity of VKC and dry eye disease.

Comparison of Rebamipide Clear Solution and Diquafosol Eyedrops in Dry Eye Disease with Short Tear Break-up Time: A Retrospective, Comparative Observational Study

Purpose::

To compare the efficacy of 2% rebamipide clear solution and 3% diquafosol eyedrops in dry eye disease (DED) patients with short tear break-up time (tBUT).

Methods::

This nonrandomized retrospective, comparative observational study included 120 eyes (60 patients) divided into Group 1 (2% rebamipide, 60 eyes) and Group 2 (3% diquafosol, 60 eyes). Evaluations at baseline, 1 month, and 3 months included Ocular Surface Disease Index Questionnaire (OSDI), Schirmer I, noninvasive tBUT (NItBUT), corneal/conjunctival staining score, meiboscore, meibum expressibility and quality, lipid thickness, tear meniscus height (TMH), impression cytology, and goblet cell density. The primary endpoint was NItBUT change at 3 months.

Results::

Both groups showed significant improvement ( P < 0.025, Bonferroni correction) in DED signs and symptoms at 1 and 3 months compared with baseline. Changes from baseline were comparable between groups for OSDI, NItBUT, corneal and conjunctival staining score, meiboscore, meibum expressibility and quality, lipid thickness, impression cytology, and goblet cell density ( P > 0.05). At 3 months, group 2 showed greater increases in aqueous-related parameters than group 1 (for Schirmer I, adjusted mean differences 1.31 mm, 95% CI: 0.49–2.13, P = 0.002; for TMH, adjusted mean differences 0.05 mm, 95% CI: 0.01–0.09, P = 0.014).

Conclusion::

In patients with short tBUT DED, both rebamipide clear solution and diquafosol eyedrops showed comparable improvement in the primary endpoint, NItBUT at 3 months, as well as in other ocular surface parameters. Under standard dosing regimens, however, diquafosol was associated with greater increases in Schirmer I test values and TMH than rebamipide clear solution.

The neuro-immune-epithelial axis in dry eye: From pathogenic crosstalk to neuromodulatory therapies

Dry eye disease (DED) is commonly described through tear-film instability, tear deficiency, evaporative stress, and ocular surface inflammation. These models remain central to diagnosis and treatment, but they do not fully explain symptom-sign discordance, persistent discomfort after surface-directed therapy, epithelial disease with reduced corneal sensation, or variable treatment responses. In this structured narrative review, we synthesize DED-specific, phenotype-specific, adjacent ocular surface, and translational evidence relevant to a bidirectional neuro-immune-epithelial axis involving corneal sensory nerves, trigeminal-parasympathetic reflexes, epithelial and goblet-cell barrier function, and immune-cell activation. Available sensory evidence indicates that corneal sensory abnormalities in DED are heterogeneous: mechanical esthesiometry more often shows reduced sensitivity, whereas non-contact esthesiometry and pain-enriched cohorts may show hypersensitivity or mixed findings. In vivo confocal microscopy (IVCM) studies suggest concurrent changes in corneal nerve morphology and epithelial immune-cell density, but validated thresholds are not sufficient to define a routine neural DED subtype. Neural dysfunction is therefore considered a phenotype-dependent contributor or amplifier, rather than a universal nerve-first mechanism. Treatment implications are discussed according to evidence strength and directness. Varenicline nasal spray and acoltremon engage reflex or sensory tear pathways where approved; motugivatrep illustrates region-specific TRPV1 translation in Japan; diquafosol and rebamipide are epithelial/mucin-supportive therapies with indirect axis relevance; and cenegermin remains primarily a neurotrophic keratitis therapy with restricted relevance to selected DED phenotypes. Neuropeptide-specific targets, availability-limited device stimulation, and direct nerve-regenerative claims for photobiomodulation remain preliminary. This framework may support mechanism-based stratification and rational adjunctive therapy without replacing established DED classifications.

Efficacy of diquafosol 3% in preventing and treating dry eye associated with pediatric orthokeratology

PURPOSE:

To evaluate the efficacy of diquafosol 3% in preventing and treating dry eye associated with orthokeratology in children.

METHODS:

First-time pediatric orthokeratology wearers were enrolled in this prospective, randomized clinical trial. Participants were randomized into the treatment group (3% diquafosol sodium four times daily from the prefitting period) and control group (no additional treatment). Noninvasive tear film parameters (first break-up time [NIBUT], average NIBUT, tear meniscus height, and lipid layer thickness), corneal staining grade, uncorrected visual acuity (UCVA), and Ocular Surface Disease Index (OSDI) were compared at baseline, before lens wear, and at 1 week, 1 month, and 3 months.

RESULTS:

A total of 64 children (128 eyes) completed the study. During the prefitting period, all tear film parameters improved significantly in the treatment group (all P < 0.001). At 3 months, the treatment group demonstrated significantly higher first NIBUT, average NIBUT, tear meniscus height, and lipid layer thickness (all P < 0.001), with lower OSDI scores (P < 0.05) and higher UCVA (P < 0.001). Corneal staining was significantly lower in the treatment group at 1 week (P < 0.05) but not at 1 or 3 months. Mild adverse events included transient discharge (3 children, treatment group) and conjunctival congestion (5 eyes, control group). There were no serious adverse events.

CONCLUSIONS:

Prophylactic and therapeutic use of diquafosol 3% improved tear film stability, tear secretion, and dry eye symptoms in children undergoing orthokeratology. Early initiation during the prefitting period may establish a more favorable ocular surface before lens wear.

Yuyu Pharma, Dry Eye Pipeline Mechanism of Action and Pre-clinical Results Published

SEOUL, South Korea, April 27, 2023 /PRNewswire/ -- At ARVO 2023, Yuyu Pharma unveiled the results of its mechanism of action and head-to- head studies for its dry eye treatment peptide (project name: YP-P10). ARVO is the world's largest ophthalmic society and held its annual meeting in New Orleans, Louisiana, USA in April of this year. A study conducted by Dr. Virginia Calder of the University College London (UCL) found that YP-P10 significantly reduces inflammatory Th2 and Th17 cells. In particular, the YP-P10 model reduced Th17 cells 25% more effectively when compared to the control group. The YP-P10 pre-clinical head-to-head study, jointly conducted by five people including Dr. Kyungho Park of Naason Science, showed statistically significant reduction of corneal damage by measuring Corneal Fluorescein Staining (CFS). The study compared the efficacy of YP-P10 to that of several competing dry eye treatment products such as Lifitegrast, Cyclosporine, Diquafosol, and Hyaluronic Acid. The results showed that YP-P10 had 12% better efficacy than Lifitegrast, 7% better than Cyclosporine, 9 % better than Hyaluronic Acid, and showed similar efficacy to Diquafosol.
