A Phase IV Clinical Study of Evaluating the Safety and Efficacy of Icatibant Injection (Icanticure) in the Treatment of Patients With Bradykinin Induced Angioedema

This is a phase IV clinical study. The investigators collect patients with a diagnosis of bradykinin induced angioedema. For a specific study period and prospectively arrange, the eligible patients who has an acute attack of angioedema could be treated by icatibant Injection (Icanticure®).
The drug, icatibant injection has been reimbursed by National Health Insurance Agency, Taiwan (NHI) in the treatment of diagnosed patients with hereditary angioedema. However, the reimbursement is still limited for some patients with atypical conditions. Therefore, Nang Kuang Company initiates a phase IV case collection study and supplies Icanticure® for free during the study period. Patients who still unable to obtain NHI's reimbursed icatibant drug is eligible for the inclusion, enter into the study and can be treated by Icanticure® to meet their clinical urgent needs. The safety and efficacy of Icanticure® are evaluated by the prospective, planed assessments in the enrolled patients.

Severe Acute Respiratory Syndrome Coronavirus 2 (SARS-CoV-2) disrupts multiple host regulatory systems, including the kallikrein-kinin system (KKS), which plays a central role in vascular homeostasis and inflammation. Following viral entry, angiotensin-converting enzyme 2 (ACE2) is internalized and inactivated, impairing the degradation of des-Arg⁹-bradykinin (DABK), a selective agonist of the inducible kinin B1 receptor (B1R), while inflammatory feedback mechanisms may further elevate bradykinin (BK), the kinin B2 receptor (B2R) agonist. It has been postulated that the kinin storm observed in COVID-19 patients may lead to pulmonary angioedema, promote the exacerbated production of pro-inflammatory cytokines, and be related to disease progression, permanent sequelae, and death. However, it remains unknown whether in vitro SARS-CoV-2 infection triggers alterations in the KKS and its downstream inflammatory and vasoactive mediators. Here, we investigated how SARS-CoV-2 infection modulates kinin signaling and its downstream inflammatory and vasoactive mediators in Vero E6 and human endothelial (HUVEC) cells. SARS-CoV-2 infection increased BK levels and markedly upregulated B1R gene expression in Vero E6 cells (up to 20-fold), while B2R expression remained unchanged. Both BK and DABK reduced ACE2 expression in Vero E6 cells, an effect most likely mediated by DABK-B1R signaling due to the minimal expression of B2R in this cell line. Viral infection induced robust production of nitric oxide (NO) and interleukin-6 (IL-6) in Vero E6 cells, and supernatants from infected cells triggered similar responses in HUVECs. Pharmacological blockade of B1R with R-715 selectively prevented SARS-CoV-2-induced NO production in Vero E6 cells, whereas IL-6 induction occurred through B1R-independent mechanisms. In contrast, in endothelial HUVEC cells exposed to serum from patients with severe COVID-19, B2R antagonism with HOE-140 prevented both NO and IL-6 production, suggesting a key role for B2R in both vascular cytokine amplification and vasoactive mediator production. Our findings reveal a compartment-specific and complementary activation of B1R and B2R during SARS-CoV-2 infection, establishing the KKS as a key mediator of endothelial alterations and inflammatory amplification in COVID-19 and positioning B1R and B2R as therapeutic targets.

Angioedema reported with tissue plasminogen activators: A need to communicate on bradykinin- and histamine-mediated mechanisms

AIM OF THE STUDY:

Angioedema (AE) is a potentially life-threatening adverse effect. The mechanism can be either bradykinin- (BK-AE) or histamine-mediated (Hi-AE), each requiring different management. AE are reported with recombinant tissue plasminogen activators (alteplase and tenecteplase), for which an immunoallergic or bradykinic mechanism is suggested. The aim of this study was to describe the characteristics of cases of AE (BK or Hi) reported with rt-PA to the French Regional Pharmacovigilance Centers in order to optimize their identification and management.

METHODS:

All cases of AE reported with alteplase or tenecteplase until 30 November, 2023 were analyzed and classified as Hi-AE or BK-AE using the clinical manifestations, the management and the allergological workup.

RESULTS:

We analyzed 151 cases of AE, with a median time to onset of 1h (IQR=1-2h), all localized at the head. From them, 150 were with alteplase use for acute ischemic stroke (98%) or myocardial infarction (2%), and 1 with tenecteplase. Of them 107 (71%) could be clearly categorized and were classified as 98 (92%) cases of BK-AE and 9 (8%) cases of Hi-AE. When the use of icatibant or C1 esterase inhibitor is specified, it was the 1st-line treatment for 32/47 (68%) patients.

CONCLUSION:

The majority of cases of AE reported with rt-PA, are BK-AE involving alteplase. This is consistent with the effects of rt-PA, which activates the kinin pathway by hydrolyzing plasminogen to plasmin, resulting in the formation of bradykinin. Clinicians need to be informed of this mechanism, in order to early prescribe the appropriate treatment, as we detect that it was not systematically used as 1st-line therapy for a lot of cases.

Clinical features of hereditary angioedema involving the gastrointestinal tract: A retrospective analysis

Background:

Hereditary angioedema (HAE) is a rare, severe, disabling, and life-threatening disorder characterized by recurrent and unpredictable edema of skin and mucous membranes. Gastrointestinal edema, often presenting as abdominal pain, is common but frequently misdiagnosed, leading to unnecessary surgeries. This study aims to characterize gastrointestinal edema in HAE patients to improve early diagnosis and guide treatment.

Method:

We analyzed 61 patients with HAE who were admitted in our hospital between April 1, 2023 and August 31, 2025. Data on demographic characteristics, clinical manifestations, laboratory findings, imaging results, and treatment outcomes were collected through questionnaires and hospital electronic records.

Result:

Among the patients, 86.9% experienced gastrointestinal edema, mostly characterized by abdominal pain (94.3%), nausea (79.2%), and diarrhea (71.7%). Misdiagnosis occurred in 73.6% of cases, and 22.6% underwent unnecessary surgeries. The median age of gastrointestinal edema onset was 16.0 years, with the most severe episodes occurring at a median age of 25.0 years. The median annual frequency of attacks was 4.0/year, with a median severity score of 7.0. Acute attacks showed elevated D-dimer, white blood cells, neutrophils, and hemoglobin. CT imaging frequently revealed intestinal wall thickening and abdominal-pelvic effusion. Lanadelumab demonstrated superior efficacy over danazol in reducing both the frequency and severity of edema attacks, while icatibant significantly shortened the duration of edema episodes.

Conclusion:

This study updates the clinical and imaging features of gastrointestinal edema in Chinese patients with HAE, identifies biomarkers of gastrointestinal edema attacks, and highlights the efficacy of lanadelumab and icatibant, aiding timely diagnosis and treatment.

Evidence of Efficacy and Safety of Deucrictibant for Prophylaxis and On-Demand Treatment of Hereditary Angioedema Attacks Published Back-to-Back in The Lancet Haematology

(Nasdaq: SEPN), a clinical-stage biotechnology company pioneering oral small molecule GPCR-targeted medicines, today announced positive results from its Phase 1 clinical trial evaluating SEP-631, a potent and selective oral negative allosteric modulator (NAM) of Mas-related G protein-coupled receptor X2 (MRGPRX2). Based on these results, Septerna plans to advance SEP-631 into Phase 2 development for chronic spontaneous urticaria (CSU) and continue evaluation of additional mast cell-driven indications. “These data mark an important milestone for the SEP-631 program and more broadly for our Native Complex Platform®, which enables new approaches to GPCR drug discovery by reconstituting functional GPCR complexes outside of cells,” said Jae Kim, M.D., chief medical officer of Septerna. “Importantly, the robust inhibition of icatibant-induced wheal formation observed is consistent with the differentiated insurmountable NAM mechanism we characterized preclinically and provides clinical proof-of-mechanism for the MRGPRX2 pathway. Overall, we believe the strength and consistency of these results position SEP-631 as a potentially differentiated oral treatment for patients living with mast cell-driven diseases. With a clear path into Phase 2 development in chronic spontaneous urticaria, we are focused on translating this early validation into meaningful outcomes for patients and exploring additional high-need indications where MRGPRX2 biology is implicated.” Phase 1 Results SEP-631 was evaluated in a randomized, double-blind, placebo-controlled Phase 1 trial in healthy volunteers, including single-ascending dose, multiple-ascending dose and food-effect cohorts. The study assessed safety, tolerability, pharmacokinetics (PK) and pharmacodynamic (PD) activity. SEP-631 was well-tolerated across all doses studied: Adverse event profile comparable to placeboNo severe or serious adverse eventsNo clinically meaningful laboratory or ECG abnormalities SEP-631 demonstrated a PK profile supportive of convenient once-daily oral dosing, including: Half-life of approximately 24 hoursNo clinically meaningful effect of food on exposure, supporting dosing without food restrictions Pharmacodynamic activity was assessed using icatibant-induced skin wheal formation, an established skin test used to measure mast cell activation and target engagement, as a translational model of MRGPRX2-mediated mast cell activation. Use of short-wave infrared imaging technology enabled accurate and precise measurement of the skin test wheals. SEP-631 produced robust suppression of wheal formation across evaluated dose levels, with complete inhibition observed at doses as low as 10 mg once daily following the 10 µg/mL icatibant challenge. Following the 100 µg/mL icatibant challenge, inhibition was dose-dependent, with progressively greater suppression observed at increasing SEP-631 doses with near to complete inhibition achieved at 90 and 200 mg once daily. These findings are consistent with the insurmountable NAM mechanism of SEP-631 observed preclinically and support potent target engagement and functional blockade of MRGPRX2 signaling in humans, providing clinical proof-of-mechanism. Phase 2 Development Strategy Septerna plans to initiate a Phase 2b clinical trial of SEP-631 in chronic spontaneous urticaria (CSU) in the second half of this year, following the completion of ongoing long-term toxicology studies. The planned Phase 2b study will be a randomized, double-blind, placebo-controlled, global trial evaluating once-daily oral SEP-631 in adult patients with moderate-to-severe CSU who remain symptomatic despite treatment with second-generation antihistamines. Following the CSU study initiation, the company also plans to pursue an open-label study in chronic inducible urticaria (CIndU), specifically in patients with symptomatic dermatographism. Beyond urticaria, Septerna is evaluating additional mast cell-driven diseases with high unmet medical need and evidence of MRGPRX2 expression, and has initially prioritized atopic dermatitis, interstitial cystitis, migraine and asthma for further assessment.
