In this study, the investigators will recruit a cohort of elderly Black and Hispanic patients with heart failure to define the number of patients who have cardiac amyloidosis by utilizing highly sensitive heart imaging and blood tests. The investigators will also explore differences in genetics and sex as they relate to heart failure disease progression in cardiac amyloidosis.

Feasibility study of radioactivity estimation of 99mTc and 123I-labeled radiopharmaceuticals using shielded syringes

This study investigates the feasibility of estimating the radioactivity of radiopharmaceuticals using shielded syringes. The radioactivities of ^99mTc-MDP, ^99mTc-HMDP, ^99mTc-ECD, ^99mTc-MAG₃, and ¹²³I-IMP were measured using a dose calibrator. Correlation coefficients and regression equations were obtained from the radioactivity in the shielded and unshielded syringes. ^99mTc-MDP was also measured for residual radioactivity after the administration. The correlation coefficients of ^99mTc-MDP, ^99mTc-HMDP, ^99mTc-ECD, ^99mTc-MAG₃, and ¹²³I-IMP were r_s = 0.9998, r_s = 0.9997, r_s = 0.9999, r_s = 0.9998, and r_s = 0.9888, respectively. The regression equations were y = 0.0364x + 0.0913, y = 0.0349x + 0.0273, y = 0.0343x - 0.0018, y = 0.0522x + 0.1215, and y = 0.0383x + 0.0058, respectively. The correlation coefficient for the residual radioactivity of ^99mTc-MDP was r_s = 0.9887 and the regression equation was y = 0.1505x + 0.0853. The radioactivity of ^99mTc- and ¹²³I-labeled radiopharmaceuticals in shielded syringes was accurately measured. It was suggested that the measuring shielded syringes could provide an estimate of the actual radioactivity.

Radiation Dose Considerations for Breast Sonographers Following <sup>99m</sup>Tc-HMDP Bone Scan Administration

PURPOSEPatients who were administered radiopharmaceuticals can be a source of radiation exposure to sonographers. This study aimed to identify factors associated with radiation exposure to breast sonographers from patients administered radiopharmaceuticals for bone scanning.METHODSThe exposure dose of six sonographers was measured during breast sonography in 59 patients administered ^99mTc-HMDP. We predicted the following factors to be related to exposure dose: time interval between administration and sonography, sonography examination time, estimated radioactivity at sonography, sonographer's years of experience, and patients' clinical data (age, renal function and surgical procedure). Spearman's rank correlation coefficient was used to examine the relationship between radiation dose and the aforementioned factors.RESULTSThe mean±standard deviation of the exposure dose for the sonographers was 9.3±3.8 µSv. The time interval between administering the radiopharmaceutical agent and sonography, the sonography examination time and estimated radioactivity at sonography were found to be factors related to the exposure of the sonographer. The exposure dose increased as a function of the shorter time interval, longer examination time and higher estimated radioactivity at sonography.CONCLUSIONThe time interval between drug administration and sonography, sonography examination time and estimated radioactivity at sonography contributed to the increased dose to breast sonographers. Although we considered that the exposure dose of sonographers would not possibly exceed the dose limit in the present study, we suggested that radiological technologists need to educate the physicians requesting sonography, and the sonographers about the radiation exposure in nuclear medicine.

ALK-positive histiocytosis associated with chronic lymphocytic leukaemia/small lymphocytic lymphoma: a multitarget response under ibrutinib

ALK-positive histiocytosis is a recently described entity with few reported cases in literature. Herein, we report an unusual case of ALK-positive histiocytosis showing an Erdheim-Chester disease (ECD)-like presentation, occurring in a 37-year-old woman with a 2-year history of chronic lymphocytic leukaemia (CLL). Our CLL patient relapsed 6 months after the end of fludarabine, cyclophosphamide and rituximab frontline therapy and complained of lower limb pains. A bone marrow biopsy was performed and showed concomitant CLL/small lymphocytic lymphoma and ALK-positive histiocytosis with an identical immunoglobulin heavy-chain gene rearrangement in both neoplasms, suggesting clonal relationship. After 4 years under ibrutinib therapy, our patient remains free of both diseases. This report extends the spectrum of composite hematolymphoid neoplasms and shows that ALK rearrangement should be considered in all histiocytosis subtypes. Moreover, both tumours eradication under ibrutinib suggests that BTK inhibitors may also be effective in histiocytic neoplasms.
