Delving into the Latest Updates on Testosterone with Synapse

This is a monocentric, single-arm prospective pilot study that will enrol hypogonadal (testosterone < 231 ng/dL) male subjects in best supportive care, with no further therapeutic options and no need for resuscitation.
Currently, testosterone formulations for intramuscular (IM) injection and subcutaneous injection as well as for oral and transdermal administration have been approved for androgen therapy. To date, injectable testosterone is the most commonly used formulation. To increase serum testosterone levels to the physiological range IM injections of testosterone every 2-3 weeks are required, which lead to supraphysiological peaks shortly after administration, followed by a sharp fall in levels thereafter. Testosterone levels before the next injection are frequently in the hypogonadal range. For this reason, Sustanon 250® (1 ml, IM) will be administered on day 0 after confirmation of hypogonadism by blood test and then every 15 days in this trial given the limited life expectancy of the subjects and to maximise the effect and benefit of testosterone supplementation.
The Edmonton questionnaire and ADL questionnaire will be completed before the injection on day 0 and then every 15 days at the time of injection until subject's death or if a subject is discontinued from the study treatment/procedures for any other reasons than death. In parallel, the EQ-5D-3L questionnaire will be completed by a family member or a proxy.

Pharmacokinetic, Clinical, Hormonal and Therapeutic Effects of the Use of Estradiol and Testosterone Hormonal Subdermal Implants in Women With Natural Menopause, Premature Ovarian Failure or Surgical Menopause Due to Cervical Cancer.

Estrogen and androgen deficiencies negatively impact the quality of life of women at different stages of life, especially after menopause. New modalities and new therapeutic alternatives have been researched. Parenteral administration of estradiol and testosterone could be effective to treat symptoms secondary to estrogen and androgen deficiencies and minimize these adverse events. This study evaluates the efficiency of subdermal implant-bioabsorbable use in women with menopausal symptoms associated with secondary estrogen and androgen deficiencies in women with natural menopause, premature ovarian failure or surgical menopause due to cervical cancer. Pharmacokinetic, biochemical, metabolic, thromboembolic and hormonal data will be evaluated, as well as the effects on quality of life, menopausal symptoms and sexual function after treatment.

Serum uric acid is associated with ovarian reserve and neonatal outcomes in women undergoing IVF/ICSI: observational and genetic evidence

Elevated serum uric acid (SUA) has been linked to adverse outcomes in polycystic ovary syndrome, yet its broader role in female fertility is unclear. This retrospective cohort study of 16,223 women undergoing a first in vitro fertilisation (IVF)/intracytoplasmic sperm injection (ICSI) cycle investigated associations between pre-treatment SUA and reproductive parameters. Higher SUA levels were independently associated with elevated luteinizing hormone (LH), testosterone, and anti-Müllerian hormone (AMH), and with lower estradiol and follicle-stimulating hormone. Each 50 µmol/L SUA increase correlated with a 16-gram reduction in neonatal birth weight and a 0.06-week decrease in gestational age, with a threshold effect near 350 µmol/L. Bidirectional Mendelian randomisation analyses supported a causal effect of elevated SUA on increased LH and testosterone, while reverse analysis suggested lower AMH may elevate SUA. These findings identify SUA as a biomarker influencing ovarian reserve and neonatal outcomes through bidirectional mechanisms, including a potential SUA-AMH feedback loop. Routine SUA monitoring in fertility treatment is supported, and interventions for individuals with SUA ≥350 µmol/L warrant clinical investigation.

Curcumin targets miR-21-3p: promising therapeutic strategy for treatment of benign prostatic hyperplasia

BACKGROUND:

It is still unclear if curcumin's therapeutic effect in benign prostatic hyperplasia (BPH) is linked to microRNA regulation. This study explored the potential role of miR-21-3p in curcumin-induced anti-inflammatory and antiproliferative effects in BPH.

METHODS AND RESULTS:

Twenty-four male adult rats were grouped randomly into four groups: normal control group, BPH group, BPH group treated with curcumin and BPH group treated with finasteride as a reference drug. The BPH model was experimentally induced by s.c. injection of testosterone enanthate (3 mg/ Kg) five times a week for two weeks, curcumin and finasteride were given orally, parallel to testosterone injection. The results showed that curcumin-induced decrease in prostate index and prostate-specific antigen (PSA)-like protein expression were associated with downregulation of miR-21-3p compared with the BPH untreated group. Immunohistochemical staining revealed increased SIRT1 expression and decreased NF-κB and TNF-α expression in the curcumin-treated group compared to the BPH untreated group. In addition, β-catenin protein expression, measured by Western blotting, as well as β-catenin-linked signaling proteins, LRP6 and c-Myc, were suppressed in the curcumin group. The results obtained with curcumin treatment were comparable to those with finasteride treatment. Our data were supported by histopathological findings.

CONCLUSIONS:

The current study demonstrated that curcumin alleviated the inflammatory manifestations of BPH by targeting the pro-inflammatory microRNA miR-21-3p, thereby upregulating SIRT1 and downregulating the pro-inflammatory mediators NF-κB and TNF-α. Furthermore, the anti-proliferative effect of curcumin may be attributed to the inhibition of the β-catenin signaling pathway.

The relationship between testosterone replacement therapy and incidence of proximal humerus fractures in men: a matched retrospective analysis

Background:

Anabolic androgenic steroid supraphysiologic dose use is linked to an increased risk of tendon rupture. Testosterone replacement therapy (TRT) is used to treat hypogonadism and is associated with increased bone mineral density. However, its relationship with fracture risk, particularly proximal humerus fractures (PHF), remains uncertain. This study evaluates the association between TRT and the incidence of PHFs using a large national database. We hypothesized that TRT use would be associated with a difference in the incidence of PHFs compared to a control group due to testosterone's role in maintaining bone mineral density.

Methods:

This retrospective study with one-to-one matching queried the PearlDiver Mariner165 dataset to obtain a random sample of 500,000 patients aged 35 to 75 who received TRT continuously for at least 3 months and a random control sample of 500,000 different patients. Of the targeted 1,000,000 patients, 335,753 pairs of patients were matched based on age, tobacco use, diabetes history, and the Charlson Comorbidity Index. Incidence of PHF was assessed over a 2-year follow-up using the International Classification of Diseases codes, with comparisons made across the different cohorts. Multivariable logistic regression was conducted to identify the association between TRT use with PHF in men. An alpha level of 0.01 was prespecified to reduce the risk of type I error and ensure statistical rigor.

Results:

In this matched-cohort study of 335,753 paired TRT and control patients, the mean age was 53.82 ± 10.58 years. Patients administered TRT demonstrated a higher incidence of PHF compared to the control cohort (0.029% vs. 0.005%, P < .001). Logistic regression analysis demonstrated that TRT was associated with a significantly increased risk of PHF (adjusted odds ratio 3.14; 95% confidence interval: 1.84-5.59; P < .001).

Conclusions:

In this retrospective matched-cohort study, TRT patients demonstrated an increase in 2-year incidence rates of PHF in men. These findings underscore the need for tailored patient management and provide actionable insights for orthopedic practice. Future studies of prospective design are needed to better address confounding factors, establish causation, and evaluate surgical outcomes in TRT patients.

NHS to offer thousands of men life-extending prostate cancer drug

Thousands of men with prostate cancer will be offered a life-extending drug on the NHS within weeks. For the first time patients whose prostate cancer has not spread will be able to receive the drug in England, known as abiraterone, as the health service widens access to the treatment. Around 2,000 men diagnosed with prostate cancer in the last three months whose cancer has not spread will get access to the treatment if it is of clinical benefit. An additional 7,000 men are expected to be diagnosed with prostate cancer each year and will be eligible for the drug, which stops the cancer spreading by starving the disease of the hormones it needs to grow, such as testosterone. Research has shown that for these earlier stage patients, the survival rate after six years is improved – with trials showing that the proportion of men alive after six years on abiraterone was 86% compared to 77% on standard treatment (hormone therapy with or without radiotherapy). NHS England has been able to give the green light to the drug for thousands more eligible patients thanks to the health service buying and delivering treatments at better value, following the clinical advice to roll this out last year. The NHS has set a target to save over £1 billion on clinically effective biosimilar drugs during this parliament – more than eight in 10 drugs the NHS now prescribes are lower cost biosimilar or generic drug – creating available funding for other treatments. The NHS in England already commissions abiraterone, a drug now available as a lower cost generic medicine, for advanced prostate cancer, having introduced a policy to commission the treatment in December 2024 – nearly one year ahead of positive NICE guidance recommending it in November 2025. Menopause Hormone Therapy Does Not Impact Risk for MCI, Dementia

TUESDAY, Dec. 30, 2025 -- There is no evidence that menopause hormone therapy (MHT) use is associated with the risk for dementia in postmenopausal women, according to a study published online Dec. 22 in The Lancet Healthy Longevity . Melissa Melville, from University College London, and colleagues conducted a systematic review to examine whether MHT versus no MHT affects the risk for mild cognitive impairment (MCI) or dementia in perimenopausal or postmenopausal women, including those with premature ovarian insufficiency or early menopause. Ten studies (one randomized controlled trial and nine observational studies), with 1,016,055 participants were included. The certainty of evidence varied from moderate to very low in the included studies. The researchers found that none of the studies examined testosterone or use in premature ovarian insufficiency. There was no significant association between MHT and MCI or dementia risk. No significant effects were seen in subgroup analyses by timing, duration, or type of MHT. "To cut through the noise, we reviewed the most rigorous research there is on the subject and found that menopause hormone therapy does not appear to impact dementia risk either positively or negatively," senior author Aimee Spector, Ph.D., also from University College London, said in a statement. "This review will help to inform the upcoming WHO [World Health Organization] guidelines on reducing the risk of cognitive decline and dementia, which are expected to be released in 2026."
