Indonesia is one of country that contributes the most cases of tuberculosis worldwide. Tuberculosis is the most commonly etiology of exudative pleural effusion. There have been many studies about undiagnosed exudative pleural effusions, but there are not many studies about the use of medical thoracoscopy for diagnosing transudative and exudative pleural effusion, especially on biomarkers of C-Reactive Protein (CRP), D-dimer, Adenosine Deaminase (ADA), Antinuclear Antibody (ANA), C3 C4 complements, Cancer Antigen 125 (CA-125), Xpert Mycobacterium Tuberculosis (Xpert MTB), Lupus Erythematosus cell (LE cell), cytology (effusion and smear) and histopathology. Information gained from those biomarkers via thoracocentesis and medical thoracoscopy, etiology of exudative and transudative pleural effusion can be detected earlier and clearly, especially etiology of infection, autoimmune, and malignancy that further can be used to reduce patients' hospitalization period, mortality, and to develop the new therapeutic agents.

Development and Validation of a Gas Chromatography‐Mass Spectrometry Method for the Determination of Fentanyl and Butyryl Fentanyl in Oral Fluid

ABSTRACTSynthetic opioids are lab‐synthesized substances that target the brain's opioid receptors, offering analgesic and sedative effects. Amongst them, fentanyl is one of the most widely used to intervene in chronic pain in moderate to severe cancer situations. Butyryl fentanyl (BF) is a novel synthetic opioid whose use is growing. Its potency is seven times that of morphine and, unlike fentanyl, BF can only be obtained through illegal sources. Fentanyl and its analogues are related to harmful intoxications and an increase in opioid‐related mortality in many countries, such as in the United States and Europe in recent years. This work developed and validated an effective and sensitive method based on solid‐phase extraction followed by gas chromatography‐mass spectrometry (GC‐MS) for the determination of fentanyl and BF in oral fluid samples. To the best of our knowledge, it is the first successful attempt to quantify these analytes using GC‐MS with a limit of quantification (LOQ) of 1 ng/mL in OF. Intra‐day and inter‐day percentage coefficient of variation were found within 1%–3% and 1%–14%, respectively, while accuracy ranged from 92% to 102% at four concentration levels (lower LOQ [LLOQ], 3, 20, 40 ng/mL) in accordance with the established criteria. The absolute recovery values were in the range of 80.0%–100.0%. The method was linear for all analytes, with quadratic regression of calibration curves always higher than 0.99. The validated method demonstrated its great potential to detect and quantify fentanyl and its analogue in OF and it can be useful not only in forensic investigations of addiction histories but also in epidemiological studies on the spread of fentanyl and BF among workers and/or drivers.

Femtosecond laser-induced Au nanostructure-decorated with plasmonic nanomaterials for sensitive SERS-based detection of fentanyl

Fentanyl and its analogs have emerged as the main factor behind the ongoing opioid abuse globally in recent years. However, the existing techniques for sensitive and accurate detection of fentanyl are often complex, laborious, expensive, and restricted to central healthcare facilities. We reported herein a plasmonic biochip fabricated by the femtosecond laser-induced nanostructures and plasmonic nanomaterials for sensitive SERS-based detection of fentanyl. Yolk-shell structured plasmonic nanomaterials are employed owing to their unique optical properties. The femtosecond laser direct writing technique creates three-dimensional silicon nanostructures followed by gold deposition and the immobilization of plasmonic nanomaterials. This SERS biochip fabricated by the femtosecond laser-induced nanostructure decorated with yolk-shell structured plasmonic nanomaterials enables the rapid and sensitive detection of fentanyl with the limit of detection of 3.33 ng/mL.

Prevalence of new psychoactive substances and drugs of abuse in the hair of individuals diagnosed with substance use disorder: Polydrug and emerging pattern of consumption

AbstractPeople diagnosed with substance use disorder (SUD) might represent a high‐risk subpopulation for New Psychoactive Drugs (NPS) consumption, and hair analysis offers a unique perspective to assess drug prevalence in this population. The present study aims to assess the prevalence of NPS and their co‐consumption with traditional drugs of abuse (DoA) in individuals diagnosed with SUD. Hair samples from patients under care at the addiction treatment service of Bologna, Italy, for a diagnosed SUD, were collected during 2023 and analyzed by ultra‐high‐performance liquid chromatography‐mass spectrometry (UHPLC–MS/MS), using a previously validated method. Among the 88 patients included, 95.5% tested positive for at least one substance, of which 88.1% for traditional DoA only, and 11.9% for NPS in addition to DoA. Among the positive samples, patients were found positive for more than two drugs in 67.9% of cases. The combination of DoA and NPS was more frequent in the younger age group (<21–30 years old, compared to 31–70, p = 0.025). Ketamine was detected in 8.0% of all samples, with mean hair levels 49.68 pg/mg (ranging 8.55–81.90 pg/mg) and was frequently accompanied by cocaine (85.7% of cases). Fentanyl was detected in 3.4% of all samples, while, among NPS, buphedrone was the only one detected. Our retrospective study highlights that the consumption of NPS is relatively low compared to other vulnerable or high‐risk populations. However, the prevalence of polydrug consumption and the high rate of ketamine–cocaine combination warrant careful monitoring even in this population.
