A pharmacovigilance analysis of carbapenem-related utilizing the FDA adverse event reporting system (FAERS) database from 2013 to 2025

Abstract:
Background:
Carbapenem antibiotics are critical therapies for multidrug-resistant infections, but their comparative haematologic safety profiles remain poorly characterized. We systematically evaluated haematologic adverse events associated with meropenem, ertapenem, imipenem and doripenem using pharmacovigilance data.
Methods:
We analyzed the FDA Adverse Event Reporting System (FAERS) database from January 2013 to July 2025. Disproportionality analysis was performed using reporting odds ratios (RORs) with 95% confidence intervals for haematologic adverse events.
Results:
Sixteen distinct haematologic adverse events were identified. Meropenem demonstrated the most extensive toxicity profile with significant signals for 15 adverse events, including severe conditions: thrombocytosis (ROR = 24.941), eosinophilia (ROR = 24.663), bone marrow failure (ROR = 13.113), agranulocytosis (ROR = 11.57) and pancytopenia (ROR = 11.131). Imipenem demonstrated signals for nine events, notably thrombocytopenia and eosinophilia. Ertapenem demonstrated nine events: leucopenia (ROR = 3.843) and neutropenia (ROR = 2.469). No reports were identified for doripenem.
Conclusions:
Significant differences exist in haematologic safety profiles among carbapenems. Meropenem shows the broadest spectrum of severe toxicity, while other carbapenems demonstrate fewer reports. demonstrates fewer reported haematologic safety signals and not a most favourable haematologic safety profile. These differential safety profiles should inform further studies on carbapenem selection, including patients with baseline haematologic disorders.

Development of two methods for detecting Streptococcus mitis/oralis-related species with meropenem-non-susceptibility

Several studies have reported clinical isolates of Streptococcus mitis, Streptococcus oralis, and their related species (S. mitis/oralis group) with reduced carbapenem susceptibility. However, to date, no simple and accurate methods of detecting S. mitis/oralis group isolates with reduced carbapenem susceptibility have been reported. The meropenem ETEST misinterpreted 3 of 25 (12 %) meropenem-non-susceptible S. mitis/oralis group isolates as susceptible to meropenem. Therefore, we screened 38 disks containing 38 different β-lactams to detect S. mitis/oralis group strains with reduced carbapenem susceptibility using the disk-diffusion method. Additionally, using the agar dilution method to determine susceptibility, we identified the optimal antibiotic concentrations in agar medium to enable selective culture of meropenem-non-susceptible S. mitis/oralis group by culturing 25 meropenem-non-susceptible (meropenem minimum inhibitory concentration [MIC] >0.5 μg/mL) and 24 meropenem-susceptible (meropenem MIC ≤0.5 μg/mL) S. mitis/oralis group isolates on agar containing different concentrations of six β-lactams. Meropenem-susceptible and non-susceptible S. mitis/oralis group isolates were successfully differentiated using oxacillin- and cefoxitin-containing disks. Moreover, cefdinir-, flomoxef-, meropenem-, and doripenem-containing disks showed clear differences in the growth inhibitory zone diameter between the meropenem-susceptible and meropenem-non-susceptible S. mitis/oralis group isolates. Media containing 2 μg/mL oxacillin, 4-16 μg/mL cefoxitin, 2 μg/mL cefdinir, 2-4 μg/mL flomoxef, 0.06-0.25 μg/mL meropenem, and 0.06-0.12 μg/mL doripenem allowed selective culture of meropenem-non-susceptible S. mitis/oralis group strains. The disk-diffusion method using six β-lactam-containing disks and selective culture agar medium are the first-reported simple and accurate methods of detecting meropenem-non-susceptible S. mitis/oralis group suitable for use in clinical microbiology laboratories.

One of the most important public health concerns is the rise of the multi-drug resistance bacteria in the recent years. Pseudomonas aeruginosa is a frequent Gram-negative bacterium prominent in the hospital-acquired illness and is considered as an opportunistic human pathogen responsible for causing nosocomial infections. Numerous burn victims, cystic fibrosis patients, and those with neutropenic malignancy die as a result of it. The current approach involves molecular docking for the predominant recognition of the drug binding site for the designing of the potent inhibitors for inhibiting the membrane protein of Pseudomonas aeruginosa. The present study has targeted 11 outer membrane proteins of Pseudomonas aeruginosa with 12 different FDA approved drugs. Protein modeling has been applied to create the target proteins. As per the results revealed out from the docking perspective, Piperacillin which has been categorized under the broad-spectrum antibiotics has emerged out as one of the forerunners as compared to the other group of antibiotics as it exhibited highest binding energy, i.e. -10.4 kcal/mol. Hence, the compound has been validated using in-silico tools such as ADME and PROTOX-II server which indicates its nontoxic nature. Molecular dynamics simulations were conducted for EGCG-OprP, Piperacillin-OprB, OprP (Apoprotein), and OprB (Apoprotein) complexes to assess their binding efficacy. Statistical parameters such as RMSD, RMSF, h-bond interactions, and % occupancies indicated stability in ligand binding. Protein RMSD values plateaued at approximately 0.5 nm, while ligand RMSD values remained below 0.2 nm, affirming stability in binding OprP and OprB. H-bond analysis revealed stable contacts for EGCG and Piperacillin, and % occupancies indicated specific interactions. Energetics analysis yielded deltaG values of -30.45 for EGCG and -56.66 for Piperacillin, suggesting efficient binding with OprP and OprB. This positioned Piperacillin as a promising candidate for future pharmacological studies, considering its classification as a broad-spectrum antibiotic against P. aeruginosa. The study served as a crucial roadmap for designing drugs to inhibit this formidable pathogen amid rising antibiotic resistance, emphasizing its significance in the ongoing battle against infectious diseases.
