This phase II trial studies if dinutuximab, GM-CSF, isotretinoin in combination with irinotecan, and temozolomide (chemo-immunotherapy) can be given safely to patients with high-risk neuroblastoma after Consolidation therapy (which usually consists of two autologous stem cell transplants and radiation) who have not experienced worsening or recurrence of their disease. Dinutuximab represents a kind of cancer therapy called immunotherapy. Unlike chemotherapy and radiation, dinutuximab targets the cancer cells without destroying nearby healthy cells. Sargramostim helps the body produce normal infection-fighting white blood cells. Isotretinoin helps the neuroblastoma cells become more mature. These 3 drugs (standard immunotherapy) are already given to patients with high-risk neuroblastoma after Consolidation because they have been proven to be beneficial in this setting. Chemotherapy drugs, such as irinotecan and temozolomide, work in different ways to stop the growth of tumor cells, either by killing the cells, by stopping them from dividing, or by stopping them from spreading. They may also affect how well immunotherapy works on neuroblastoma cells. Giving chemo-immunotherapy after intensive therapy may work better in treating patients with high-risk neuroblastoma compared to standard immunotherapy.

Late Relapse in Neuroblastoma: Case Report and Review of the Literature

Background::

Neuroblastoma is the most com mon extra-cranial solid tu mor in children. The survival rate of relapsed/refractory neuroblastoma is dismal. Late recurrence may occur rarely.

Case Presentation::

We have, herein, presented a case with stage IV neuroblastoma who relapsed after 11 years and had a subsequent relapse after 15 years from the initial diagnosis, and reviewed cases with late relapsed (after >5 years) neuroblastoma in the literature. The case presented with recurrent disease at the T7 vertebra after 11 years from the initial diagnosis. The patient received surgery, che motherapy, MIBG treatment, and antiGD2 combined with che motherapy, and had a further local recurrence in the paravertebral area of the re moved T7 vertebra after three years. The patient was operated, received anti-GD2 combined with che motherapy, and is still alive with no symptoms for 19 months after the last relapse.

Conclusion::

There is not a well-established treatment regimen for the majority of these patients. MIBG treatment and antiGD2 combined with che motherapy may be promising options for relapsed/ refractory neuroblastoma.

Characterization of metabolic and imaging profiles in the diagnosis of succinate dehydrogenase- related hereditary pheochromocytomas and paragangliomas

PURPOSE:

Hereditary pheochromocytomas and paragangliomas can be associated with mutations in the mitochondrial enzyme succinate dehydrogenase. Due to their heterogenous clinical presentation, no clear universal screening guidelines are currently available. We sought to examine the utility of metabolic testing and imaging in diagnosing hereditary pheochromocytomas and paragangliomas.

MATERIALS AND METHODS:

Our retrospective study included patients with confirmed familial succinate dehydrogenase mutations and histologically proven pheochromocytomas/paragangliomas or with suggestive metabolic and imaging features. We extracted information on age, race, sex, tumor size, tumor location, presence of metastatic disease and other succinate dehydrogenase-associated manifestations, urine and plasma metabolic testing, and diagnostic imaging. Patients who lacked any diagnostic information were excluded.

RESULTS:

Our cohort consisted of 9 patients with 13 tumor occurrences from 2003 to 2023. The average age at diagnosis was 31.7 years with a 2.9 cm average tumor size. Three patients developed metastatic disease. Five tumors (38.5%) were biochemically silent, with all tumors detected on imaging. Most of the remaining tumors had a noradrenergic profile, with positive norepinephrine and normetanephrine in plasma and urine. I-123 MIBG was the least sensitive (50%) imaging modality, and Ga-68 DOTATATE PET/CT was the most sensitive (100%). 10 (76.9%) tumors were treated with surgical resection; all metabolic results were subsequently negative, except in patients with metastatic disease.

CONCLUSIONS:

Our investigation adds to the current literature on diagnosing hereditary pheochromocytomas and paragangliomas. We highlight the importance of multimodal screening that consists of both imaging and metabolic screening, especially given the prevalence of biochemically silent tumors.

Long-Term Risk of Subsequent Neoplasms in 5-Year Survivors of Childhood Neuroblastoma: A Dutch Childhood Cancer Survivor Study-LATER 3 Study

PURPOSE:

Neuroblastoma survivors have an increased risk of developing subsequent malignant neoplasms (SMNs), but the risk of subsequent nonmalignant neoplasms (SNMNs) and risk factors are largely unknown. We analyzed the long-term risks and associated risk factors for developing SMNs and SNMNs in a well-characterized cohort of 5-year neuroblastoma survivors.

METHODS:

We included 563 5-year neuroblastoma survivors from the Dutch Childhood Cancer Survivor Study (DCCSS)-LATER cohort, diagnosed during 1963-2014. Subsequent neoplasms were ascertained by linkages with the Netherlands Cancer Registry and the Dutch Nationwide Pathology Databank (Palga) and medical chart review. We calculated standardized incidence ratios (SIRs), absolute excess risk (AER), and cumulative incidences. Multivariable competing risk regression analysis was used to evaluate risk factors.

RESULTS:

In total, 23 survivors developed an SMN and 60 an SNMN. After a median follow-up of 23.7 (range, 5.0-56.3) years, the risk of SMN was elevated compared with the general population (SIR, 4.0; 95% CI, 2.5 to 5.9; AER per 10,000 person-years, 15.1). The 30-year cumulative incidence was 3.4% (95% CI, 1.9 to 6.0) for SMNs and 10.4% (95% CI, 7.3 to 14.8) for SNMNs. Six survivors developed an SMN after iodine-metaiodobenzylguanidine ( 131I MIBG) treatment. Survivors treated with 131I MIBG had a higher risk of developing SMNs (subdistribution hazard ratio [SHR], 5.7; 95% CI, 1.8 to 17.8) and SNMNs (SHR, 2.6; 95% CI, 1.2 to 5.6) compared with survivors treated without 131I MIBG; results for SMNs were attenuated in high-risk patients only (SMNs SHR, 3.6; 95% CI, 0.9 to 15.3; SNMNs SHR, 1.5; 95% CI, 0.7 to 3.6).

CONCLUSION:

Our results demonstrate that neuroblastoma survivors have an elevated risk of developing SMNs and a high risk of SNMNs. 131I MIBG may be a treatment-related risk factor for the development of SMN and SNMN, which needs further validation. Our results emphasize the need for awareness of subsequent neoplasms and the importance of follow-up care.
