Perioperative Use of a Beta-adrenergic Blocker and a COX-2 Inhibitor in Patients Undergoing Surgery With Primary Pancreatic Cancer: Intervention Aiming to Reduce Pro-metastatic Processes

In Israel, of the
1000 patients diagnosed annually with pancreatic cancer (PC), approximately 250 (25 percent) will be eligible for curative surgery, of which 80 percent will succumb to post-surgical metastatic disease. A reduction in post-surgical metastatic disease will save dozens of patients in Israel annually, and tens-of thousands-around the world. The short perioperative period (days to weeks around surgery) is characterized by stress-inflammatory responses, including catecholamines (CAs, e.g., adrenaline) and prostaglandins (PGs, e.g., prostaglandin-E2) release, and induce deleterious pro-metastatic effects. Animal studies implicated excess perioperative release of CAs and PGs in facilitating cancer progression by affecting the malignant tissue, its local environment, and anti-metastatic immune functions. Congruently, our animal studies indicate that combined use of the beta-adrenergic blocker, propranolol, and the prostaglandins inhibitor, etodolac - but neither drug separately - efficiently prevented post-operative metastatic development. We recently conducted two clinical trials in three medical centers in Israel, recruiting breast (n=38) and colorectal (n=34) cancer patients, assessing the safety and short-term efficacy of perioperative propranolol and etodolac treatment. Drugs were well tolerated, without severe adverse events. Importantly, molecular/biological analyses of the excised primary tumor indicated that drug treatment caused promising anti-metastatic transformations, as well as improvements in immune and inflammatory indices. These included (i) decreased tumor cell capacity to migrate, (ii) reduced pro-metastatic capacity of the malignant tissue, and (iii) improvement in immune infiltrating into the tumor (Paper published in Clinical Cancer Research, 2017). Herein, we propose to conduct a double-blind placebo-controlled two-arm Phase II clinical trial in 210 pancreatic cancer patients undergoing curative surgery in Israel. A perioperative 35-day drug treatment will be initiated 5 days before surgery. Primary outcomes will include (i) 1-year disease-free-survival (DFS), and 5-year overall survival (OS); and (ii) biological markers in blood samples, and in the excised tumor tissue. Secondary outcomes will include safety indices and psychological measures of depression, anxiety, distress, and fatigue.

Implementation of a novel method for the determination of plasma protein binding of highly bound compounds using the equilibrium dialysis method coupled with extraction to the organic phase and its comparison to known methods

Accurate determination of plasma protein binding (PPB) is crucial in understanding the pharmacokinetics and pharmacodynamics of drugs, particularly for highly bound compounds where traditional methods may fall short. In this study, we present a pioneering approach for the precise determination of PPB that takes advantage of the lipophilicity of highly bound compounds. Twenty four highly bound compounds (with a fraction unbound (f_u) from 10^-1 to 10^-6) were tested with the most commonly used method, i.e., the rapid equilibrium dialysis (RED) method, along with its modifications adapted especially for strongly bound compounds, including dilution, presaturation, competition, and flux-dialysis methods. The results of these methods were compared to data obtained with the modification of RED coupled with extraction to organic phase, and the correlations between them were presented. Comparison studies demonstrate the accuracy of our approach across a set of highly bound compounds within a twofold range. Moreover, PPB values were determined for venetoclax, amiodarone, montelukast, and fulvestrant, for which, to the best of our knowledge and after extensive review of the literature, it has not been possible with the standard RED method until now. In conclusion, our new method can be a big step forward in finding the PPB of strongly plasma protein-bound compounds. It gives us a better understanding of how drugs and proteins interact, which helps us make safer and more effective medicines.

Comparative Evaluation of Cyclooxygenase Inhibition Profiles Across Various NSAID Forms and Doses: Implications for Efficacy and Adverse Effects

INTRODUCTIONNonsteroidal anti-inflammatory drugs (NSAIDs) are commonly used for pain disorders and exert pharmacological effects by inhibiting cyclooxygenase (COX). Although previous studies have evaluated the COX inhibitory activity and selectivity of NSAIDs, none has compared COX inhibitory concentrations with the plasma concentrations of clinical doses or investigated the efficacy and adverse effects of different dosage forms. Therefore, in this study we evaluated the COX inhibitory activities and inhibition rates of clinical doses of the various NSAID formulations, especially diclofenac sodium.METHODSHuman blood and the drug (diclofenac sodium, celecoxib, ibuprofen, flurbiprofen, or etodolac) were mixed and incubated, and the supernatant was collected and quantified the COX inhibitory activity of each drug by ELISA. Logistic regression analyses were used to calculate the inhibition rates at maximum plasma drug concentration (C_max) of clinical doses of marketed formulations. For diclofenac sodium, we also calculated the concentrations at which COX inhibition rates were 50% and 80% (IC₅₀ and IC₈₀).RESULTSCOX-2 inhibition rate at C_max of clinical doses exceeded 50% except celecoxib 100 mg. For diclofenac sodium, the C_max at the clinical doses of the oral and suppository formulations showed almost complete inhibition of COX-2 and an inhibition rate exceeding IC₈₀ for COX-1. The C_max at repeated doses of the transdermal formulation showed an inhibition rate above IC₈₀ for COX-2 but below IC₈₀ for COX-1.DISCUSSIONThis result explains why gastrointestinal disorders frequently occur with oral and suppository formulations of diclofenac sodium despite its relatively high COX-2 selectivity. Although the plasma drug concentration of the transdermal formulation is lower than oral and suppository formulations, it has an inhibition rate above IC₅₀ for COX-2, which is required for analgesic efficacy, and has a lower COX-1 inhibition rate than these formulations.CONCLUSIONThe findings explain why the transdermal formulation exerts an analgesic effect despite having a lower C_max than other diclofenac sodium formulations.

Optimizing etodolac and quercetin loaded MSNs using taguchi design: an approach for enhancing drug loading efficiency

The application of mesoporous silica nanoparticles (MSN) as a drug carrier system got immense attention in the past few years due to their exceptional high drug loading efficiency. However, the process of drug loading is quite challenging compared to other lipid-based drug delivery systems. Hence, the MSNs using different catalysts were synthesized, and their mesoporous material characteristic was confirmed by the type IV adsorption-desorption isotherm using BET analyzer. The effect of solvent selection and other process parameters (drug to MSNs ratio, period of loading, and stirring speed) on the loading of BCS class II and IV model drugs etodolac(ETD) and quercetin(QUR) respectively were investigated with the help of Taguchi DOE. The predicted value for the highest % drug loading was close to the experimental value(19.04 ± 0.50 % and 11.40 ± 0.18 % for ETD and QUR respectively). It was interesting to note, that the solvent selection had the highest impact on the drug loading of ETD into the MSNs, whereas for QUR this parameter was insignificant. Hence reflecting that a generalized procedure for the drug loading into the MSNs cannot be followed and had to be critically studied. Also, the loading of ETD and QUR into the MSNs improved the aqueous solubility (3.08 and 2.5 folds respectively). Further the in-vitro drug release properties were evaluated for ETD and QUR from MSNs in various drug release mediums to explore their release mechanism from MSNs using in vitro drug release kinetic modelling.

Three generic firms shell out $447M more to settle federal price fixing allegations

Generic drug firms Taro Pharmaceuticals USA, Novartis’ Sandoz, and Apotex have agreed with the US Department of Justice to pay $447.2 million on top of the more than $400 million in criminal penalties they already paid to resolve alleged violations that they illegally collaborated on price, supply and allocation of customers with other pharma manufacturers. Each of the companies entered a five-year corporate integrity agreement, which includes internal monitoring and price transparency provisions, among other checks and balances. New York-based Taro will pay almost half of that latest sum, or $213.2 million, for alleged violations involving the nonsteroidal drug etodolac, and nystatin-triamcinolone cream and ointment to treat skin infections. Sandoz agreed to pay $185 million related to alleged issues with benazepril HCTZ, used to treat hypertension, and clobetasol, a corticosteroid used to treat skin conditions. “The conduct at issue here occurred at Sandoz from March 2013 through December 2015 in direct contravention of the company’s values, policies and trainings in place at the time. The individuals implicated in the underlying conduct are no longer employed by the company,” Sandoz said in a statement. And Florida-based Apotex agreed to pay $49 million in connection with its sale of pravastatin, a drug used to treat high cholesterol and triglyceride levels. Apotex is also part of another ongoing investigation involving pravastatin price-fixing allegations, involving Teva Pharmaceuticals and Glenmark Pharmaceuticals. This latest $447 million payout also follows the payment of separate criminal penalties, in which Taro paid $205.6 million and admitted to conspiring with two other generic drug companies to fix prices on certain generic drugs, while Sandoz paid another $195 million and admitted to conspiring with four other generic drug companies to fix prices on certain generic drugs, and Apotex paid $24.1 million and admitted to conspiring to increase and maintain the price on pravastatin.
