Evaluation of urinary vanin-1 for the early prediction of cisplatin-induced acute kidney injury during neoadjuvant chemotherapy for esophageal cancer

PURPOSECisplatin (CDDP) induces acute kidney injury (AKI) as a side effect during neoadjuvant chemotherapy (NAC). Urinary vanin-1 excretion may increase during CDDP treatment. We investigated whether urinary vanin-1 is an early biomarker for CDDP-induced AKI.METHODSThirty patients were administered 80 mg/m² CDDP on day 1 as NAC for esophageal cancer. Blood and urine samples were collected on days 1, 2, 3, 4, and 6 after CDDP administration. Serum creatinine (sCr) and urinary vanin-1 levels were measured. Creatinine clearance (cCr) and estimated glomerular filtration rate (eGFR) were calculated from sCr. Based on the change in sCr after CDDP administration, the AKI and non-AKI groups were defined using the Kidney Disease Improving Global Outcomes classification. Changes in sCr, cCr, eGFR, and urinary vanin-1 levels were compared between the two groups.RESULTSA gradual increase in sCr and a decrease in eGFR were observed over time post-CDDP administration, with differences between the two groups becoming significant by day 4. However, urinary vanin-1 levels increased on day 3 after CDDP administration, and the difference between the two groups was significant on day 3. Receiver operating characteristic curves of urinary vanin-1 on day 3 revealed that a cut-off value of 3.17 ng urinary vanin-1/mg urinary creatinine yielded an area under the curve, sensitivity, and specificity of 0.83 (P < 0.05), 75.0%, and 22.7%, respectively. The non-AKI incidence below the cut-off value of urinary vanin-1 of 3.17 ng/mg uCr was 89.5%.CONCLUSIONUrinary vanin-1 is a superior minimally invasive biomarker for the early prediction of CDDP-induced AKI.

18β-glycyrrhetinic acid improves pulmonary hypertension by regulating the vascular non-inflammatory molecule-1/L-arginine/nitric oxide signaling pathway

PURPOSEPulmonary hypertension (PH) is a progressive cardiopulmonary disease characterized by elevated pulmonary arterial pressure and resistance. 18β-Glycyrrhetinic acid (18β-GA), a major bioactive component in Glycyrrhiza glabra L., exhibits various pharmacological activities. Although 18β-GA has been found to alleviate PH, the underlying mechanisms are still unclear. This study was aimed to elucidate the molecular processes by which 18β-GA attenuates PH.PATIENTS AND METHODSA monocrotaline-induced PH rat model was created, and cardiac function and pulmonary arterial pressure were assessed. Rat blood markers were analyzed via enzyme-linked immunosorbent assay, and protein levels of Vanin-1, NF-κB, IL-6, TNF-α, PPAR-γ, and iNOS in lung tissue were determined by western blotting. A hypoxia-induced pulmonary artery smooth muscle cell (PASMCs) proliferation model was created, and cell proliferation and migration were evaluated using CCK-8 and scratch wound assays. Cell cycle, apoptosis, and mitochondrial membrane potential were assessed via flow cytometry, and protein levels were measured using western blotting.RESULTSPH rats treated with, 18β-GA exhibited reduced mean pulmonary arterial pressure (mPAP) and delayed pulmonary vascular remodeling. In lung tissue, Vanin-1, NF-κB, IL-6, and TNF-α were downregulated, while PPAR-γ and iNOS were upregulated. PASMCs treated with 18β-GA exhibited reduced hypoxia-induced proliferation and increased apoptosis. In addition, NO levels in the supernatant of PASMCs were elevated, and Vanin-1 and NF-κB were downregulated, whereas PPAR-γ and iNOS were upregulated.CONCLUSIONOur findings reveal a novel mechanism by which 18β-GA attenuates PH by improving pulmonary vascular remodeling and inhibiting PASMCs proliferation via the Vanin-1/L-Arg/NO pathway.

Identification of potential modulators of intrauterine adhesion pathogenesis with RNA sequencing, histology and in vitro assays

Intrauterine adhesion (IUA), also referred to as intrauterine stenosis or synechiae, is a prevalent gynecological issue, which is characterized by the fusion of the walls of the intrauterine canal. However, the molecular changes during its pathogenesis are still unclear. In the present work, tissue samples from patients with IUA and normal endometrial tissues from healthy subjects were collected, and then RNA sequencing and bioinformatics analyses were performed to screen the differentially expressed genes (DEGs). Subsequently, immunohistochemistry was used for detecting the protein expression level of the representative genes including XDH, VNN1, CD36, and after transfection, enzyme-linked immunosorbent assay and Western blotting were used to evaluate their functions in regulating inflammatory response and the expression level of matrix metalloproteinases. It was revealed that multiple genes were dysregulated in the pathological tissues of patients with IUA, and these DEGs were associated with multiple biological processes and signal pathways including Hedgehog pathway. DEGs including XDH, VNN1, CD36 were also highly expressed in IUA tissues at protein level, and their expression levels correlated with the expression levels of inflammation mediators NLRP3 and STING. XHD, VNN1 and CD36 also promoted the expression and secretion of TNF-α, IL-1β and IL-6 in ishikawa cells, and up-regulated the expression level of MMP-2 and MMP-9. Collectively, our data suggested that Hedgehog signaling is a potential crucial pathway in IUA pathogenesis, and some DEGs contribute to endometrial fibrosis by regulating inflammatory response and matrix remodeling.
