Analysing the Genetic Mosaic of Relapsing Remitting vs Clinically isolated syndrome Multiple Sclerosis: A Bioinformatics Perspective on Subtype‐Specific Variations

AbstractBackgroundMultiple Sclerosis (MS) is a chronic, etiologically complex disease of the central nervous system (CNS) characterized by inflammation, demyelination, and neuronal damage. MS has seven categories based on disease course. Seventy to eighty percent of individuals with MS initially develop a clinical pattern with periodic relapses and remissions, called relapsing‐remitting MS (RRMS). Clinically isolated syndrome (CIS) refers to a single clinical inflammatory attack on the CNS. Evidence suggests that CD8+ T cells play a substantial role in MS pathophysiology. In this bioinformatic study, we aim to identify specific genes and pathways up‐regulated or down‐regulated in CD8+ T cells of RRMS and CIS subtypes.MethodThe GSE19285 dataset on the Gene Expression Omnibus (GEO) consists of the whole transcriptome of sorted T cells (CD8+) from treatment‐naive MS patients. Age‐matched sampling maintained uniformity. Data analysis used GEO2R with the Benjamini & Hochberg method (False discovery rate, padj < 0.05), and log fold change threshold greater than 1.5 or less than 1.5. We explored genetic profiles in immune cells, comparing RR (n = 17, age 30.76±7.05 years), CIS (n = 17, age 30.76±6.90 years), and healthy controls (n = 17, age 30.71±7.09 years). Gene pathway and protein‐protein interaction analyses of differentially expressed genes used the STRING platform.ResultThe study revealed upregulation of TRBV4‐2 gene in CD8+ cells of RRMS patients compared to healthy controls, while several genes, including AATK, PPP1R14C and TRIM36, were downregulated in RRMS. Two genes, DACT1 and CD4, were identified as upregulated in CD8+ cells, while several genes, including MYOM2, AATK, GPR27, LOC105370088, TRR‐ACG1‐2, KIR2DL3, and SIGLEC10, were downregulated in CIS compared to healthy controls. Additionally, CD4, DACT1, and CYBRD1 genes were up‐regulated in CIS compared to RRMS.ConclusionIn RRMS, TRIM36’s role in proteostasis and neurodegeneration is highlighted, with protein‐protein interaction analysis showing dysfunctional ubiquitination. Altered antigen processing and presentation in CIS compared to healthy individuals and RRMS suggest differences in pathophysiology in this subtype. Furthermore, recent study has suggested CYBRD1 as a potential MS biomarker.

Killer‐cell immunoglobulin‐like receptor polymorphism is associated with COVID‐19 outcome: Results of a pilot observational study

The pathogenesis of COVID‐19 warrants unravelling. Genetic polymorphism analysis may help answer the variability in disease outcome. To determine the role of KIR and HLA polymorphisms in susceptibility, progression, and severity of SARS‐CoV‐2 infection, 458 patients and 667 controls enrolled in this retrospective observational study from April to December 2020. Mild/moderate and severe/death study groups were established. HLA‐A, ‐B, ‐C, and KIR genotyping were performed using the Lifecodes® HLA‐SSO and KIR‐SSO kits on the Luminex® 200™ xMAP fluoroanalyser. A probability score using multivariate binary logistic regression analysis was calculated to estimate the likelihood of severe COVID‐19. ROC analysis was used to calculate the best cut‐off point for predicting a worse clinical outcome with high sensitivity and specificity. A p ≤ 0.05 was considered statistically significant. KIR AA genotype protected positively against severity/death from COVID‐19. Furthermore, KIR3DL1, KIR2DL3 and KIR2DS4 genes protected patients from severe forms of COVID‐19. KIR Bx genotype, as well as KIR2DL2, KIR2DS2, KIR2DS3 and KIR3DS1 were identified as biomarkers of severe COVID‐19. Our logistic regression model, which included clinical and KIR/HLA variables, categorised our cohort of patients as high/low risk for severe COVID‐19 disease with high sensitivity and specificity (Se = 94.29%, 95% CI [80.84–99.30]; Sp = 84.55%, 95% CI [79.26–88.94]; OR = 47.58, 95%CI [11.73–193.12], p < 0.0001). These results illustrate an association between KIR/HLA ligand polymorphism and different COVID‐19 outcomes and remarks the possibility of use them as a surrogate biomarkers to detect severe patients in possible future infectious outbreaks.

Association between Killer Immunoglobulin-like receptor genes and susceptibility to inflammatory bowel disease: An updated meta-analysis

BackgroundSeveral studies have associated members of the KIR genes as susceptibility factors to inflammatory bowel diseases (IBD): ulcerative colitis (UC) and Crohn's disease (CD).ObjectivesTo assess the association between the presence and absence KIR genes and IBD susceptibility through a meta-analysis.MethodA systematic search was performed through the PubMed, Scopus, and Web of Science databases to obtain relevant articles published before March 2024. Associations between genes and susceptibility to IBDs were estimated by OR with 95 % CI.ResultsWe found positive associations of the KIR2DS1 and KIR2DS3 genes with susceptibility to UC, while the KIR2DL3 and KIR2DS4 ^full genes showed a negative association. In addition, the KIR2DS1, KIR2DS3, KIR2DS4, KIR2DS5, and KIR3DS1 genes showed a positive association with susceptibility to CD, whereas the KIR2DL1 gene showed a negative association.ConclusionsOur meta-analysis indicates that individuals carrying the KIR2DS1 and KIR2DS3 genes exhibit increased susceptibility to UC, whereas carriers of the KIR2DS1, KIR2DS3, KIR2DS4, KIR2DS5, and KIR3DS1 genes are more prone to CD. However, further studies are required to clarify the role of the KIR genes and their corresponding ligands in the pathology of IBD.
