Introduction

A neoplastic clonal expansion of endometrial glands characterized by cytologic changes of the epithelium and the presence of an increased number of endometrial glands. The glands form crowded aggregates with tubular or branching patterns which are cytologically distinct from the background architectural and cytological pattern. It is associated with molecular changes seen in endometrioid endometrial carcinoma, including microsatellite instability, PAX2 inactivation, and PTEN, KRAS, and CTNNB1 gene mutations.

Serous Endometrial Intraepithelial Carcinoma (SEIC): Prospective Registration Study

Background: Serous endometrial intra-epithelial carcinoma (SEIC) is a rare condition affecting the lining of the uterus. It's a pre-cancerous change that might lead to cancer if not treated. Doctors aren't sure of the best way to manage it because it's so uncommon. A previous study looked at past cases in the Netherlands from 2012 to 2020. This study found that none of the 23 women with SEIC had their cancer spread during surgery, and none had a recurrence in the following three years. This suggests that a simpler surgery might be safe for patients with SEIC.
Methods: Information on disease details from patients who agree to participate in our study will be collected. All participants will fill out online forms and health questionnaires at several points: when they join the study, and again at six months, two years, and five years after their diagnosis. During the years after diagnosis, various details like health status, test results, treatments will be checked in the electronic medical record.
Results: This study is just starting, with the first patients expected to join on January 1, 2025.
Conclusion: A large international study is created to track SEIC patients over time. This will help us learn more about the condition and improve treatment strategies.

An effective surgical approach to treat obese patients with gynecological disease using a subcutaneous abdominal wall-retraction device to perform low-pressure laparoscopy: A prospective, single-center study

OBJECTIVELaparoscopic surgery of obese patients still represents a real challenge in clinical practice. High-pressure pneumoperitoneum and steep Trendelenburg position are the main anesthesiologic indications to laparotomic conversion. The aim of this prospective study was to assess effectiveness and safety of low-pressure laparoscopic (LPL) procedures using a subcutaneous abdominal wall-retraction (LaparoTenser ®).MATERIALS AND METHODSWe enrolled obese patients (BMI ≥ 30 kg/m²) with early-stage endometrial cancer, atypical endometrial hyperplasia and suspicious adnexal mass who were planned for laparoscopic surgery.RESULTSA total of 33 patients were included in this study. The median age was 69 (range: 40-83), with a median BMI of 39 kg/m² (range: 33-48). At final pathologic report, 24 patients had endometrial cancer, 4 atypical endometrial hyperplasia, 2 had ovarian borderline tumors and 3 benign cysts. All LPL procedures were performed using the LaparoTenser ® device. Total hysterectomy plus bilateral salpingo-oophorectomy and sentinel node biopsy was the main procedure (69.7 % of cases). Laparotomy conversion rate due to inadequate visualization of the surgical field was 6.1 %. Postoperatively, no patient reported relevant abdominal discomfort caused by lifting of the abdominal wall. Grade < 2 early complications rate was 21.1 %. One grade ≥ 3 complications was reported.CONCLUSIONSLPL technique using the LaparoTenser ® device is feasible and safe in morbidly obese patients. The wall-lifting device enables adequate viscera exposure creating a large intra-abdominal operative space avoiding the disadvantages of intraperitoneal high-pressure and CO₂ absorption.

ECPPF stratification identifies occult high-risk subgroups in stage I, grade 1 or 2, ≤50 % invasive endometrial cancer: Candidates for adjuvant therapy

OBJECTIVETo determine whether stratification with ECPPF (E2F1 + CCNA2 log2 expression and POLE, PPP2R1A, and FBXW7 variants) could identify occult cases of high-risk endometrial cancer (EC) in a traditionally low-risk cohort.METHODSWe identified 97 cases of clinicopathologic low-risk endometrioid EC (defined as stage I, grade 1 or 2, limited [≤50 %] myometrial invasion) from The Cancer Genome Atlas (TCGA). Twelve cases had POLE mutations (mu) and 15 had PPP2R1Amu or FBXW7mu. Log2 CCNA2 + E2F1 expression was low (<4.75) for 56 cases and high (>4.75) for 19 (termed CCNA2 + E2F1 low or high, respectively). CCNA2 + E2F1 high and PPP2R1Amu/FBXW7mu were simultaneously present for 5 cases. Survival comparisons were based on log-rank tests.RESULTSFive-year progression-free survival (PFS) curves for POLEmu and CCNA2 + E2F1 low differed substantially from CCNA2 + E2F1 high and PPP2R1mu/FBXW7mu cases (P < .001). The latter 2 subgroups, combined (n = 29) and designated as molecular high risk (MHR), had an estimated 5-year PFS <50 %. Adverse outcomes were associated with MHR for cases harboring CTNNB1mu (P < .001), ARID1Amu (P = .03), and PTENmu (P = .002). TCGA classification was not prognostically significant for this cohort (P = .10), but ECPPF MHR identified compromised subgroups within major TCGA subclasses (P = .004). CCNA2 + E2F1 high and expression of its downstream targets were positively correlated (P < .001) with expression of genes involved in chemoresistance (ie, homologous recombination, cell cycle regulation, antiapoptotic processes).CONCLUSIONSECPPF supports a taxonomy in which occult, high-risk disease is identified among cases traditionally considered low risk. With high-risk cases unlikely to respond to current first-line chemotherapy, case identification should prompt proactive therapeutic intervention with alternative molecular-based treatment targets.
