Safety and Preliminary Efficacy of Sm-p80 + GLA-SE (SchistoShield®) Vaccine Against Controlled Human Schistosome Infection in Healthy, Schistosoma-naïve Adults

The goal of this clinical trial is to learn about the Sm-p80 + GLA-SE (Schistoshield®) vaccine in healthy participants who have not had schistosomiasis before. The main questions it aims to answer are:
if the vaccine is safe
if after vaccinated people start producing antibodies
if the vaccine works against schistosomiasis.
Participants will receive three vaccines (or placebo) and are then exposed to 20 male Schistosoma cercariae. Afterwards they are treated with praziquantel to cure the infection.
Researchers will compare the group vaccinated with Schistoshield® and placebo (fake vaccination) to see if the vaccine has worked.

A Comprehensive Evaluation of an Artificial Intelligence Based Digital Pathology to Monitor Large-scale Deworming Programs Against Soil-transmitted Helminths

The goal of this observational study is to test a new AI diagnostic tool for detection, specification and quantification of parasitic infections (Ascaris, Trichuris, hookworm and S. Mansoni) in School aged children in Ethiopia and Uganda. The main questions it aims to answer are:
Diagnostic Performance of the AI tool and compare to traditional manual microscopy
Repeatability and reproducibility of the AI tool and compare to traditional manual microscopy
Time-to-result for the AI tool
Cost efficiency for the AI tool and traditional manual microscopy to inform programmatic decisions.
Usability of the AI tool
Participants will be asked to provide a stool sample for examination by the AI tool and traditional manual microscopy. Participants with a positive test result will receive the proper treatment (Deworming drug).

Female Genital Schistosomiasis in Tanzania: Evaluation of Prevalence and Performance of Schistosoma Haematobium-specific Polymerase Chain Reaction (PCR) on Operator-based and Self-collected Cervical-vaginal Samples Among Women Living in Endemic Areas, North-western Tanzania

A cross-sectional survey will be conducted among 200 volunteering women aged 18- 45 years and having had prior sexual activity living in the target villages of Itilima and Maswa districts, North-western Tanzania. A single midday urine sample and two cervical-vaginal swabs (both self-collected and speculum-aided collected by a female healthcare worker) will be obtained from participating women and processed using urine filtration and polymerase chain reaction (PCR) for cervico-vaginal samples. A pre-tested structure questionnaire will be used to collect sociodemographic, clinical, and sampling acceptability information from participants.

Efficacy of spilanthol and Acmella oleracea (L.) R. K. Jansen (Asteraceae) extract against Schistosoma mansoni infection

ETHNOPHARMACOLOGICAL RELEVANCEAcmella oleracea (L.) R. K. Jansen, commonly referred to as "Jambu", is a valuable medicinal plant native to the Amazon regions. Inflorescences of A. oleracea is utilized as local anesthetic properties and for its insecticidal, antiparasitic, and anthelmintic attributes.AIMS OF THIS STUDYThis study aimed to evaluate the in vitro and in vivo antiparasitic properties of A. oleracea extract and spilanthol (SPL) against Schistosoma mansoni.MATERIALS AND METHODSThe ethanolic extract of A. oleracea inflorescences (AoE) was prepared, analyzed by HPLC-DAD, and characterized by UHPLC-HRMS/MS. SPL was isolated from AoE by chromatographic fractionation. The antischistosomal properties of AoE and SPL were evaluated in vitro against adult schistosomes and in preclinical assays using murine models of patent S. mansoni infection.RESULTSThrough UHPLC-HRMS/MS analysis, 14 alkamides were annotated in AoE. HPLC-DAD analysis of AoE revealed a peak with a substantial relative area of ∼85%, which was isolated and identified as SPL. AoE and SPL caused mortality of adult schistosomes ex vivo, showing EC₅₀ values of 32.6 μg/mL and 27.8 μM, respectively, without affected Vero cells or Caenorhabditis elegans. In preclinical studies, the oral administration (400 mg/kg) of AoE and SPL resulted in significant reductions in worm burden of 28% and ∼42%, respectively. Additionally, SPL exhibits remarkable effects on oogram, decreasing egg burden and the number of immature eggs by over 80%, while also promoting significant reductions in hepato- and splenomegaly.CONCLUSIONSThe research underscores the antischistosomal activity of A. oleracea highlighting the potential of SPL as a lead for the development of new schistosomicidal drugs and encouraging further studies to validate the traditional anthelmintic use of A. oleracea inflorescences.

Influence of Chlorella vulgaris and Pediastrum boryanum extracts carried on nanocellulose on the immune response of Biomphalaria alexandrina snails against Schistosoma mansoni infection

Continuous efforts are made to explore alternative methods for reducing Schistosomiasis. So, this study evaluated the effectiveness of Chlorella vulgaris and Pediastrum boryanum extracts carried on their nanocrystalline cellulose (NCC) as immunostimulants for Biomphalaria alexandrina snails against Schistosoma mansoni infection. The results showed that the lowest cercarial shedding/snail was 340 and 330 with 400 mg/L of C. vulgaris extract and NCC + C. vulgaris extract, respectively. Meanwhile, it was decreased with 200 mg/L of P. boryanum extract and 400 mg/L of NCC + P. boryanum extract, with high survival rates for all treatments. In addition, snails treated with 400 mg/L of C. vulgaris extract and 200 and 400 mg/L of NCC + C. vulgaris extract showed a significant decrease in the contents of hydrogen peroxide (0.639, 0.426, and 0.564 mM/L), respectively, compared to the control group (0.695 mM/L). Furthermore, C. vulgaris extracts induced numerous hemocytes around immature cercariae and sporocysts. P. boryanum extracts showed degenerated sporocysts surrounded by plenty of hemocytes. Nanocellulose carriers improved the delivery of microalgal components within B. alexandrina snails, causing the cercariae to lose their ideal shape. Gas chromatography-mass spectrometry (GC-MS) analysis revealed that C. vulgaris and P. boryanum extracts contained oleic acid, which improved the immunological response of snails, and glutamic acid and flavonoids acted as immune modulators and antioxidants. Ultimately, the toxicity assay indicated that NCC + C. vulgaris extract and NCC + P. boryanum extract were the safest for Artemia salina as a non-target aquatic organism.

Brief report: Chronic murine schistosomiasis causes aberrant hemostasis

Schistosomiasis afflicts >250 million people worldwide, leading to an annual loss of >3 million disability-adjusted life years. Schistosoma mansoni causes intestinal schistosomiasis with parasite eggs either transversing intestinal tissue or lodging within the liver and other organs, causing intestinal hemorrhage and liver pathology. Large (∼1 cm) adult worms survive for years within blood vessels, but we lack a clear understanding of their impact on hemostasis. We used a chronic mouse model of schistosomiasis to determine the impact on platelet numbers, phenotype and function. Hemostatic function was assessed by platelet phenotyping (flow cytometry and proteomics), whole blood aggregometry, and longitudinal coagulometry. Although platelets from schistosome-infected mice lack elevated surface P-selectin and activated αIIbβ3, unbiased proteomic analysis reveals infection-induced increases in MHC-I, IgM and IgG antibodies, and complement components. Whole blood from schistosome-infected mice spontaneously aggregates in the absence of exogenous agonists. Conversely, prothrombin and activated partial thromboplastin times are prolonged at the chronic stage of infection (10-12 weeks). A mouse model of S. mansoni infection shows wide-ranging changes in hemostatic function which may have clinically relevant implications for populations in endemic regions.
