Inhaled Milrinone and Epoprostenol for the Prevention of Difficult Cardiac Pulmonary Bypass Separation: A Randomized, Double-blind, Controlled Trial

In cardiac surgery, the presence of pulmonary hypertension (PH) is a prognostic factor associated with increased mortality and morbidity. In this context, one of the main causes of PH is related to reperfusion ischemia during weaning from extracorporeal circulation (CPB). One of the consequences of PH is right ventricular dysfunction. During weaning from CPB, the development of a right ventricular dysfunction is associated with increased requirements for vasopressor and inotropic agents, duration of mechanical ventilation, prolonged intensive care and hospital stay, and increased mortality compared with patients with left ventricular (LV) dysfunction. The management of patients with PH with or without right ventricular (RV) dysfunction relies on several strategies such as the administration of intravenous and inhaled agents, or mechanical ventricular support. Among those agents, the administration of inotropes or pulmonary vasodilators such as epoprostenol, milrinone and nitric oxide are among the most widely used treatments recommended by the Canadian Cardiovascular Society. At the Montreal Heart Institute, inhaled epoprostenol and milrinone are routinely administered to patients with PH or LV dysfunction in the perioperative setting. Despite the frequent use of inhaled epoprostenol and milrinone, Health Canada has not yet approved the use of these molecules.
The primary objective of this multicenter, double-blind, randomized clinical trial is to evaluate the clinical efficacy of the combined administration of inhaled epoprostenol and milrinone in a cardiac surgery setting. This trial will compare the clinical outcome of 71 patients who will receive inhaled epoprostenol and milrinone before the start of bypass surgery to 71 patients who will receive a placebo before the start of the CPB. The primary clinical outcome is the proportion of patients with an "unsuccessful" CPB weaning defined by the use of an inotrope +/- vasopressor agent or the use of mechanical circulatory support or a return to bypass grafting for hemodynamic reasons.
This clinical trial will evaluate the clinical efficacy of the combination of inhaled agents in a cardiac surgery setting. Therefore, if the results of this study are positive, the combination of inhaled epoprostenol and milrinone will optimize the management of patients with pulmonary hypertension with or without a right ventricular dysfunction.

Human prostacyclin and thromboxane synthases: Molecular interactions, regulation, and pharmacology

Prostanoids are lipid mediators of the human body that are involved in the inflammation and platelet aggregation. Prostacyclin is a vasodilator and inhibitor of platelet aggregation, and a product of the enzymatic reaction catalyzed by prostacyclin synthase (PTGIS). Thromboxane is a vasoconstrictor and synthesized by thromboxane synthase (TBXAS1). An imbalance of prostanoids can accompany cardio-/cerebrovascular diseases and cancers. PTGIS and TBXAS1 are clinically relevant membrane-bound enzymes of the multigene family of cytochromes P450 (CYPs), also known as CYP8A1 and CYP5A1, respectively. Particular studies of these functional antagonists will contribute to the elucidation of pathogenic mechanisms. The purpose of this work was to analyze the literature landscape over a period of 2020-2024 in the field of biological, pharmacogenomic, and pharmacological features of PTGIS and TBXAS1 as well as to explore the potential of their regulation at the post-transcriptional and post-translational levels using systems biological analysis. The review discusses recent findings on the novel aspects of both synthases established in gene knockout and overexpression experiments, current preclinical pharmacology, and potential ways of gene expression regulation. Identification of protein-protein interactions and post-translational modifications appear to be the main options for modulating PTGIS and TBXAS1 activity. The microsomal CYPs are known to form complexes with each other and direct interactions of CYP2E1 with both synthases can probably lead to modulation of their activity. Progress in the preclinical development of low molecular weight compounds as inhibitors of TBXAS1 is more prospective than PTGIS that is applied as gene therapy biologicals for in vivo production of prostacyclin due to its noticeable anticancer and vasodilator effects.

Low phylloquinone intake deteriorates endothelial function in normolipidemic and dyslipidaemic mice

While the plasma phylloquinone (PK) concentration is inversely correlated with cardiovascular risk, the involvement of PK in regulating endothelial function has not been directly investigated. Therefore, in this study we assessed the effects of short-term treatment with PK-deficient diets (5-10 weeks) on endothelial function in normolipidemic 14-week-old male C57BL/6JCmd mice and age-matched dyslipidaemic male E3L.CETP mice. Our results show that in normolipidemic mice dietary PK deficiency was associated with a marked reduction of PK levels in the plasma and liver (liquid chromatography-mass spectrometry measurements) and with impaired endothelium-dependent vasodilation assessed in vivo by magnetic resonance imaging (MRI). Dietary PK deficiency-induced endothelial dysfunction was fully reversed by PK supplementation. In dyslipidaemic E3L.CETP mice, dietary PK deficiency exacerbated preexisting endothelial dysfunction. Furthermore, dietary PK deficiency decreased menaquinone-4 (MK-4) levels in the aorta but did not affect blood coagulation (calibrated automated thrombography), microbiota composition (culturing and next-generation sequencing), and gut menaquinone production. In conclusion, our study demonstrated for the first time that sufficient dietary PK intake supports endothelial function in normolipidemic and dyslipidaemic mice indicating nutritional significance of dietary PK in the maintenance of endothelial function in humans.

Effects of exercise on cAMP-mediated platelet inhibition in young women: a pilot study

PURPOSE:

Exercise has been shown to reduce platelet reactivity and increase platelet sensitivity to prostacyclin, an endothelium-derived inhibitor of platelet activation, in middle-aged men and women. It is currently unknown if these beneficial effects can also be observed in young women and the intracellular mechanisms involved have not been identified. In this study, the feasibility of detecting changes in platelet reactivity, prostacyclin sensitivity and cAMP signalling were tested.

METHODS:

10 well-trained and 10 sedentary but healthy young women participated in this study. Responses of washed platelets to thrombin receptor activating peptide 6, the thromboxane A₂ receptor agonist U46619, and prostaglandin E₁ were measured by light transmission aggregometry. Expression levels of proteins in the cAMP pathway including phosphorylation of the vasodilator-stimulated phosphoprotein were analysed by western blotting.

RESULTS:

There was no evidence of reduced basal reactivity in platelets from the well-trained group ( __-mml:math xmlns:mml="http://www.w3.org/1998/Math/MathML"-__ V ˙ O 2 max = 51.1 ± 3.6 ml/kg/min) compared to the untrained group ( __-mml:math xmlns:mml="http://www.w3.org/1998/Math/MathML"-__ V ˙ O 2 max = 31.1 ± 4.7 ml/kg/min). Platelets from the trained group showed evidence of greater sensitivity to the anti-aggregatory effects of prostaglandin E₁. The slope of the aggregation curves indicated an overall faster rate of aggregation in the untrained group. Mean phosphorylation levels of vasodilator-stimulating phosphoprotein were consistently higher in the trained group, indicative of increased protein kinase A activity.

CONCLUSION:

Platelets from young women may exhibit an exercise-induced increase in sensitivity to prostacyclin leading to stimulation of the cAMP pathway. A larger study is warranted to explore this vasoprotective effect further.

Cereno Scientific presented CS1’s Phase IIa trial results at the 5th Baltic Pulmonary Hypertension Conference 2025

Cereno Scientific (Nasdaq First North: CRNO B), an innovative biotech pioneering treatments to enhance and extend life for people with rare cardiovascular and pulmonary diseases, today announced that an oral presentation titled “Exploratory outcomes of CS1 in Pulmonary Arterial Hypertension: Phase 2A, Prospective, Randomized, Open-Label, Multicenter Trial” was presented at the 5th Baltic Pulmonary Hypertension Conference 2025 on May 9, 2025, in Kaunas, Lithuania. The data presented in an abstract at the Baltic PH Conference highlighted key results from the Phase IIa trial of CS1 in PAH. The primary endpoint of safety and tolerability was met without any drug-related safety concerns. After 12 weeks, REVEAL Risk Score 2.0 was improved from baseline in 40.9% of patients and stable in a further 31.8% of patients. A total of 31.8% and 54.5% of patients improved or maintained their NYHA/WHO functional class, respectively. The majority of patients (71%) showed an improvement in quality of life as measured by the Minnesota Living with Heart Failure Questionnaire (MLHFQ). The abstract concludes that the findings from the Phase IIa trial suggest that treatment with CS1 in patients with pulmonary arterial hypertension (PAH) improve risk score, functional class and quality of life. The conclusion states that CS1 warrants further study.
