The PENTO Protocol in Medication-related Osteonecrosis of the Jaw

Medication-related osteonecrosis of the jaw (MRONJ) occurs after taking bisphosphonates or targeted therapies. It leads to a significant decrease in quality of life with pain, eating and chewing disorders, and malnutrition. Current treatments are only partially effective. PENTO (pentoxifylline and tocopherol) has been shown to be effective in maxillary osteoradionecrosis. The objective of this study is to evaluate the proportion of bone recovery in patients receiving PENTO in MRONJ at 12 months.

Potential of Low-Frequency Raman Spectroscopy for Structural Screening of Lipid-Based Drug Delivery Systems for Poorly Water-Soluble Drugs

Dynamic changes in the structure of lipid lyotropic liquid crystals upon mixing of different lipid systems were investigated using simultaneous low-frequency Raman (LFR) and small-angle X-ray scattering (SAXS) at a synchrotron facility. Monoolein and phytantriol were investigated as lipids with broadly highlighted potential for drug delivery. Their dispersed cubic phase structures were perturbed by mixing with a vitamin E acetate emulsion, known to modify the mesophase structures formed by monoolein and phytantriol, in the presence or absence of a lipophilic drug (cinnarizine). This mixing strategy enabled the exploration of structurally distinct and compositionally complex scenarios to establish direct correlations between LFR spectral features and mesophase structural parameters determined by SAXS, thereby assessing how LFR can be used to sensitively track composition-driven mesophase transformations. Crucially, the concurrent LFR-SAXS approach also allowed the evaluation of the affinity for cinnarizine to favorably interact with these lipids and form specific nanostructures leading to increased drug solubilization. These results illustrate how combined LFR-SAXS measurements can inform and expand the future use of LFR as a partially independent, structure-sensitive probe of lipid mesophase dynamics.

Leavening Inspired Synthesis of Mesoporous High‐Entropy Oxide Nanozymes for Detection and Discrimination of Multiple Biological Antioxidants

ABSTRACT:

High‐entropy oxides (HEOs) with mesoporous framework and high enzyme‐like catalytic properties hold great promise for biomedical applications. However, the synthesis of mesoporous HEO nanozymes remains challenging due to mesopore collapse during crystallization process. Herein, a leavening inspired synthesis strategy is proposed to fabricate a family of mesoporous HEO nanozymes. Glucose and urea produce a large amount of NH 3 under calcination, causing the precursor to expand and carbonize rapidly. The carbon supports the crystallization of the mesoporous framework and prevents it from collapsing. This method is universal for preparing a variety of mesoporous HEO nanozymes. Different kinds of mesoporous HEOs, including spinel‐, fluorite‐, and bixbyite‐type with large pore sizes (8–12 nm), high specific surface area (up to 63.2 m 2 /g), and well‐crystallized frameworks are synthesized, exhibiting oxidase (OXD)‐like activities. Leveraging oxidase‐like activity, a four‐channel colorimetric sensing array is constructed for simultaneous detection and discrimination of five key serum antioxidants: ascorbic acid, glutathione, L‐cysteine, uric acid, and tocopherol with range from 10 to 50 µmol/L. The Support Vector Machine algorithm is used, and the accuracy rate reaches 92%. This work provides a universal synthesis strategy for mesoporous HEO nanozymes with tunable compositions and excellent OXD‐like activities, showing great potential in complex bioanalytical applications.

Vitamin E and related tocols in cancer: Unraveling the paradox of antioxidant and pro-oxidant roles

Vitamin E, strictly defined as α-tocopherol, exhibits a complex dual role in cancer pathogenesis through its context-dependent pro-oxidant and antioxidant activities. Other members of the tocopherol and tocotrienol families (collectively referred to as tocols) have also been extensively studied for their bioactivities. At physiological levels, certain tocols such as γ-tocopherol and δ-tocotrienol act as potent antioxidants by neutralizing reactive oxygen species, inhibiting lipid peroxidation, and activating NRF2-mediated defenses, thereby suppressing tumor initiation, proliferation, and metastasis in models of colon, breast, and prostate cancers. On the contrary, under conditions such as high concentration of vitamin E in plasma, metabolic dysregulation, and the presence of transition metals (e.g., Cu²⁺), or specific genetic backgrounds, vitamin E exerts pro-oxidant effects. However, such effects are relatively rare and more often documented in vitro than in vivo. These include promoting reactive oxygen species generation, reducing p53 expression, enhancing angiogenesis, and facilitating cancer cell survival-ultimately driving tumor progression and metastasis. Critically, vitamin E modulates ferroptosis, a regulated cell death pathway pivotal in cancer; it inhibits ferroptosis via GPX4 upregulation and NRF2 activation but may paradoxically promote it in certain settings. Clinical studies highlight isomer-specific outcomes, with tocotrienols showing promise in adjuvant therapy. The dichotomy hinges on dosage, cellular microenvironment, redox balance, and vitamin E isoform. Overall, the biological impact of vitamin E is highly context-dependent, influenced by dosage, cellular microenvironment, redox status, and the specific tocol studied. Future research must prioritize isoform-specific mechanisms, optimal dosing strategies, and interactions with conventional therapies to harness vitamin E and related tocols's anticancer potential while mitigating risks.

New Delhi: In a significant move aimed at safeguarding public health, the Indian government has banned 156 fixed-dose combination (FDC) drugs, including a variety of antibiotics, painkillers, multivitamins, and medications for treating fever and hypertension. This decision marks the largest crackdown on FDCs since 2016, when 344 such drugs were prohibited. The Union Ministry of Health and Family Welfare issued a gazette notification on Thursday, officially prohibiting the manufacture, sale, and distribution of the banned FDCs. The ban was based on the recommendations of an expert panel that reviewed a total of 324 FDCs. Impact on Pharmaceutical Industry The banned FDCs include popular combinations such as mefenamic acid and paracetamol injections, commonly used for pain relief and fever, and omeprazole magnesium with dicyclomine HCl, used to treat abdominal pain.
