Voice disorders encompass a wide range of pathologies that result in various degrees of voice impairment and decreased quality of life, Minimal Associated Pathological Lesions (MAPLs), include a group of non-malignant growths on the vocal folds like vocal fold nodules, polyps, papilloma, Reinke's oedema, polypoidal degeneration and cysts, The pathophysiology of MAPLs include abnormal inflammatory response to local trauma. Histologically, the superficial layer of the lamina propria shows oedema, fibrosis, fibrin and hemosiderin deposit, interstitial haemorrhage and inflammatory cell infiltration, Treatment options include voice therapy, intralesional steroid injections, and surgical. Although vocal function improves and lesion regresses by vocal hygiene advice and voice therapy, each patient receives two therapy sessions per week for about 3 months of voice treatment in the form of a holistic voice rehabilitation program. A total of 20 sessions are conducted, with each session lasting approximately 20 minutes; Which some patients find difficult.
Vocal folds (VFs) local steroid injection could bridge the therapeutic gap between conservative treatment and conventional micro laryngeal surgery (MLS), Steroids are thought to be useful for MAPLs as they share common histological features, including oedema, thick basement membrane, inflammation, and vessel wall thickness.
Steroids act by inhibiting both collagen deposition and collagen synthesis. Also, it acts by inhibiting fibroblastic proliferation and activity plus reduction of the extracellular matrix deposition.
Office-based vocal fold steroid injection (VFSI) can be performed through transoral, percutaneous and trans nasal routes, With several benefits Compared to MLS, it may be conducted in the office under local anaesthesia in about 20 minutes, saving high costs of general anaesthesia and hospital admission, decreasing lost workdays associated with recurrent voice therapy sessions.
Hyaluronidase (HAase) is a preparation of proteolytic enzyme. Its injection is indicated as an adjuvant in subcutaneous fluid administration for achieving hydration and for improving the dispersion and the absorption of injected drugs. HAase can be used in phono-surgery or in office applications. HAase has been useful in the following indications: (1) HA over-injection in the vocal fold after injection laryngoplasty, (2) treatment of Reinke's oedema and (3) in selected patients with acute vocal fold haemorrhage, improvement of the absorption of blood products. HAase appears to be useful in the office and the operative setting with minimal complications. There is a growing evidence in literature that using a steroid in combination with HAase maximizes the anti-inflammatory properties of the steroid and minimizes the side effects of steroid therapy.
Although voice therapy may be the first line of treatment for many benign vocal folds lesions, satisfactory outcomes are not obtained in some patients for many reasons. A patient would attend 20 to 25 sessions of voice therapy in order to show improvement and this would take 2 to 3 months.
Some patients, especially those from far residence, would find it difficult for them to comply to this relatively lengthy course of therapy, resulting in a number of drop out cases. Similarly, some professional voice users would hope to have their voice problem alleviated in a faster way. Moreover some patients show poor understanding of the procedure itself and hence could not implement it properly. For those reasons voice therapy may not be appropriate for those patients.
Based on previous background, we thought that intralesional injection of a combination of HAase and steroids into MAPls would not only provide a faster way for the patient to restore her/his normal voice but also would augment the effect of steroids by improving its delivery and dispersion into the lesion.
This is the first study to assess the effect of intralesional injection of a combination of steroid and HAase in treatment of MAPLs.
The aim of this study is to assess the effect of intralesional injection of minimal associated pathological lesions of the vocal folds by a combination of steroids and hyaluronidase in terms of subjective and objective voice outcomes and lesion regression.

Cord Blood Cortisol Level - A Possible Predictor for Respiratory Distress Syndrome in Preterm Neonates

Background::Respiratory Distress Syndrome (RDS) is a leading cause of death in premature infants. There are different clinical/ biochemical markers associated with the RDS. One of the potential biochemical markers is cortisol in cord blood.Purpose::This study aims to correlate cortisol levels in preterm neonates with RDS and to establish whether cord blood cortisol is a reliable predictor for RDS.Materials and Methods::This prospective analytical study was conducted in a tertiary care hospital over nine months among fifty preterm neonates. Data was collected using proforma, and cord blood was collected at the time of delivery. Cortisol levels were compared and correlated to the development of RDS.Results and Discussion:The mean ± SD cord blood cortisol level among preterm neonates was 5.97 ± 2.74 (SD) μg/dl. The levels were higher in neonates whose mothers received antenatal steroids and were significantly lower (2.86 ± 1.66 μg/dl) in those who developed RDS. Association between cord blood cortisol level and RDS was found with an odds ratio of 57.4, which was statistically significant. The percentage of babies developing RDS in mothers not covered with antenatal steroids was significantly higher than those covered (p-value is 0.000). The mean cord blood cortisol levels were exceptionally low (1.89 μg/dl) in neonates who expired compared to those who survived (7.02 μg/dl).Conclusion::There is an association between cord blood cortisol levels and RDS. Hence, Cord blood cortisol levels may be used to predict RDS and help initiate early treatment, thus preventing mortality and morbidity.

High-throughput screening of new psychoactive substances and related compounds in food by UHPLC-Q/Orbitrap

Currently, there is concern about food products to which new psychoactive substances (NPS) have been added illegally. However, rapid, high-sensitivity, and high-throughput screening methods for such harmful substances have not been reported yet. In this study, a high-throughput non-targeted screening strategy was developed based on UHPLC-Q/Orbitrap. First, optimized pretreatment methods for typical high-risk matrices were established. Second, an accurate, efficient, and scalable high resolution mass spectrometry database containing 206 NPS and related compounds was constructed. The fragmentation pathways of three different types of fentanyl-related compounds were then systematically investigated to enhance compound identification. Additionally, a non-targeted screening strategy was developed using warning ions derived from 24 fentanyl-related compounds, streamlining the identification of NPS. This strategy was applied to 100 food samples with possible illegal additives and demonstrated its effectiveness. In summary, this study provides robust technical support for regulatory authorities in combating the illegal addition of food products.

Exclusive: A small biotech will bring Roche's Angelman drug back to life

Roche devastated the Angelman syndrome community in 2023, when it ended clinical development of rugonersen, a treatment candidate for the rare neurodevelopmental condition. But after losing a potential acquirer for the drug last year, the Swiss pharma giant has found new life for the drug. Oak Hill Bio is picking up rugonersen and aims to dose the first patient in a Phase 3 trial in the first quarter of 2026, the small biotech exclusively told Endpoints News . Oak Hill and Roche declined to disclose terms of the deal. “This is a positive step forward for patients living with Angelman syndrome and their families,” a Roche spokesperson said in an emailed statement. Angelman leads to severe developmental delays, leaving many unable to speak. The rare condition is thought to impact about 500,000 people worldwide, and is the result of a mutation in a gene inherited from a person’s mother. With rugonersen, an antisense oligonucleotide, Roche hoped to boost the paternal version of the gene, called UBE3A, in the central nervous system. But the pharma company stopped development of the treatment in early 2023 because it “did not meet Roche’s internal criteria.” At least 10 other Angelman treatment candidates have been stopped in recent years. Roche continues to develop a different type of drug, a small molecule called alogabat, for the condition. “When we looked at the data,” Oak Hill CEO Josh Distler said in an interview, “we concluded very rapidly that there was really promising data and it was desperately needed.” The Cambridge, MA-based biotech will be at least a few quarters behind Ionis and Ultragenyx in the development of an antisense oligonucleotide for Angelman. Ultragenyx dosed the first patient with its candidate, called GTX-102, in December, and Ionis plans to start its own Phase 3 for its candidate ION582 in May, according to the study entry in the federal trials database. “We benefit from a couple companies that have gone before us. We’ve been able to look at what they’ve done, and there’s some advantages of not being the first mover,” Distler said. The company is working with clinical investigators and patient groups and will share its Phase 3 design later this year, Distler said. Late-stage development may be a tall task for Oak Hill, which has said little about its funding to date. The company emerged in 2022 with rare disease drug candidates from Takeda . The company is backed by a family office, but Distler declined to disclose the investor’s name. Oak Hill is also working off of money it’s receiving as part of a license and development agreement with Chiesi for an experimental drug for complications of extremely premature birth. The partners didn’t disclose terms of that January 2024 deal.
