Fatigue, cognitive problems, post-exertional malaise (PEM) and postural orthostatic tachycardia syndrome (POTS) are common and debilitating symptoms after COVID-19. The pathophysiology of post-COVID is not well understood and there is no established biomedical treatment. Treatment options for post-COVID are thus much needed.
A promising candidate intervention is fluvoxamine, a selective serotonin reuptake inhibitor (SSRI), that may reduce post-COVID symptoms because of its regulatory effect on the (neuro) immune system, the hypothalamic-pituitary-adrenal (HPA) axis and the tryptophan system. The investigators will randomize 160 participants to either fluvoxamine or placebo for 12 weeks.
The investigators will use advanced functional neuroimaging techniques during cognitive challenge (optional substudy) and plasma biomarkers (inflammatory markers, cortisol, serotonin, IDO-2 activity), to facilitate identifying potential mechanistic pathways of post -COVID treatment.

Real-world pharmacovigilance and clinical risks of fluvoxamine: A disproportionality analysis based on FAERS data

Fluvoxamine is a widely used treatment for depressive disorders, yet real-world safety data remain understudied. This study aimed to evaluate the safety profile of fluvoxamine by analyzing adverse events (AEs) reported in the FAERS pharmacovigilance database. Data on fluvoxamine-related AEs were collected from the first quarter of 2008 to the second quarter of 2025. Disproportionality analyses were performed using the Reporting Odds Ratio (ROR), Proportional Reporting Ratio (PRR), Bayesian Confidence Propagation Neural Network (BCPNN), and Empirical Bayes Geometric Mean (EBGM) methods. A total of 694 AEs reports related to fluvoxamine were identified, spanning 27 different System Organ Classes and 99 Preferred Terms. Psychiatric disorders were the most frequently reported AEs. Notably, signals for eye disorders and ear and labyrinth disorders were stronger but not related to the pharmacological properties of fluvoxamine, thus warranting special clinical attention. Oscillopsia, neonatal behavioural syndrome, and activation syndrome ranked among the top three signals and require vigilance. Drug interactions were observed, leading to increased antipsychotic drug levels and extrapyramidal symptoms, such as slow speech, akinesia, and tardive dyskinesia. Cardiac-related AEs such as Brugada syndrome, long QT syndrome, and electrocardiogram abnormalities were also identified. Homicidal ideation and impulsive behaviour necessitate clinical risk management. Additionally, significant symptoms of serotonin syndrome were observed, including tic and clonus. Our study also uncovered AEs not previously documented in product labels, such as Cushing's syndrome, disinhibition, and vasoplegia syndrome. These findings provide a comprehensive overview of the real-world safety profile of fluvoxamine, highlighting both known and previously unreported AEs. Clinicians should remain vigilant for psychiatric, neurological, ocular, and cardiac AEs, as well as potential drug interactions. This study offers valuable evidence to inform clinical monitoring, risk management, and future research aimed at optimizing the safe use of fluvoxamine in diverse patient populations.

Temporal Patterns of Adverse Events Associated With Selective Serotonin Reuptake Inhibitors: A Global Pharmacovigilance Analysis of Early-Onset Versus Late-Onset Toxicity

Background Selective serotonin reuptake inhibitors (SSRIs) are commonly used as first-line antidepressant medications, but adverse events (AEs) remain a common reason for treatment discontinuation. Understanding when these AEs occur can help improve patient care and treatment adherence. Objective This study aims to explore the timing of AEs associated with six commonly used SSRIs: sertraline, fluoxetine, fluvoxamine, paroxetine, citalopram, and escitalopram using global pharmacovigilance data, with a focus on early- versus late-onset profiles. Methods This study analyzed Individual Case Safety Reports (ICSRs) from VigiBase, the World Health Organization's global safety database. Reports were included if an SSRI was the suspected drug and time-to-onset (TTO) data were available. AEs with at least 10 reported TTOs were grouped as early-onset (TTO ≤28 days) or late-onset (TTO >28 days). Results A total of 1,428 AEs met the inclusion criteria. Of these, 914 (64%) were early onset, including nausea (median TTO: 1 day), insomnia and dizziness (2 days), and sexual dysfunction (16.5 days). Late-onset AEs, 514 (36%), included weight gain (31 days), hyperhidrosis (76.5 days), diabetes mellitus (151 days), and osteoporosis (959.5 days). Early AEs were mostly gastrointestinal, neurological, or activation-related; late AEs were largely metabolic or endocrine. Conclusions SSRIs show distinct temporal AE patterns. Early-onset symptoms require timely management to improve tolerability, while late-onset effects highlight the need for ongoing monitoring. These findings can inform personalized monitoring strategies and guide patient counseling to support safer long-term SSRI use.

Efficacy of the herbal product of Cuscuta campestris Yunck. and Polypodium vulgare L. in patients with obsessive-compulsive disorder: A triple-blind clinical trial

ETHNOPHARMACOLOGICAL RELEVANCE:

Cuscuta campestris Yunck. and Polypodium vulgare L. are medicinal plants widely used in traditional Persian medicine to treat various mental health conditions, such as Obsessive-compulsive disorder (OCD).

AIM OF THE STUDY:

OCD is among the top ten disorders that cause disability and decrease quality of life. Considering the lack of a definitive treatment method and the side effects of the existing drugs, the application of complementary medicine in treating this disorder seems reasonable. The current study aimed to investigate the therapeutic impact of a dodder and polypody herbal product (DPHP) in OCD patients.

MATERIALS AND METHODS:

In this triple-blind clinical trial, 84 individuals with OCD were assigned to two groups of DPHP and placebo receivers at random. While taking fluvoxamine tablets (100 mg twice daily), the first group was prescribed DPHP tablets, and the second group was prescribed placebo tablets twice daily. The intensity of manifestations and the participant's quality of life were evaluated at the beginning of the trial and after four and eight weeks of intervention using the Yale-Brown and WHOQOL-BREF questionnaires, respectively.

RESULTS:

The severity of OCD symptoms in both groups decreased significantly after four and eight weeks (P-value <0.001). The DPHP group's reduction rate was significantly higher than the placebo group (P-value = 0.043). The quality of life improved significantly in both groups (P-value <0.001). However, the increase in the quality of life in the DPHP group was significantly higher compared to the placebo group (P-value = 0.042).

CONCLUSIONS:

DPHP, taken alongside fluvoxamine, can effectively alleviate the symptoms of OCD and increase the quality of life. Therefore, it could be regarded as a complementary therapeutic alternative to OCD.

Antidepressants Vary Significantly in Cardiometabolic Effects

TUESDAY, Oct. 28, 2025 -- Antidepressants induce cardiometabolic and other physiological alterations, which vary between medications, according to a study published online Oct. 21 in The Lancet . Toby Pillinger, Ph.D., from King's College London, and colleagues compared and ranked antidepressants based on physiological side effects using data from randomized controlled trials. A total of 151 studies and 17 U.S. Food and Drug Administration reports met the inclusion criteria. Overall, 58,534 participants were included and 30 antidepressants were compared to placebo. The researchers found that in terms of metabolic and hemodynamic effects, there were clinically significant differences between antidepressants, including an approximate difference of 4 kg in weight change between agomelatine and maprotiline, a difference of more than 21 beats per minute in change in heart rate between fluvoxamine and nortriptyline, and a difference of more than 11 mmHg in systolic blood pressure between nortriptyline and doxepin. Increases in total cholesterol were seen in association with paroxetine , duloxetine , desvenlafaxine , and venlafaxine ; duloxetine was associated with increased glucose concentrations, despite reductions in body weight with all drugs. No strong evidence was found for any antidepressant affecting QTc or concentrations of sodium, potassium, urea, or creatinine to a clinically significant extent. "We found strong evidence that antidepressants differ markedly in their physiological effects, particularly for cardiometabolic parameters," the authors write. "Treatment guidelines should be updated to reflect differences in physiological risk, but choice of antidepressant should be made on an individual basis, considering clinical presentation and preferences of patients, carers, and clinicians." Some Antidepressants Linked to Weight, Heart Health Changes

THURSDAY, Oct. 23, 2025 — Antidepressants are widely used to treat depression and anxiety , but a new study finds that some of these medications can cause noticeable changes in weight, heart rate and cholesterol levels. The study, published Tuesday in The Lancet , analyzed data from more than 58,000 participants to compare 30 different antidepressants or a placebo for an average of eight weeks. Researchers found what they called “clinically significant” differences in how each medication affected the body’s metabolism and blood flow. For example, people taking maprotiline gained an average of 4 pounds, while nearly half of users of amitriptyline experienced some weight gain. In contrast, agomelatine was linked to an average weight loss of 5.5 pounds among 55% of participants. Heart rate changes also differed. Some drugs, like nortriptyline , increased heart rate by about 14 beats per minute, while fluvoxamine caused a decrease of around 8 beats per minute. Researchers emphasized that while these findings highlight important differences, antidepressants remain effective treatments for depression and anxiety . The results show the need for personalized prescriptions so that doctors can better match a drug to patient’s health needs. "We do know that these medications are life-saving and life-changing, but they can have variation from person to person depending on a person's metabolism, their age (and) what other medications they're on," psychiatrist Dr. Sue Varma told CBS News after reviewing the findings. She noted that untreated depression can also cause physical changes like weight gain or loss, so it’s important to “weigh the risks and the benefits” of starting medication. Varma added that doctors should discuss possible side effects with patients before prescribing antidepressants and regularly monitor for any changes. FDA Accepts Shionogi’s Ensitrelvir NDA for Review as the First Oral Therapy for the Prevention of COVID-19 Following Exposure

OSAKA, Japan, September 3, 2025 – Shionogi & Co., Ltd. (Head Office: Osaka, Japan; Chief Executive Officer: Isao Teshirogi, Ph.D.; hereafter “Shionogi”) announced the U.S. Food and Drug Administration (FDA) has accepted for review a New Drug Application (NDA) from Shionogi Inc., a New Jersey-based subsidiary of Shionogi, for ensitrelvir (Generic name: ensitrelvir fumaric acid, Code No.: S-217622), an investigational oral antiviral for the prevention of COVID-19 following exposure to an infected individual. The FDA has set an action date of June 16, 2026 under the Prescription Drug User Fee Act (PDUFA). The NDA is supported by results from the global, double-blind, randomized, placebo-controlled Phase 3 study, SCORPIO-PEP, which studied ensitrelvir as post-exposure prophylaxis (PEP). If approved, ensitrelvir would be the first and only oral therapy for the prevention of COVID-19 following exposure to an infected individual.1,2 SARS-CoV-2 remains highly transmissible and up to half of people living with an infected individual may develop COVID-19.3,4 The virus is constantly evolving, with novel symptoms reported.5,6 COVID-19 continues to impact daily life and may lead to absences from school and work, may cause long COVID, and in some cases, may progress to severe disease.7-12 Even patients with mild COVID-19 may experience worsening of preexisting chronic conditions.13-17 Following exposure to COVID-19, the best way to avoid its potentially serious and long-term complications is to stop viral replication, which prevents the development of the disease.1,2 There are currently no approved antiviral therapies proven to prevent COVID-19 following exposure.1,2,18 COVID-19 vaccines are received before exposure, and they do not stop viral replication.19,20 Other COVID-19 antivirals are taken following exposure and diagnosis, after viral replication has occurred and disease is already established.21 “Shionogi has a long history of innovation in infectious disease treatment and prevention. About ensitrelvir Ensitrelvir is a SARS-CoV-2 main protease inhibitor created through joint research between Hokkaido University and Shionogi. SARS-CoV-2 has an enzyme called the main protease (3-CL), which is essential for the replication of the virus. Ensitrelvir suppresses the replication of SARS-CoV-2 by selectively inhibiting the main protease. Ensitrelvir, known as Xocova® in countries where it is approved, received emergency regulatory approval in Japan in November 2022 and full approval in March 2024 for the treatment of COVID-19 based on results from SCORPIO-SR, a Phase 3 study conducted in Asia, during the Omicron-dominant phase of the pandemic. In SCORPIO-SR, ensitrelvir showed both clinical symptomatic efficacy (symptom resolution sustained for at least 24 hours) for five typical Omicron-related symptoms (primary endpoint) and antiviral efficacy (key secondary endpoint) in a predominantly vaccinated population of patients with mild-to-moderate SARS-CoV-2 infection, regardless of risk factors. Most adverse events were mild in severity and no deaths were seen in the study. Among the most common treatment-related adverse events were temporary decreases in high-density lipoprotein and increased blood triglycerides, as observed in previous studies. Results from this study were published in JAMA Network Open. In 2025, Shionogi submitted two new drug applications in Japan for post-exposure prophylaxis of COVID-19 and for COVID-19 treatment in pediatric patients aged six to under 12 years. The pediatric submission is based on a multicenter, randomized, double-blind, placebo-controlled trial of ensitrelvir in mild-to-moderate COVID-19 patients aged 6 to 12 years in Japan. The study confirmed safety and tolerability and found the pharmacokinetics of ensitrelvir in this age group to be similar to that in adults. Ensitrelvir became available in Singapore for the treatment of COVID-19 via a Special Access Route application in 2023, and it is currently under regulatory review in Taiwan for the treatment of COVID-19. Ensitrelvir is also under regulatory review with the European Medicines Agency for COVID-19 post-exposure prophylaxis and treatment.22 Ensitrelvir is an investigational drug outside of Japan and Singapore. In addition, the brand name Xocova® has not been approved for use outside of Japan and Singapore and pertains only to the approved drug in Japan and Singapore.
