This study will determine the functional status of the nasal immune environment with LAIV exposure in COPD persons with frequent exacerbations (defined as individuals with two or more episodes of worsening in COPD symptoms requiring treatment with antibiotics and/or steroids in the prior 12 months) and COPD persons without frequent exacerbations to determine acute exacerbation of COPD (AECOPD)-associated dysfunction in a) cytokines and immune effector cells of the nasal mucosa and b) viral replication. The investigators hypothesize that: 1) COPD frequent exacerbators, compared to COPD infrequent exacerbators, will demonstrate altered mucosal immune responses to LAIV exposure, and 2) COPD frequent exacerbators, compared to COPD infrequent exacerbators, will demonstrate increased markers of influenza viral replication after LAIV exposure.

Influenza vaccine effectiveness against medically-attended influenza infection in 2023/24 season in Hangzhou, China

From 2020, influenza viruses circulation was largely affected by the global coronavirus disease (COVID-19) pandemic, notably leading to the extinction of the B/Yamagata lineage and raising questions about the relevance of the quadrivalent influenza vaccine, which includes this lineage. Evaluating vaccine effectiveness (VE) against influenza infections is important to inform future vaccine programs. A test-negative case-control study was conducted in five tertiary hospitals in Hangzhou, the capital city of Zhejiang province, China, enrolling medically-attended patients aged >6 months who presented with influenza-like illness (ILI) from October 1, 2023, to March 31, 2024. The VE was estimated using multivariate logistic regression models adjusted for sex, age, influenza detection methods, and influenza testing timing. Of the 157,291 medically-attended ILI participants enrolled 56,704 (36%) tested positive for influenza. Adjusted overall VE against any medically-attended influenza infection was 48% (95% Confidence interval [CI]: 46%-51%). The overall VE of the trivalent inactivated influenza vaccine (IIV3) was 59% (95% CI: 50%-66%), followed by the trivalent live attenuated vaccine (LAIV3) (VE = 53%, 95% CI: 42%-62%) and quadrivalent inactivated influenza vaccine (IIV4) (VE = 47%, 95% CI: 45%-50%). IIV3 provided even better protection against medically-attended influenza B infection than IIV4 (VE = 87%, 95% CI: 81%-92% for IIV3 versus VE = 53%, 95% CI: 50%-57% for IIV4). In the 2023/24 season in Hangzhou, China, the influenza vaccine offered moderate protection during a major epidemic. The results supported the World Health Organization recommendation to exclude the B/Yamagata lineage antigen in quadrivalent influenza vaccines in 2023.

Exogenous Increases in Basic Income Provisions Increase Preventive Health-Seeking Behavior: A Quasi-Experimental Study

INTRODUCTION:

Universal Basic Income (UBI) policies have the potential to promote a wide range of public health objectives by providing those who qualify with direct cash payments. One overlooked mechanism of particular importance to health researchers is the possibility that guaranteed income might increase consultation of primary and preventive care (e.g., annual doctors' visits; regular vaccination against infectious disease) by providing people with both the time and monetary resources to do so, thereby improving general health.

METHODS:

This study assesses the effects of an exogenous shock to Alaska's UBI payments to all state residents: a 2022 decision to reclassify dividend "energy relief" provisions as nontaxable (thereby increasing payments by approximately $2,000 inflation-adjusted dollars). It estimates quasi-experimental treatment effects (in 2022 vs. 2021) via mixed linear probability models that compare pre/post policy change in primary care seeking behavior in Alaska vs. the US adult population; controlling for respondent-level fixed effects and state-level random effects. Data were collected in 2021-2022, and analyzed in 2024.

RESULTS:

The likelihood that Alaskans sought primary care postreform (relative to beforehand) increased by 6pp, which was significantly greater than the same difference (2pp) observed across all other (non-UBI) US States (∆=4pp, p<0.01). The study provides suggestive evidence that comparatively fewer Alaskans had difficulty affording primary care during this period, with less-consistent evidence of increased flu vaccine uptake.

CONCLUSIONS:

Enhanced UBI payments ought to be thought about as a form of health policy, as they have the potential to advance a wide range of health objectives related to preventive care.

Formulation development of a stable influenza recombinant neuraminidase vaccine candidate

Current influenza vaccines could be augmented by including recombinant neuraminidase (rNA) protein antigen to broaden protective immunity and improve efficacy. Toward this goal, we investigated formulation conditions to optimize rNA physicochemical stability. When rNA in sodium phosphate saline buffer (NaPBS) was frozen and thawed (F/T), the tetrameric structure transitioned from a "closed" to an "open" conformation, negatively impacting functional activity. Hydrogen deuterium exchange experiments identified differences in anchorage binding sites at the base of the open tetramer, offering a structural mechanistic explanation for the change in conformation and decreased functional activity. Change to the open configuration was triggered by the combined stresses of acidic pH and F/T. The desired closed conformation was preserved in a potassium phosphate buffer (KP), minimizing pH drop upon freezing and including 10% sucrose to control F/T stress. Stability was further evaluated in thermal stress studies where changes in conformation were readily detected by ELISA and size exclusion chromatography (SEC). Both tests were suitable indicators of stability and antigenicity and considered potential critical quality attributes (pCQAs). To understand longer-term stability, the pCQA profiles from thermally stressed rNA at 6 months were modeled to predict stability of at least 24-months at 5°C storage. In summary, a desired rNA closed tetramer was maintained by formulation selection and monitoring of pCQAs to produce a stable rNA vaccine candidate. The study highlights the importance of understanding and controlling vaccine protein structural and functional integrity.

"I always have a little bit of dread when flu season is around the corner," Kristina Bryant, M.D., a pediatric infectious disease doctor at Norton Children's in Louisville, Kentucky, told NBC News . "Children experience illness and suffering during flu season, and much of that can be prevented through vaccination." For the entirety of the last flu season, just over half of children -- 55 percent -- received the flu shot, marking the lowest rate in 12 years. Alicia Budd, head of the CDC domestic influenza surveillance team, told NBC News that while flu vaccination rates were slowly increasing before the pandemic, they have not rebounded since. This year's flu shot covers the two most common strains of the virus circulating, H1N1 and H3N2. Budd emphasized that any strain of the flu can be dangerous for children. THURSDAY, Dec. 12, 2024 -- Despite a record number of American children dying from the flu last year, the percentage of kids getting their flu shots keeps falling. In new data posted this week by the U.S. Centers for Disease Control and Prevention , only 37% of kids had received the vaccine as of Nov. 30 -- down from 43% at the same time last year. This decline is raising concerns among pediatricians, who are already seeing an uptick in flu cases this season. “I always have a little bit of dread when flu season is around the corner,” Dr. Kristina Bryant , a pediatric infectious disease doctor at Norton Children’s in Louisville, Ky., told NBC News . “Children experience illness and suffering during flu season, and much of that can be prevented through vaccination.” For the entirety of the last flu season, just over half of children -- 55% -- received the flu shot, marking the lowest rate in 12 years. However, a preliminary report released by the CDC in October suggested the flu shot might be less effective at preventing hospitalizations for children ages 5 and younger this year, with just 39% effectiveness. Last year, the vaccine’s effectiveness in preventing hospitalizations was estimated to be as high as 61%. Flu season was especially deadly for children last year, with 205 pediatric deaths -- the highest number seen since before the pandemic, the report stated. However, the CDC estimates that the actual number of flu-related child deaths could be much higher, with 724 potentially occurring. Experts believe the high death toll last year can be partly attributed to a long flu season and lower vaccination rates. “Last year was more drawn-out, so unfortunately, there was more opportunity for children to get infected,” Budd said. The CDC continues to recommend that everyone age six months and older get a flu shot every year. For most people, a single shot is enough.
