Trans-arterial chemoembolization (TACE) is the most commonly used therapy for patients with unresectable hepatocellular carcinoma (HCC). TACE is a minimally invasive procedure that involves placing a catheter into the artery in the liver that feeds the tumor, administering chemotherapeutics and then blocking the artery with embolics in order to kill tumor cells by depriving them of essential oxygen and nutrients. While TACE has a proven survival benefit, local recurrence is common, and long-term survival rates are poor. Prior studies demonstrate that HCC cells survive the oxygen and nutrient deprivation through autophagy, a process of cellular self-eating, to provide nutrients required for survival. The proposed project will leverage this dependency to develop a novel approach to TACE that integrates autophagy inhibition to improve therapeutic response by increasing tumor cell killing and enhancing anti-tumor immunity.

Lipiodal Injection Technique for Free Flap Sparing Adjuvant Radiation Planning in Head and Neck Cancer

Patients with advanced mouth/throat cancers require surgery to remove the cancer followed by reconstruction with transplanted skin/muscle/bone from another location in their body to restore function. This is called a free-flap (skin/muscle/bone used from another part of the body to reconstruct the cancer site). Post-operative radiation therapy is often recommended to reduce the risk of cancer coming back. Unfortunately, radiation can exacerbate issues with swallowing and speech and lead to other complications. To date, there has been no mechanism to allow sparing of the healthy tissue from radiation while ensuring delivery to all at-risk areas.
This is a proof-of-principle study that injects a safe contrast agent (material injected to enhance x-ray and ct images) at the time of surgery to improve delineation of the interface between the removed mouth/throat cancer and the healthy tissue reconstructing the defect. In doing so, the investigators hope to demonstrate the potential to spare post-operative radiation to the healthy reconstructive tissue and improve patient function.
The investigators propose using Lipiodal, a poppyseed oil-extract that has been in safe clinical use for > 100-years, in 10 patients. There will be no change to the way in which patients are treated for their mouth/throat cancer or in the way radiation is delivered. The investigators will assess the feasibility and safety of injecting Lipiodal at the time of surgery and visualization on post-operative CT scans. This has been shown to be both safe and feasible in patients with other cancers, such as bladder cancer, however patients will be closely monitored for any side effects. Patients will provide informed consents and be able to withdraw at any time.

Regorafenib and R837 nanoparticle-stabilized Pickering emulsions overcome the angiogenesis-immune tolerance axis in transarterial chemoembolization

Transarterial chemoembolization (TACE) is hindered by the instability of embolic agents, as well as the tumor microenvironment post-embolization, which is characterized by angiogenesis and immune tolerance. Here, we identify a coupled angiogenesis-immune tolerance axis as a key mechanism underlying post-TACE therapeutic resistance. In this study, we developed a novel combination formulation of regorafenib and R837 designed to reprogram the tumor microenvironment following embolization. Using a supercritical carbon dioxide-assisted method, we successfully synthesized carrier-free pure drug nanoparticles of regorafenib and R837, achieving nanoparticles-stabilized lipiodol Pickering emulsions (PE-RR) preparation. This stable formulation not only significantly enhances the embolic stability and sustained drug release, but also effectively reverses post-embolization hypoxia, angiogenesis and immune tolerance through the therapeutic combination of regorafenib and R837. This study demonstrated that locoregional therapy with PE-RR significantly suppressed the hypoxia-angiogenesis signaling axis. These effects are mediated by the coordinated inhibition of angiogenesis (HIF-1α/VEGF axis) and activation of anti-tumor immunity, establishing a mechanistic linkage between vascular normalization and immune reprogramming. By strengthening antigen release and localized delivery of immune agonists, PE-RR facilitated anti-tumor immune activation and improved the therapeutic response to embolization, overcoming the adverse tumor microenvironment associated with conventional lipiodol embolization. Additionally, in bilateral tumor models, PE-RR could boost the therapeutic efficacy of PD-1 checkpoint blockade, increase the generation of anti-tumor immune memory and minimize the low responsiveness of immune checkpoint inhibitors in liver cancer. The stable Pickering embolic emulsion offers an innovative approach for the clinical translation of embolization therapy.

Lipiodol-Based Balloon-Occluded Transcatheter Arterial Chemoembolization Can Be a Curative Treatment Option for Hepatocellular Carcinoma

PURPOSE:

To investigate whether ethiodized oil-based balloon-occluded transcatheter arterial chemoembolization (B-TACE) could be a curative treatment option for hepatocellular carcinoma (HCC) on the basis of pathological evaluation and clinical follow-up results.

MATERIALS AND METHODS:

Eighty-two patients who presented with treatment-naive HCC between September 2018 and January 2024 were included in this single-center retrospective study. Radiological evaluations of oily subsegmentectomy, defined as necrotic area that includes the entire ethiodized oil-laden HCC and peritumoral liver parenchyma with absent perfusion on arterial and portal venous image, were performed.

RESULTS:

Subsegmental B-TACE was technically successful in all 82 patients with a single HCC (mean diameter, 36 mm; range, 18-65 mm). A complete response (CR) and oily subsegmentectomy on first follow-up computed tomography (CT) were observed in 81 (98.8%) and 33 (40.2%) patients, respectively. Curative response, including pathological complete necrosis (36 of 39) and CR without local recurrence (LR) (28 of 43), was achieved during follow-up in 64 (78%) of 82 patients. During the median follow-up of 39.1 months, 14 HCCs (32.6%) demonstrated LR. All 33 patients with oily subsegmentectomy showed pathological complete necrosis (n = 19), and none demonstrated any LR (n = 14). Univariate log-rank analysis showed that the oily subsegmentectomy-positive group had a significantly lower LR rate than the oily subsegmentectomy-negative group (P = .013).

CONCLUSIONS:

Ethiodized oil-based subsegmental B-TACE is an effective method with a high initial CR rate in patients with a single HCC. Oily subsegmentectomy on first follow-up CT is a good predictor of complete necrosis of HCC. Therefore, ethiodized oil-based B-TACE can be considered a curative treatment option in selected patients showing oily subsegmentectomy.

ABSTRACT:

Background:

Malignant hepatocellular neoplasm, not otherwise specified (HCN, NOS), is a rare primary liver tumor showing overlapping features of hepatoblastoma and hepatocellular carcinoma. Because of its rarity and histopathologic heterogeneity, the optimal treatment strategy for HCN, NOS remains undefined.

Aim and Methods:

We report a case of a 20‐year‐old woman with initially unresectable HCN, NOS, who was treated with hepatic arterial infusion chemotherapy using a fine‐powder formulation of cisplatin suspended in Lipiodol combined with continuous 5‐fluorouracil administration (New FP). The therapeutic response was assessed radiologically and serologically after each treatment cycle, and the patient subsequently underwent surgical resection after adequate tumor regression.

Results:

After eight cycles of New FP, marked tumor shrinkage and a significant decline in tumor markers were achieved, enabling subsequent systemic chemotherapy according to the SIOPEL‐4 regimen. Thereafter, conversion hepatectomy was performed, and histological examination of the resected specimen demonstrated approximately 90% tumor necrosis. The patient has remained disease‐free for 20 months after surgery.

Conclusion:

This case demonstrates the potential of New FP as an effective hepatic arterial infusion regimen capable of achieving conversion therapy in rare liver malignancies such as HCN, NOS.
