This study will evaluate the tolerability and effectiveness of four FDA-approved treatments in Neurofibromatosis Type 1 Cutaneous Neurofibromas. These treatments are: a 1064nm laser, a 755nm laser, and Kybella and Polidocanol injections. Each patient will have a treatment and a control site.
This study is designed with the goal of improved efficacy/tumor reduction via multiple treatment visits. If there is minimal to no clinical improvement in tumor size with one treatment after three treatment visits, the subject will be given the option of crossover treatment with the most effective of the four treatments. Three treatment visits with the crossover treatment will then take place.

Sclerotherapy for hemorrhoidal disease: systematic review and meta-analysis

Abstract:

Background:

This systematic review and meta-analysis aimed to evaluate the safety and efficacy of sclerotherapy methods for hemorrhoidal disease (HD) over the past 40 years.

Methods:

The review followed the 2020 Preferred Reporting Items for Systematic Reviews and Meta-analyses (PRISMA) guidelines. A comprehensive literature search was conducted, including studies reporting the use of sclerotherapy in patients with HD. Study eligibility criteria were defined, and data were extracted independently by the authors. Random-effects meta-analyses were performed to assess outcomes of interest.

Results:

Out of 1965 records identified, 44 studies met the inclusion criteria, involving 9729 patients. The majority of studies were conducted in Japan, followed by the UK, Italy, and Portugal. The median age of participants was 52 years, and the majority were male. The Goligher grade distribution indicated varying degrees of HD severity. Sclerotherapy was predominantly administered through anoscopy, with polidocanol being the most commonly used agent. The procedure was generally performed without pre-injection analgesia. The meta-analysis of 14 randomized controlled trials (RCTs) revealed that sclerotherapy was not inferior to control interventions in terms of success rate (risk ratio [RR] 1.00, 95% CI 0.71–1.41) and recurrence rate (RR 1.11, 95% CI 0.69–1.77), while resulting in fewer complications (RR 0.46, 95% CI 0.23–0.92).

Conclusions:

This systematic review highlights the safety and efficacy of sclerotherapy for HD, which yields similar success rates and fewer complications compared to other conservative or surgical approaches. Further research is warranted to optimize sclerotherapy techniques and evaluate long-term outcomes.

Outcomes of a single dose of injection polidocanol application in aneurysmal bone cyst

Background:

Aneurysmal bone cysts are common osteolytic tumours that have ability to expand and produce pathological fractures. Recurrence is a major issue in such tumours posing a treatment dilemma to clinicians. Injection polidocanol has shown promising results, but the number of injections still remains a topic of debate.

Methodology:

A prospective interventional study done on 13 patients of biopsy-proven aneurysmal bone cysts. Patient related outcome measures(PROM's) seen using VAS score, SF-12 score and ROM of the involved extremity. Radiologically volume of tumour and cortico-medullary ratio were calculated. All patients received single dose of injection polidocanol and were followed up to 6 months.

Results:

The mean age of patients was 9.5 years, with slight female predominance. Most cases had Campana grade 1. Mean VAS score improved from 9/10 to 1/10, ROM of the proximal and distal joints of the involved bone improved from 60 % (± 20 %) to 100 %. Mean SF12 score increased from 41 (±7): 47 (±3) to 54 (±2): 58 (±2) at the end of 6 months(p < 0.05).Mean volume of the lesion decreased from 6.30 cc (± 1.30) to 2.86 cc (±1.34) (p < 0.0005) with a reduction of MCR from 4.8 (±0.6) to 3.6 (±0.5) on a similar time span (p < 0.0005) at end of 6 months. 2 patients showed recurrence with none of the patients showing any complication.

Conclusion:

Single dose of Injection Polidocanol for Aneurysmal Bone Cysts is a unique study, not described in literature before. The smaller lesion in initial staging can be treated effectively with single dose of polidocanol reducing morbidity and treatment cost to the patient. It is as good as giving multiple doses of Polidocanol as per as functionality of the limb is concerned.Also, we propose a new parameter "Medullary-cortical ratio", which is an effective way to assess the improvement and resolution of ABC after administration of Polidocanol.

Percutaneous Sclerotherapy of Large Venous Malformations Using Consecutive Polidocanol and Bleomycin Foam: MR Imaging Volumetric and Quality-of-Life Assessment

PURPOSE:

To retrospectively evaluate sclerotherapy using consecutive polidocanol and bleomycin foam (CPBF) for large untreated venous malformations (VMs) and/or those resistant to prior treatment.

MATERIALS AND METHODS:

This retrospective study included all patients treated with CPBF for untreated VMs larger than 10 mL and/or refractory to treatment between May 2016 and October 2019. Baseline and follow-up VM volumes were measured on fat-suppressed T2-weighted magnetic resonance (MR) imaging. Outcome was evaluated on postprocedural MR imaging volumetry and by a retrospective survey assessing clinical response and adverse events. Imaging response was considered good for volume reduction from 50% to 70% and excellent for volume reduction ≥70%. Symptoms and quality-of-life (QoL) scores were compared before and after CPBF sclerotherapy.

RESULTS:

Forty-five patients (mean age, 16 years; range, 1-63 years; 25 males) with 57 VMs were analyzed and treated by 80 sclerotherapy. Sixty percent (27 of 45) of patients had undergone prior treatment for VM. Median VM volume was 36.7 mL (interquartile range, 84 mL) on pretherapy MR imaging. Good and excellent results after the last sclerotherapy were achieved in 36% (16 of 45) and 29% (13 of 45) of patients, respectively, corresponding to a decrease of >50% in 60% (34 of 57) of VMs. QoL score increased by at least 3 points, regardless of initial symptoms. Most patients did not desire additional sclerotherapy owing to near complete symptomatic relief, even for patients who did not achieve a good response. Swelling, pain, and motor impairment scores significantly improved after CPBF. Adverse events included fever (44%, 15 of 34) and nausea/vomiting (29%, 10 of 34).

CONCLUSIONS:

CPBF sclerotherapy represents an effective therapy for large and/or refractory VMs with minimal adverse events.

10xBio Announces Data from Interim Analysis of Phase 2b Clinical Trial Showing Superior Efficacy of Novel Drug for Submental Body Contouring

The findings were presented in a late-breaking research abstract at the 2024 American Academy of Dermatology (AAD) Annual Meeting SAN DIEGO, March 11, 2024 /PRNewswire/ -- 10xBio, LLC, a biotechnology company that focuses on new uses for existing therapies and technologies, announced today promising initial clinical trial results on its novel injectable drug for submental body contouring, 10XB101. An innovative alternative to other fat removal options, 10XB101 provides superior efficacy, fewer side effects, enhanced patient tolerability and shorter treatment intervals. In this randomized, double-blind, placebo-controlled study with three active treatment groups, patients received multiple injections of 10XB101 in a predefined pattern, with up to six treatments spaced four weeks apart. Efficacy was assessed by clinician- and patient-based assessments (CSFS and PSFS) using a submental fat rating scale by comparing Baseline to outcomes four weeks after their last treatment. "The results of this study clearly demonstrated far superior efficacy and tolerability for 10XB101 than the currently marketed body contouring drug, deoxycholate, which I have more than 10 years of experience with," said Mitchel P. Goldman, M.D., a dermatologic surgeon and medical director of Cosmetic Laser Dermatology. "10XB101 has the appropriate product-market fit to capture the tremendous opportunity for submental contouring. In addition, its attributes hold promise for potential expansion to other body contouring applications, such as treatment of the abdomen and flanks." Key findings include: In the intent to treat (ITT) population, more than 50% of patients in the two higher dose groups achieved a concordant Grade 2 or more improvement in both their CSFS and PSFS scores, the FDA designated primary endpoint, compared to 0% in the placebo group. In the completer population (e.g. patients receiving at least four doses of 10XB101), about 80% of patients in the two higher dose active groups demonstrated concordant Grade 2 or more improvement in both their CSFS and PSFS scores, compared to 0% in the placebo group. Onset of effect was rapid in the higher dose groups with more than 65% of ITT and more than 84% of completer populations achieving a minimum of a Grade 1 improvement or greater after their initial two treatment sessions. Local injection site reactions were measured using a four-point scale ranging from absent to severe and included bruising, edema, pain and erythema at each visit during the trial. About 98% of injection site reactions were measured as zero (absent) or one (mild). The results compare favorably to deoxycholate, the only marketed drug for non-invasive fat removal in the U.S., with up to four times as many patients achieving concordant Grade 2 or more improvement on the CSFS/PSFS. With 10XB101, the frequency and severity of injection site reactions are meaningfully lower than with its counterpart.
