Quantitative analysis of proteins and their post-translational modifications (PTMs) in complex biological samples is critical to understanding cellular biology as well as disease detection and treatment. Top-down proteomics methods provide a "bird's eye" view of the proteome by directly detecting and quantifying intact proteoforms. Here, we developed a high-throughput quantitative top-down proteomics platform to probe intact proteoform and phosphoproteoform abundance changes in HeLa cells as a result of treatment with staurosporine (STS), a broad-spectrum kinase inhibitor. In total, we identified and quantified 1187 proteoforms from 215 proteoform families. Among them, 55 proteoforms from 37 proteoform families were significantly changed upon STS treatment. These proteoforms were primarily related to catabolic, metabolic, and apoptotic pathways that are expected to be impacted as a result of kinase inhibition. In addition, we manually evaluated 25 proteoform families that expressed one or more phosphorylated proteoforms. We observed that phosphorylated proteoforms in the same proteoform family, such as eukaryotic initiation factor 4E binding protein 1 (4EBP1), were differentially regulated relative to the unphosphorylated proteoforms. Combining relative profiling of proteoforms within these proteoform families with individual proteoform profiling results in a more comprehensive picture of STS treatment-induced proteoform abundance changes that cannot be achieved using bottom-up methods.

Treatment of hereditary hemorrhagic telangiectasias with sclerotherapy: A case series

BACKGROUNDHereditary hemorrhagic telangiectasia (HHT) is characterized by abnormal blood vessel formation. One treatment for HHT-related arteriovenous malformations (AVMs) is sclerotherapy, which collapses the blood vessels by irritating the endothelial lining.METHODSThis case series describes two HHT patients undergoing in-office sodium tetradecyl sulfate sclerotherapy for non-nasal telangiectasias and AVMs. The first patient had AVMs on the red lip while the second patient had an AVM and several telangiectasias on the tongue.RESULTSBoth patients' AVMs required only one treatment visit and were no longer noticeable within a week. Neither of the patients had any side effects from treatment.CONCLUSIONSclerotherapy was used to successfully treat HHT-related AVMs in-office with no scarring and is an option that should be considered by providers.

Clinical benefit and fragility evaluation of systemic therapy trials for advanced soft tissue sarcoma

ABSTRACTBackgroundThe clinical benefit of systemic anticancer therapies can be unclear despite positive trials, and outcomes may not translate to real‐world practice. This study evaluated the benefit of soft tissue sarcoma (STS) treatments using the European Society of Medical Oncology Magnitude of Clinical Benefit Scale (MCBS) v1.1 and measured the robustness of STS trial results using Fragility Index (FI).MethodsDatabase searches for adult phase II or III trials in advanced STS (January 1998–December 2023) were performed. Therapies with trial outcomes that met the criteria for MCBS were scored 1–5 (≥4 represents substantial clinical benefit). For randomized clinical trials with positive time‐to‐event endpoints, the number of additional events that would render results nonsignificant, FI, was calculated and expressed as a proportion of the experimental arm size (fragility quotient [FQ]). Higher FI/FQ implies more robust results.ResultsAmong 194 trials, 19 (9.8%) were phase III. Most phase II trials (146/175; 83.4%) had single‐arm or non‐comparative design. Trials that were eligible for MCBS scoring (n = 78; 40.2%) evaluated 56 different agents/regimens. Median MCBS score was 2. Only three agents/regimens (all cytotoxic therapies) had an MCBS score ≥4. Among 47 randomized clinical trials, 16 (8 phase II; 8 phase III) trials had positive outcomes. Median FI was 7 (range, 2–52) and 10 trials (62.5%) had an FQ < 10%, with median of 7% (range, 1%–59%).ConclusionsMost systemic therapies in STS trials did not confer substantial clinical benefit per European Society of Medical Oncology‐MCBS. Additionally, positive randomized trials were often fragile. Novel STS therapy trials should use clinically meaningful endpoints and real‐world efficacy confirmation is essential, especially for less robust trials.
