The goal of this clinical trial is to learn if nitazoxanide (NTZ) can treat Cryptosporidium infection in children age 6-12 months. The main questions it aims to answer are:
* Does NTZ treatment of diarrheal Cryptosporidium infection lower the number of days of diarrhea?
* Does NTZ treatment of diarrheal and non-diarrheal Cryptosporidium infection lower the number of days that parasites can be found in the stool?
Researchers will compare NTZ to a placebo (a look-alike substance that contains no drug) to see if NTZ works to treat Cryptosporidium.
Participants will:
* Take NTZ or placebo for 3 days
* Receive regular visits from field research assistants
* Provide blood and urine samples

Comparison of CSF biomarkers in multiple sclerosis patients treated with natalizumab and rituximab

BACKGROUND:

Patients with relapsing remitting multiple sclerosis (RRMS) treated with the high-efficacy drugs natalizumab (NTZ) or rituximab (RTX) generally show no evidence of disease activity. Currently, there is no head-to-head comparison between NTZ and RTX of neurodegenerative and neuroinflammatory biomarkers in cerebrospinal fluid (CSF).

OBJECTIVES:

To compare CSF biomarkers in a stable RRMS cohort treated with NTZ or RTX. A secondary objective was to explore potential associations between CSF biomarkers, fatigue, and cognition.

METHODS:

This Swedish multicentre cross-sectional study assessed kappa-free light chain (k-FLC) index, oligoclonal bands (OCBs), glial fibrillary acidic protein (GFAP) and neurofilament light (NfL) in CSF of 30 RRMS patients on NTZ or RTX for at least 24 months. A longitudinal comparison of biomarkers was performed for IgG Indices. Fatigue and cognition outcomes were explored in relation to CSF biomarkers.

RESULTS:

GFAP level was significantly higher in the NTZ group compared to RTX (mean difference (CI): 2 716 (155; 5278) ng/L, p=0.047). NfL concentration did not differ between the groups. OCBs and k-FLC index were present and elevated in 97 % and 87 % of participants, respectively. IgG-index was significantly reduced only for NTZ. No significant associations were found between fatigue, cognition and the biomarkers.

CONCLUSION:

Our results support that intrathecal inflammatory activity is still ongoing in patients with NTZ and RTX. Cross-sectional GFAP might indicate a lower risk for long-term disability in the RTX group. Our data should be interpreted with caution because of small sample size, making it difficult to control for multiple confounders.

Nitazoxanide and Umbelliferone improves Imiquimod-induced psoriasis in Balb/C mice

Signal Transducer and Activator of Transcription (STAT) 3 is a significant contributor to the development and pathogenesis of psoriasis (Pso). Research demonstrated STAT3 signalling to be upregulated in Pso. Additionally, Pso results in oxidative stress that activates various signalling pathways like factor nuclear kappa-B (NF-κB). Nitazoxanide (NTZ) is an antiprotozoal drug shown to inhibit the STAT3 pathway. Umbelliferone (UMB) is an antioxidant, and anti-inflammatory and can suppress NF-κB signalling. Therefore, we propose to hypothesize that NTZ and UMB would be effective in treating Pso. Balb/c mice were treated with IMQ to induce Pso. The clinical characteristics of Pso were assessed using the Psoriasis Area and Severity Index (PASI), back skin thickness, spleen length and mass, and histology of the skin sample tissue. In addition, we measured the levels of interleukin-17 (IL-17), tumor necrosis factor-α (TNF-α), STAT3, and NF-κB. Furthermore, to gain the interaction of NTZ and UMB with STAT3, a detailed in-silico study has been performed. The impact of treatment on oxidative stress was evaluated by estimating the activity of superoxide dismutase (SOD) and catalase (CAT). The results demonstrate that mice subjected to IMQ-induced Pso exhibited positive responses to treatment with NTZ and UMB. Additionally, IL-17, TNF-α levels, STAT3, and NF-κB were decreased in NTZ and UMB-treated groups. SOD and CAT levels in NTZ and UMB groups were elevated. Our findings show that NTZ and UMB are potential therapeutic medications for Pso.

Nitazoxanide inhibits pili assembly by targeting BamB to synergize with polymyxin B against drug-resistant Escherichia coli

Gram-negative bacteria rely on pili assembly for pathogenicity, with the chaperone-usher (CU) pathway regulating pilus biogenesis. Nitazoxanide (NTZ) inhibits CU pathway-mediated P pilus biogenesis by specifically interfering with the proper folding of the outer membrane protein (OMP) usher, primarily mediated by the β-barrel assembly machinery (BAM) complex. In this study, we identified the BAM complex components BamB and the BamA POTRA2 domain as key binding targets for NTZ. Molecular dynamics simulations and Bio-Layer Interferometry revealed that BamB residues S61 and R195 are critical for NTZ binding. NTZ activated the Cpx two-component system and induced inner membrane perturbations, which resulted from the accumulation of misfolded P pilus subunits. Upregulation of the ibpAB gene, which protects the bacteria against NTZ-induced oxidative stress, was also observed. Importantly, NTZ combined with polymyxin B enhanced the latter's antibacterial activity against both susceptible and MCR-positive E. coli strains. This enhancement was achieved through NTZ-induced increases in inner membrane permeability, oxidative stress, and inhibition of efflux pump activity and biofilm formation. This study provides new insights into the antimicrobial mechanism of NTZ and highlights its potential as an antibiotic adjuvant by targeting BamB to inhibit the CU pathway, restoring the efficacy of polymyxin B against multidrug-resistant bacteria.
