The goal of this clinical trial is to learn if nitazoxanide (NTZ) can treat Cryptosporidium infection in children age 6-12 months. The main questions it aims to answer are:
* Does NTZ treatment of diarrheal Cryptosporidium infection lower the number of days of diarrhea?
* Does NTZ treatment of diarrheal and non-diarrheal Cryptosporidium infection lower the number of days that parasites can be found in the stool?
Researchers will compare NTZ to a placebo (a look-alike substance that contains no drug) to see if NTZ works to treat Cryptosporidium.
Participants will:
* Take NTZ or placebo for 3 days
* Receive regular visits from field research assistants
* Provide blood and urine samples

A rare case of pulmonary cryptosporidiosis associated with pulmonary tuberculosis in a patient with advanced HIV infection

Cryptosporidium spp., an important food and waterborne protozoan pathogen, can cause severe diarrhea and extraintestinal manifestations in immunocompromised individuals. The case of a 75 -years old patient diagnosed as a late presenter with advanced HIV infection and multiple opportunistic illnesses associated to HIV: cryptosporidiosis, intestinal and pulmonary, Cytomegalovirus colitis and pulmonary tuberculosis is presented. Cryptosporidium parvum was identified in fecal sample and bronchoalveolar lavage using rapid tests, PCR diagnosis, and sequencing, the two sequences being identical. In spite of adequate treatment for all associated infections, including Nitazoxanide for Cryptosporidium spp. infection, antiretroviral therapy, and management in the intensive care unit, the patient evolution was unfavorable, without immune reconstitution, leading to death on the 30th day of hospitalization. This case describes a rare localization of pulmonary cryptosporidiosis in association with pulmonary tuberculosis in a patient with advanced HIV infection. Clinicians should be aware of the possible pulmonary localization of this opportunistic parasite in immunodeficient patients.

Cheminformatics bioprospection of selected medicinal plants metabolites against trypsin cleaved VP4 (spike protein) of rotavirus A

Rotaviruses have continued to be the primary cause of acute dehydrating diarrhoea in children under five years of age despite the global introduction of four World Health Organization (WHO) prequalified oral vaccines in over 106 countries. Currently, no medication is approved by the Food and Drug Administration (FDA) specifically for treating rotavirus A-induced diarrhoea. Consequently, it is important to focus on developing prophylactic and curative therapeutics to combat rotaviral infections. For the first time, this study computationally screened and identified metabolites from Spondias mombin, Macaranga barteri and Dicerocaryum eriocarpum as potential novel inhibitors with broad-spectrum activity against VP5* and VP8* (spike protein) of rotavirus A (RVA). The initial top 20 metabolites identified through molecular docking were further filtered using drug-likeness and pharmacokinetics parameters. The molecular properties of the resulting top-ranked compounds were predicted by conducting density functional theory (DFT) calculations, while molecular dynamics (MD) simulation revealed their thermodynamic compatibility with a significant affinity towards VP8* than VP5*. Except for ellagic acid (-11.78 kcal/mol), the lead compounds had higher binding free energy than the reference standard (VP5* (-11.81 kcal/mol), VP8* (-14.12 kcal/mol)) with 2SG (-20.98 kcal/mol) and apigenin-4'-glucoside (-23.56 kcal/mol) having the highest affinity towards VP5* and VP8*, respectively. Of all the top-ranked compounds, better broad-spectrum affinities for both VP5* and VP8* than tizoxanide were observed in 2SG (VP5* (-20.98 kcal/mol), VP8* (-20.13 kcal/mol)) and sericetin (VP5* (-20.46 kcal/mol), VP8* (-18.31 kcal/mol)). While the identified leads could be regarded as potential modulators of the investigated therapeutic targets for effective management of rotaviral infection, additional in vitro and in vivo evaluation is strongly recommended, and efforts are on-going in this regard.Communicated by Ramaswamy H. Therapeutic potential of platelet rich plasma against experimental Cryptosporidium parvum infection: in vivo study in immunosuppressed mice

The present study was designed to assess the possible effects of platelet rich plasma (PRP) when used individually and in combination with nitazoxanide (NTZ) on experimental Cryptosporidium parvum (C. parvum) infection. It was conducted on 100 male albino mice, laboratory bred in Theodore Bilharz Research Institute. Starting from the 7th day post infection (p.i), therapeutics were given to immunosuppressed infected mice, which were divided as follows; oral NTZ treated group (0.2 mg/g/day for 6 consecutive days), six-PRP-treated groups (0.5 μl/g/week) to be administered intravenously (IV) in 1st, 2nd, 3rd week as PRP alone in (3 groups) and combined with oral NTZ (0.2 mg/g/day for 6 consecutive days) in (3 groups). Parasitological, histopathological and immunohistochemical assessments of therapeutics under study were done. Fecal pellets collected from groups at different intervals were stained using modified Ziehl-Neelsen and examined under microscope. Among PRP-treated groups, the highest significant percentage of oocyst reduction (89.96%) was observed in the group received 3 doses of PRP in combination with NTZ on the 35th day post infection. Likewise, the histopathological examination of small intestinal tissue sections showed improvement in villous architecture with mild to moderate stunting and moderate inflammatory infiltrates in lamina propria. Immunohistochemical staining of small intestinal tissue sections showed moderate increase in the expression of TGF-β1. Therefore, PRP can be a novel strategy in the treatment of cryptosporidiosis particularly when combined with NTZ.

GENFIT Reports Third Quarter 2024 Financial Information

Revenue growth is due to a milestone payment invoiced to Ipsen in June 2024 (collected in August 2024) following the first commercial sale of Iqirvo in the U.S. Of the €59.7 million in revenues for the first nine months of 2024, €48.7 million was attributable to a milestone payment invoiced to Ipsen in June 2024 and €0.9 million was attributable to royalty revenue from U.S. sales of Iqirvo/elafibranor which commenced mid-June in application of the Collaboration and License Agreement with Ipsen signed in December 2021. This follows the positive opinion issued by the Committee for Medicinal Products for Human Use (CHMP) of the European Medicines Agency (EMA) on July 26, 2024 and the U.S. FDA Accelerated Approval on June, 10, 2024 .
