The Prevention of Nephrotoxin-Induced Acute Kidney Injury Using Cilastatin (PONTiAK) Trial: A Phase 2B Vanguard Design, Randomized, Double-Blind, Placebo-Controlled Study

The goal of this clinical trial is to repurpose cilastatin for preventing acute kidney injury (AKI) in hospitalized patients receiving nephrotoxic medications. The trial will evaluate the efficacy of the re-purposed drug.
The main questions it aims to answer are:
- whether cilastatin will prevent nephrotoxic AKI in hospitalized patients.
Researchers will compare the drug cilastatin to a placebo (a look-alike substance that contains no drug) to see if drug cilastatin works to prevent AKI in hospitalized patients receiving nephrotoxic medications.
Participants will:
* Receive drug Cilastatin or a placebo intravenously every 6 hours for up to 24 hours after last exposure to nephrotoxic medication
* Have blood test for kidney function every day they are on treatment.
* Have a follow-up blood test at 90 days after randomization
* Have a telephone survey at 90 days after randomization

Increasing antimicrobial resistance (AMR) is one of the greatest threats to global health. In 2021, 4.71 million deaths worldwide were closely associated with AMR, and 1.14 million deaths could be directly attributed to infections caused by multidrug-resistant organisms (MDROs), particularly multidrug-resistant Gram-negative bacteria (MDRGN). Enterobacterales (e.g., Escherichia coli and Klebsiella spp.) resistant to third-generation cephalosporins and carbapenems, multidrug-resistant Pseudomonas aeruginosa, and carbapenem-resistant Acinetobacter baumannii (CRAB) have been identified by the World Health Organization as the most problematic pathogens. In addition to new diagnostic methods for the rapid identification of AMR, several new antibiotics have been approved in the last 10 years, expanding the treatment options, particularly for MDRGN infections. Pharmaceutical strategies have so far focused primarily on modifying already known classes of antibiotics with the aim of circumventing class-specific resistance mechanisms and reducing resistance rates. In addition to cefiderocol, the first siderophore cephalosporin, new combinations of β‑lactam antibiotics and β‑lactamase inhibitors (BLIs) such as ceftolozan/tazobactam, ceftazidime/avibactam, cefepime/enmetazobactam, imipenem/cilastatin/relebactam, and meropenem/vaborbactam, as well as the monobactam/BLI combination aztreonam/avibactam, have been approved and successfully implemented in clinical care. In addition, there is the synthetic tetracycline antibiotic eravacycline, which has a broad spectrum of activity against Gram-positive, Gram-negative (particularly clinically relevant Enterobacterales), anaerobic, and multidrug-resistant bacteria, and the new glycopeptide antibiotic dalbavancin, which is effective against Gram-positive bacteria. Since a change in legislation in 2021, the Joint Federal Committee (Gemeinsamer Bundesausschuss, G‑BA) in Germany has been authorized to classify newly approved antibiotics as reserve antibiotics. This classification allows for an exception to the regular additional benefit assessment as part of the early benefit assessment. The pipeline of antibiotics in development with novel targets and chemical structures-which had almost completely dried up-has been re-filled with new candidates for clinical trials. Some of these agents have already been tested with promising results in smaller phase 1/2 studies. In addition, monoclonal antibodies, antimicrobial peptides, small molecules, microbiome-modifying biotherapeutics, and bacteriophages, all enabling targeted and personalized treatment, are currently being investigated in studies.

Superselective embolization of the medial calcaneal artery: a minimally invasive and novel solution for rapid pain relief in plantar fasciitis

PURPOSE:

This study evaluated the efficacy and safety of superselective embolization of the medial calcaneal artery using imipenem/cilastatin in patients with chronic, treatment-resistant plantar fasciitis.

MATERIALS AND METHODS:

In this single-center prospective study, 32 patients (17 females, 15 males; mean age: 47.3 ± 9.1 years) with chronic plantar fasciitis unresponsive to conservative treatments underwent superselective embolization of the medial calcaneal artery using imipenem/cilastatin. In 31 patients (96.9 %), the target artery originated from the posterior tibial artery or lateral plantar artery; in one patient (3.1 %), it originated from the peroneal artery due to anatomical variations. Pain severity was assessed using the Visual Analog Scale (VAS), functional status with the Foot Function Index (FFI), and plantar fascia thickness using ultrasound imaging. Technical success, complications, and recurrence were recorded.

RESULTS:

All procedures were technically successful, with no major or minor complications. The mean VAS score decreased from 7.3 ± 0.7 at baseline to 1.3 ± 0.5 at six months (p < 0.001), while the mean FFI score improved from 55.5 ± 4.9 to 20.4 ± 6.3 (p < 0.001). Plantar fascia thickness decreased from 5.2 ± 0.6 mm to 3.7 ± 0.3 mm at six months (p < 0.001). Minimal recurrence was observed in five patients (15.6 %) with physically demanding occupations and orthopedic heel insoles were recommended; however, no repeat embolization was required.

CONCLUSION:

Superselective embolization of the medial calcaneal artery using imipenem/cilastatin represents a novel, safe, and minimally invasive strategy for rapid and sustained pain relief in patients with chronic plantar fasciitis.

Case Report: Rare Presentation of Nocardia cyriacigeorgica Pleural Nocardiosis in a Kidney Transplant Patient

BACKGROUND:

Nocardia species are gram-positive, filamentous bacteria causing opportunistic infections in immunocompromised patients. Nocardiosis in kidney transplant recipients is rare, with Nocardia cyriacigeorgica being an emerging pathogen first identified in 2001.

CASE PRESENTATION:

A 55-year-old male with end-stage renal disease underwent deceased-donor kidney transplantation with standard immunosuppression including tacrolimus, corticosteroids, and mycophenolate mofetil. Five months post-transplantation, he presented with cough, fever, and dyspnea. Initial chest computed tomography showed pleural effusion without consolidation. Despite percutaneous drainage and empirical antibiotics, symptoms persisted for 3 weeks. Repeat imaging revealed multiloculated pleural masses mimicking post-transplant lymphoproliferative disease. Surgical excision of the mass revealed inflamed granulation tissue, and cultures identified Nocardia cyriacigeorgica. Treatment was changed to intravenous imipenem/cilastatin and oral trimethoprim-sulfamethoxazole, with concurrent reduction of immunosuppression. The patient showed significant clinical improvement, with follow-up imaging demonstrating marked reduction in pleural masses. He was discharged after 10 weeks without graft dysfunction.

CONCLUSION:

This rare case of pleural nocardiosis caused by Nocardia cyriacigeorgica in a kidney transplant recipient highlights diagnostic challenges, as the presentation mimicked malignancy. High clinical suspicion is crucial when standard antimicrobial therapy fails in immunosuppressed patients. Early diagnosis through tissue culture and aggressive treatment with appropriate antibiotics, combined with immunosuppression adjustment, led to successful outcomes. This report adds valuable insights to the limited literature on this emerging pathogen in transplant recipients and emphasizes the importance of considering rare opportunistic infections in the differential diagnosis.

Arch Biopartners Scientists Publish New Data Linking the Cytokine IL-32 to Inflammation and Diabetic Kidney Disease

Approval for Cefiderocol in China

(Head Office: Shanghai, China; Chairman: Hirofumi Nagatome), has received approval from the National Medical Products Administration (NMPA) of China for cefiderocol (generic name: Cefiderocol Sulfate Tosylate for Injection; hereinafter “cefiderocol”) for the treatment of complicated urinary tract infections including pyelonephritis caused by Gram-negative bacteria.
