Classification of Skin Injury Risk Caused by Extravasation of Electrolyte Solutions or Infusions in a Rat Model

Cytotoxic agents are classified according to the severity of skin injury after extravasation. However, injuries caused by extravasation of noncytotoxic agents have not been sufficiently investigated, although the risk of extravasation is mentioned in medical safety information published by the Japan Council for Quality Health Care. Therefore, in this study, we focused on noncytotoxic electrolyte solutions and infusions and evaluated skin injuries during leakage using extravasation model rats. Rats were anesthetized and intradermally injected with 100 µL of an electrolyte solution or infusion. Injection lesions were macroscopically and histopathologically evaluated for extravasation injuries. Each electrolyte solution and infusion were classified into three categories (vesicants, irritants, and non-tissue-damaging agents) depending on the degree of skin injury. Similar to saline, 0.3% potassium chloride and 0.6% magnesium sulfate showed almost no injury, and 3% sodium chloride and BFLUID^® caused erythema and induration. Erythema, induration, and ulceration were observed with the following: 10% sodium chloride, 2% calcium chloride, 8.5% calcium gluconate, 12.3% magnesium sulfate, MAGSENT^®, FESIN^®, and Intralipos^®. The duration of damage with these agents was markedly prolonged. Electrolyte solutions and infusions can be classified into vesicants (10% sodium chloride, 2% calcium chloride, 8.5% calcium gluconate, 12.3% magnesium sulfate, MAGSENT^®, FESIN^®, and Intralipos^®), irritants (3% sodium chloride and BFLUID^®), and non-tissue-damaging agents (0.3% potassium chloride and 0.6% magnesium sulfate) according to their composition. The characteristic symptoms and severity of each drug extravasation revealed in this study will provide basic information for preparation of guidelines for treatment of extravasation.

Nebulized Magnesium Sulfate for Treatment of Persistent Pulmonary Hypertension of Newborn: A Pilot Randomized Controlled Trial

OBJECTIVESTo investigate the effectiveness of nebulized magnesium sulfate in treating persistent pulmonary hypertension of newborn (PPHN).METHODSTwenty-eight mechanically ventilated term neonates with severe PPHN were randomized into two groups: NebMag group (n = 14), who receiving nebulized isotonic magnesium (1024 mg/h), and IVMag group (n = 14), who received intravenous magnesium (200 mg/kg over 30 min, followed by 50 mg/kg/h). The study time frame was 24 h. Outcome measures were the changes in oxygenation index (OI), mean arterial blood pressure (MABP), vasoactive inotropic score (VIS), and serum magnesium level.RESULTSBaseline demographic, ventilatory, and hemodynamic characteristics were comparable between the two groups. At the end of the study, the OI decreased by 44.3% in the NebMag group compared with 35.3% in the IVMag group (mean difference -3.14; 95%CI -5.08, -1.19; p 0.003). The NebMag group had a higher MABP (mean difference 2.29 mmHg; 95% CI 1.80, 2.77; p 0.000) and lower VIS (mean difference -14.64; 95% CI -16.52, -12.77; p 0.000) at the 24-h study time point. The increase in serum magnesium level, measured at 12-h study time point, was lower in the NebMag group (mean difference -2.26 mmol/L; 95% CI -2.58, -1.96; p 0.000).CONCLUSIONNebulized magnesium sulfate may be an effective therapeutic modality for neonates with severe PPHN on mechanical ventilation, but this should be confirmed by larger studies. Physico-chemical properties of biodiesel manufactured from waste frying oil using domestic adsorbents

We have evaluated the efficiency of sugar cane bagasse ash (SCBA), date palm seed carbon (DPSC), and rice husk ash (RHA) as natural adsorbents and compared them with the synthetic adsorbent Magnesol XL for improving the quality of waste frying oil (WFO) and for the impact on the physicochemical properties of the obtained biodiesel. We measured moisture content, refractive index (RI), density, acid value (AV), iodine value (IV), peroxide value (PV), and saponification value (SV), as well as fatty acid profile. Purification treatments with various levels of adsorbents caused significant (P ≤ 0.05) decreases in free fatty acids (FFAs), PVs, and IVs. The highest yields (86.45 and 87.80%) were observed for biodiesel samples produced from WFO treated with 2% Magnesol and 3% of RHA, respectively, followed by samples treated with 2 and 3% of DPSC or RHA. Pre-treatments caused a significant decrease in the content of C 18:2 linoleic acids, consistent with a significant increase in the content of monounsaturated and saturated fatty acids (MUFA) in the treated samples. The highest oxidation value (COX) (1.30) was observed for biodiesel samples produced from WFO without purification treatments. However, the lowest values (0.44-0.73) were observed for biodiesel samples produced from WFO treated with different levels of adsorbents. Our results indicate that pre-treatments with different levels of adsorbents regenerated the quality of WFO and improved the quality of the obtained biodiesel.
