A Phase II Study of Iodine-131-Labeled Tositumomab in Combination With Cyclophosphamide, Doxorubicin, Vincristine, Prednisone and Rituximab Therapy for Patients With Advanced Stage Follicular Non-Hodgkin's Lymphoma

RATIONALE: Radiolabeled monoclonal antibodies, such as iodine I 131 tositumomab, can find cancer cells and carry cancer-killing substances to them without harming normal cells. Monoclonal antibodies, such as rituximab, can block cancer growth in different ways. Some block the ability of cancer cells to grow and spread. Others find cancer cells and help kill them or carry cancer-killing substances to them. Drugs used in chemotherapy, such as cyclophosphamide, doxorubicin, vincristine, and prednisone, work in different ways to stop the growth of cancer cells, either by killing the cells or by stopping them from dividing. Giving a radiolabeled monoclonal antibody together with rituximab and combination chemotherapy may kill more cancer cells.
PURPOSE: This phase II trial is studying the side effects of giving iodine I 131 tositumomab together with rituximab and combination chemotherapy and to see how well it works in treating patients with previously untreated stage II, stage III, or stage IV follicular non-Hodgkin lymphoma.

The purpose of this study is to find the highest safe dose of Iodine-131 Tositumomab (Bexxar®) that can be given to patients who have relapsed/refractory Hodgkin's lymphoma, what side effects these patients get when they take Bexxar® and if Bexxar® is effective in treating relapsed/refractory Hodgkin's lymphoma. In Greek mythology, The Phoenix is an immortal bird that dies, but then achieves new life by rising from the ashes of its predecessor. Radioimmunotherapy (RIT) of B-cell Non-Hodgkin lymphoma (NHL) is a field which once began to fly high-with FDA approval of the anti-CD20 RITs Zevalin® and Bexxar® in 2002 and 2003 respectively, as safe and effective therapies of NHL. However, despite their therapeutic efficacy, Bexxar® was withdrawn from the market by the manufacturer in 2014 due to limited commercial demand and Zevalin® has had very limited to no availability of late. I-131 rituximab is used to a limited extent in Australia, India and other countries, as well. But has RIT of NHL been (perhaps prematurely) left for dead by many? Given the current great clinical and commercial interest in radiopharmaceutical therapies of cancer, notably PSMA and SSTR targeting agents in prostate and neuroendocrine cancers, can radioimmunotherapy of NHL-like the mythical Phoenix-now rise from its ashes in an even better form to fly higher, faster, farther and longer than before?

A Systematic Review of Clinical Applications of Anti-CD20 Radioimmunotherapy for Lymphoma

Abstract:

Purpose:

The clinical efficacy of anti-CD20 radioimmunotherapy (RIT) is due to a combination of extracellular mechanisms involving immune-mediated cytotoxicity, and intracellular mechanisms related to inhibition of CD20 signaling and DNA damage from ionizing radiation. In 2002, the first RIT was approved by the U.S. Food and Drug Administration for the treatment of patients with indolent B-cell follicular non-Hodgkin lymphoma (NHL). The 2 approved agents, 90 Y-ibritumomab tiuxetan (90Y-IT, Zevalin, Acrotech Biopharma) and 131 I-tositumomab (131-IT, Bexxar, GlaxoSmithKline) both target CD20. The aim of this study was to review the clinical applications and supporting clinical trial data of anti-CD20 RIT for lymphoma.

Methods:

A review of published articles and abstracts on the clinical efficacy and safety of 90Y-IT and iodine I 131 tositumomab was performed.

Results:

The clinical efficacy and safety of anti-CD20 RIT have been demonstrated in numerous clinical trials and case series. Agents have produced significant responses in patients with follicular NHLs and in off-label applications. Importantly, RIT has demonstrated promising findings in high-risk lymphomas and heavily pretreated and refractory patient populations. Associated toxicity profiles are noted as tolerable, acceptable, and most often reversible.

Conclusions:

In the 2 decades since its approval, anti-CD20 RIT continues to demonstrate efficacy, particularly with a proportion of patients maintaining long-term remissions. The combination of prolonged efficacy, tolerability, and treatment convenience makes RIT a reasonable alternative to other systemic therapies. It is recommended that further research on RIT should focus on biomarkers of long-term response, pretargeting, and sequencing of RIT in the treatment course.

Iodine-131 intervention in hyperthyroidism with hepatic insufficiency: Metabolomic evaluation

Hyperthyroidism, often accompanied by hepatic insufficiency (HI), poses significant clinical challenges, highlighting the necessity for identifying optimal treatment strategies and early diagnostic biomarkers to improve patient outcomes. This study aimed to determine the optimal iodine-131 (¹³¹I) intervention dose for alleviating hyperthyroidism with HI and to identify serum metabolic biomarkers for early diagnosis using UPLC-Q/TOF-MS technology. A mouse model for early ¹³¹I intervention was established to monitor changes in physiological response, body weight, fur condition, thyroid, and liver function. Metabolite identification was achieved through UPLC-Q/TOF-MS and further analyzed via MetaboAnalyst. Six biomarkers were identified and subjected to ROC analysis. Early intervention with 80 μCi ¹³¹I per gram of thyroid tissue effectively controlled hyperthyroidism and improved liver function. Metabolomics analysis uncovered 63 differentially abundant metabolites, six of which (L-kynurenine, Taurochenodesoxycholic acid, Glycocholic acid, Phytosphingosine, Tryptamine, and Betaine) were identified as early warning biomarkers. Post-intervention, these biomarkers progressively returned to normal levels. This study demonstrates the efficacy of UPLC-Q/TOF-MS in identifying metabolic biomarkers for early diagnosis of hyperthyroidism with HI and highlights the therapeutic potential of early ¹³¹I intervention in normalizing these biomarkers.

Radiopharmaceuticals for cancer: Making radiation precise

Editors note: BioPharma Dive, as part of Emerging Biotech Weekly, is taking a closer look at competitive areas of startup activity. We aim to give an overview of the companies developing a new technology and what their goals are.When the Food and Drug Administration approved a new prostate cancer drug last year, it validated a bet made by Novartis to acquire the drugs maker, Endocyte. The decision was also another step forward for a long-studied research field that appears to finally be coming of age.The medicine, Pluvicto, is known as a radiopharmaceutical. Unlike the small molecule or biologic medicines drugmakers use to flip cellular switches on or off, radiopharmaceuticals are designed to precisely deliver radioactive material into the body. The result is a targeted punch of radiation that can knock out tumors in a way other drugs cant.However, since Pluvictos approval, Novartis has struggled to meet demand, highlighting one of the many issues that has slowed development of radiopharmaceuticals. Theyre difficult to produce, distribute and administer. The earliest examples never lived up to commercial expectations, hamstrung by manufacturing problems, competition, high costs and safety concerns. GSK stopped making one, known as Bexxar, in 2014. Another, named Zevalin and originally developed by Idec Pharmaceuticals, never became a big seller.A group of companies, from biotechnology startups to pharmaceutical giants, think theyve solved these problems. Borrowing from progress in another cancer drug field, theyre leaning on technical advancements that have enabled researchers to design medicines capable of safely delivering a radioisotope to just the right spot.Novartis is the most invested, having spent $6 billion on acquisitions of Endocyte and Advanced Accelerator Applications. Right alongside Novartis is Bayer, which has also brought a radiopharmaceutical to market and is testing others.Behind them are a handful of publicly traded companies and at least a dozen biotech startups, making radiopharmaceuticals a hotly competitive area of drug research. Heres where things stand:What are radiopharmaceuticals, and how do they work?Born shortly after the discovery of the X-ray, radiation therapy has more than a century of history in cancer treatment. Its a way of using high-energy particles or waves to damage the DNA of cancer cells, preventing division and growth.Along with surgery, radiation therapy is a mainstay of cancer care, used to treat or prevent the recurrence of tumors in more than half of people with the disease, according to the American Cancer Society.But radiation therapy is a blunt force. Rays beamed from outside the body can damage healthy tissue, while a more targeted, inside-the-body approach is costly and complex.By comparison, radiopharmaceuticals can deliver the destructive power of radiation directly into tumors. They are akin to microscopic smart bombs radioactive material guided by a molecular courier to cells with specific protein flags.With Novartis drug Pluvicto, a radioisotope known as lutetium is chemically fused to a small molecule that binds to PSMA, a protein overexpressed in most prostate cancers. Novartis other radiopharmaceutical, Lutathera, delivers radioactive lutetium to SSTR, a target found on certain neuroendocrine tumors.The drugs are complex, made of hand-picked parts. Different radioactive materials may weigh more or less, last longer or pack a bigger punch. Some radiopharmaceuticals may require a chelator, a molecule that keeps the radioactive material intact as its carried through the body.Drugmakers also have to design chemical linkers that hold their therapies together, but dont stick around too long after reaching their target. A molecular guide that seeks out malignant cells is another necessary component.Once designed, radiopharmaceuticals are tricky to produce at scale. Supplies of radioactive materials are limited, and their transport is controlled. The drugs must be made and delivered quickly, before the radioactive components decay too much.Why are radiopharmaceuticals a hot area of investment?Interest in radiopharmaceuticals mirrors the ascent of another class of targeted cancer medicines, known as antibody-drug conjugates, or ADCs.Like radiopharmaceuticals, ADCs deliver a toxic substance, usually a chemical, directly to a tumor. After years of slow progress, theyve undergone a renaissance catalyzed by technical advances, regulatory approvals and interest from large drugmakers. Venture investors are in turn backing new startups.Radiopharmaceuticals are now in the spotlight, too. Despite the poor sales of the fields first medicines, newer drugs are performing better. Studies supporting some, like Pluvicto, have shown radiopharmaceutical drugs can extend survival and, in some cases, outperform other types of treatments.Radiopharmaceutical drugs are now being recognized as an effective, safe, and economically and logistically viable treatment modality, wrote physicians from Johns Hopkins University School of Medicine and Memorial Sloan Kettering Cancer Center, in a 2020 article in Nature Reviews Drug Discovery.As is often the case, large pharma investment is sparking a chain reaction of activity. Novartis acquisitions of Advanced Accelerator and Endocyte yielded Lutathera and Pluvicto, which, despite production challenges, are seeing strong demand. Bayer reached close to $500 million in peak yearly sales with its radiopharmaceutical Xofigo; its now working on newer medicines and, in 2021, acquired two startups.Diagnostics company Lantheus Holdings bought the maker of Azedra, another radiopharmaceutical recently approved by the FDA for rare neuroendocrine tumors.Those deals, along with radiochemistry advances and a greater ability to innovate in complex therapeutic modalities, have spurred a wave of investments in new radiopharmaceutical companies, wrote Faisal Khurshid, an analyst with Leerink Partners, in a report last year.Some, like the publicly traded biotech Fusion Pharmaceuticals and startup RayzeBio, are going after the same targets as Pluvicto and Lutathera, but with different radioactive components or guiding molecules.Others are pursuing different cancer-associated proteins. The startup Abdera Therapeutics has a program in development aimed at DLL3, a long-studied target in a tough-to-treat form of lung cancer. RayzeBio began a Phase 3 trial in May, while ITM is further along in its late-stage program. Molecular Targeting Technologies, Arecium Therapeutics and Orano Med also have SSTR-targeting medicines in human trials.A group of developers are following in Pluvictos footsteps with drugs aimed at cells expressing PSMA. Curium Pharma has a Phase 3 study underway in prostate cancer. Point and partner Lantheus are expecting late-stage results later this year. Medicines from Fusion, Convergent and Advancell are in earlier stages.ArtBios lead program is also for prostate cancer and has entered human trials. It uses a radioactive isotope called Pb212 that the startup claims has an ideal clinical profile. Advancell and Orano Med use Pb212 as well.Other companies are earlier in their journeys. Aktis, for example, hasnt disclosed a lead program. But it has revealed the radioactive material, Actinium-225, it intends to deliver with drugs. Cancer Biotech Abdera Reveals $142M for Better Antibody-Based Radiation Drugs

Less than three years ago, a group of Canadian scientists got together to brainstorm a better radiopharmaceutical. Radiation has a long history in cancer care, used in imaging to detect the disease as well as in therapeutics that treat it. Radiopharmaceuticals, therapies that deliver a radioactive substance that kills cancer cells, have seen a research resurgence in recent years as big pharmaceutical companies and biotech startups aim to make these therapies more effective against tumors and less toxic to healthy tissue. The scientists, based in Vancouver, British Columbia, settled upon an idea that they thought could set them apart from the growing radiopharmaceuticals field. They formed a company, secured some initial financing, and then validated their approach in preclinical research. On Thursday, their startup, Abdera Therapeutics, revealed its science along with $142 million in financing behind it. Abdera, which now splits its operations between Vancouver and Menlo Park, California, came about as a thought exercise, said Adam Judge, one of the co-founders and the startup’s senior vice president of research. Judge’s experience includes senior roles at Genevant Sciences and Tekmira Pharmaceuticals, where his research included building delivery systems for hard-to-deliver drugs. Mike Abrams, another co-founder and a senior advisor to Abdera, has extensive experience in radiopharmaceutical imaging agents. The third co-founder, Lana Janes, was a venture partner at adMare BioInnovations, where Abdera incubated. She is now the startup’s senior vice president, operations and innovation. The scientists tossed about various ideas for improving radiopharmaceuticals. Wherever the radio-particle goes in the body is where you’re going to see toxicity, Judge said. Ideally, a radiopharmaceutical will deliver as much of the isotope to the tumor as possible, while also clearing out of other tissues to avoid causing toxic effects. Some research efforts are employing small molecules or peptides to target the delivery of a radiopharmaceutical. The problem with both approaches is that the therapy is excreted through the kidneys, which leads to toxic effects there, Judge said. That toxicity problem led the founders to turn to antibodies. Antibodies avoid the kidneys, and the idea of using an antibody for targeted delivery of a radioisotope has been done successfully. The non-Hodgkin lymphoma radiopharmaceuticals Zevalin, approved in 2002, and Bexxar, approved in 2003, were both made with antibodies from mice. But even though these early antibody-based radiopharmaceuticals avoided the kidneys, they still caused other side effects. Because of their long half-lives, they stayed around for a long time in circulation, causing systemic toxicities, Judge said. Neither Zevalin nor Bexxar was commercially successful and both were eventually discontinued by their manufacturers. The Abdera scientists still liked the idea of working with antibodies. But rather than using a mouse antibody or repurposing an existing one, Abdera’s founders decided to start from scratch. Abdera’s engineered antibodies behave like a conventional antibody, offering similar targeting capabilities, Judge said. But at half the size and half the molecular weight of a typical antibody, Abdera’s antibodies are better able to penetrate into solid tumors. Furthermore, they don’t stay around in circulation long enough to cause systemic side effects. “Wherever the antibody goes, the isotope goes,” Judge said. “The antibody drives the clearance.” The accelerated clearance of the antibody is driven through the liver. This approach is preferred to the kidney because the liver is a large organ that can handle a large radioactive dose and clear the isotope through the gut, Judge said. Also, the liver is more radioresistant than the kidney and it has the capacity to regenerate if needed. In preclinical testing, Abdera saw good biodistribution into the tumor, as well as adequate and rapid clearance of the therapy from tissues, Judge said. The biodistribution data serve as a positive sign of finding a therapeutic window, the dose range that balances safety and efficacy. The results also show high uptake of therapy by a variety of solid tumors. Abdera launched in early 2021 backed by 8 million Canadian dollars in seed financing from adMare BioInnovations and AbCellera Biologics, a Vancouver-based antibody developer. In mid-2021, the startup was able to raise its Series A financing, which was led by Versant Ventures and Amplitude Ventures. Other investors in the round include Northview Ventures as well adMare BioInnovations and AbCellera. Judge said Abdera didn’t announce the $32.5 million Series A round at the time because the startup did not yet have a story to tell. The cash came before the company had shown its technology can work in animal testing. Those data came soon enough. Abdera went “from a thought exercise to in vivo preclinical proof of concept in 10 months,” Judge said. On the heels of achieving the key preclinical data, the company was able to raise more money. The $110M Series B round is new cash announced Thursday. The latest financing was led by venBio Partners with participation from the earlier investors as well as new ones, including Viking Global Investors, Qiming Venture Partners USA and RTW Investments. The preclinical data that caught investor interest showed Abdera was able to hit five cancer targets. The startup is disclosing just one of them: Delta-like ligand 3, or DLL3. This protein is expressed in a variety of solid tumor types while also being rarely found on healthy cells. Abdera’s lead program, ABD-147, is being developed for treating small cell lung cancer. Radiopharmaceuticals are seeing stronger research interest as two new Novartis therapies gain traction in the market, said Abdera President & CEO Lori Lyons-Williams, a former executive at neuroscience drug startup Neumora. In 2018, the FDA approved Novartis’s Lutathera as a treatment for certain gut cancers. Last year, the FDA approved Pluvicto, a bladder cancer radiopharmaceutical that Novartis added via its $2.1 billion acquisition of Endocyte. “The medical community has been more willing to adopt them,” she said. “I think that’s a reason why there’s been a renaissance in radiopharmaceuticals.” Novartis is looking to expand its radiopharmaceutical scope beyond Lutathera and Pluvicto. Last month, the pharma giant paid $50 million up front to start a partnership focused on developing new radiopharmaceuticals based on the peptide technology of Bicycle Therapeutics. Among the advantages touted by the Cambridge, U.K.-based biotech is kidney excretion—it avoids liver toxicity. Bayer entered the radiopharmaceutical field with 2021 acquisitions of Noria Therapeutics and PSMA Therapeutics. Other radiopharmaceutical companies in the space include Aktis Oncology, a startup with financial backing from Novartis and Bristol Myers Squibb; Mariana Oncology, a startup that emerged from stealth in 2021 as Curie Therapeutics; and RayzeBio, a company that like Abdera, counts Versant Ventures as an investor. Venture capital firms usually don’t bet against their own portfolio companies. In Abdera’s funding announcement, Joel Drewry, principal at Versant, said his firm believes “Abdera’s approach represents a new wave of innovation in this space to address important cancer targets that may be intractable to other approaches.” Lyons-Williams joined Abdera last May when the company had nine employees. Current headcount is 30. Lyons-Williams expects to add 15 more by the end of this year as Abdera works toward an investigational new drug application filing for ABD-147 in 2024. The targets for Abdera’s four other programs remain undisclosed. Lyons-Williams said some are targets that have clinical validation but have yet to lead to successful therapies with other drug modalities due to toxicity. In other cases, the target is one that few drug hunters are pursuing.
