[Deflazacort-induced Steven-Johnson syndrome: a case report and literature review].

Objective: To summarize the clinical features and outcomes of deflazacort-induced Steven Johnson syndrome (SJS)-toxic epidermal necrolysis (TEN) to raise awareness among patients with Duchenne muscular dystrophy (DMD), neurologists as well as other deflazacort users. Methods: The clinical data of a boy with DMD who had SJS induced by deflazacort treated at the Department of Rheumatology & Clinical Immunology of Tianjin Children's Hospital in July 2024 was analyzed retrospectively. Taking "deflazacort" "Steven-Johnson syndrome" "toxic epidermal necrolysis" in Chinese or English as the keywords, literature was searched at CNKI, Wanfang, China Biomedical Literature Database and PubMed up to July 2024. The clinical characteristics, treatment and outcomes of deflazacort-induced SJS-TEN were summarized. Results: A 12-year-old boy was admitted with a 3-day history of rash. He was diagnosed with DMD at the age of 3 and had been treated with prednisolone since the age of 8. Forty-four days before admission, the patient started deflazacort to replace prednisolone. Three days before admission, progressively worsening erythematous maculopapular rashes, blisters and skin peeling (8% body surface area), oral mucosal erosion, and exudative conjunctivitis occurred, thus deflazacort was discontinued. Complete remission of SJS was achieved after treatment with intravenous immunoglobulin (IVIG, total 1.4 g/kg), 2 doses of etanercept (0.9 mg/kg, once), subcutaneous injection and intravenous methylprednisolone (0.7 mg/(kg·d)). Based on the literature, there were 5 reports in English while none in Chinese, altogether 7 cases were reported. All the patients were male, aged 3-45 years. Duration of deflazacort exposure was 2-8 weeks. Dermatology diagnosis of our case was SJS, and 5 cases were TEN. One patient was diagnosed with exudative erythema multiforme, and subsequent deflazacort oral challenge test was positive. Treatment included methylprednisolone or dexamethasone in 5 cases, IVIG in 6 cases, etanercept in 3 cases and cyclosporine in 1 case. All patients recovered completely. Conclusion: The synthetic corticosteroid deflazacort can cause rare but severe adverse reactions such as SJS-TEN, which needs close monitoring and prompt recognition and management.

Abstract:

Green, selective and accurate high-performance liquid chromatography (HPLC) chromatographic method is presented for simultaneous separation and quantitation of the co-prescribed drugs in chemotherapy omeprazole, ondansetron and deflazacort in spiked human plasma. An isocratic HPLC separation was performed on X Bridge C18 (4.6 × 250 mm) column with 5 μm particle size using mobile phase consisting of methanol: ammonium acetate buffer pH 4 adjusted by acetic acid (60: 40, v/v). The injection volume was 20μL with UV detection wavelength at 237 nm at room temperature. Flow rate of the mobile phase was adjusted to be 2.0 ml/min. Dexamethasone was used as internal standard to correct the variation during sample pretreatment. FDA guidelines were followed to validate the developed method. Successful application of the developed method was revealed by simultaneous determination of omeprazole, ondansetron and deflazacort in spiked human plasma in ranges of 1–20, 0.1–8 and 0.2–8 μg mL−1 for omeprazole, ondansetron and deflazacort, respectively. Four greenness assessment tools were used to evaluate the greenness of the developed method and the results were accepted. This method permitted the accurate simultaneous determination of the studied drugs, thus it can be used during therapeutic drug monitoring in daily clinical practice.

The impact of genotype on age at loss of ambulation in individuals with Duchenne muscular dystrophy treated with corticosteroids: A single‐center study of 555 patients

Abstract:

Introduction/Aims:

Studies have demonstrated that certain genotypes in Duchenne muscular dystrophy (DMD) have milder or more severe phenotypes. These studies included individuals treated and not treated with corticosteroids and multiple sites with potentially varying standards of care. We aimed to assess genotype–phenotype correlations for age at loss of ambulation (LoA) in a large cohort of individuals with DMD treated with corticosteroids at one center.

Methods:

In this retrospective review of medical records, encounters were included for individuals diagnosed with DMD if prescribed corticosteroids, defined as daily deflazacort or prednisone or high‐dose weekend prednisone, for 12 consecutive months. Encounters were excluded if the participants were taking disease‐modifying therapy. Data were analyzed using survival analysis for LoA and Fisher's exact tests to assess the percentage of late ambulatory (>14 years old) individuals for selected genotypes.

Results:

Overall, 3948 encounters from 555 individuals were included. Survival analysis showed later age at LoA for exon 44 skip amenable (p = .004), deletion exons 3–7 (p < .001) and duplication exon 2 (p = .043) cohorts and earlier age at LoA for the exon 51 skip amenable cohort (p < .001) when compared with the rest of the cohort. Individuals with deletions of exons 3–7 had significantly more late ambulatory individuals than other cohorts (75%), while those with exon 51 skip amenable deletions had significantly fewer (11.9%) compared with other cohorts.

Discussion:

This confirms previous observations of genotype–phenotype correlations in DMD and enhances information for trial design and clinical management.

PTC Therapeutics’ Translarna misses European regulatory hurdle again

The EU first gave Translarna a conditional marketing authorization in 2014 for the treatment of DMD, a progressive condition that causes the muscles to weaken, eventually becoming life-threatening. Eligible patients for the treatment are at least 2 years old, are able to walk, and their disease is caused by a so-called nonsense mutation in the protein dystrophin, which strengthens and protects muscle fibers The initial approval decision was weighted by a lack of treatments for DMD and was contingent on further trials to confirm its benefits. However, two studies of Translarna after the authorization called 020 and 041 missed their efficacy goals. While a specific subgroup of patients in study 020 did show evidence of responding to the drug, patients in 041 saw no significant differences in the deterioration of their walking ability relative to those in the placebo group. The CHMP produced a negative opinion for Translarna in September 2023, which it confirmed in January 2024. The CHMP then went back into discussion in May this year when the European Commission requested that the committee consider the evidence in addition to more real-world data, but upheld the rejection. In July this year, PTC Therapeutics requested a re-evaluation of the treatment from the studies in addition to data from a different study comparing patients from different registries, which appeared to show a delay in disability in some treated patients. However, the CHMP once again declined, citing differences between the registries and biases. It also took into account third-party perspectives from patients and healthcare professionals. googletag.cmd.push(function () { googletag.display('text-ad1'); }); In a public statement , Matthew Klein, CEO of PTC Therapeutics, said the "CHMP again based its decision on the results of the primary analysis subpopulation of Study 041 instead of the totality of evidence for Translarna.” He later said that the decision “is clearly against the expressed wishes of physicians, patients and families throughout Europe.” The opinion will now be reviewed by the European Commission, which will make a legally binding decision within around two months. For now, Translarna is still commercially available and PTC is working to make sure the EC has the evidence it needs. Klein told Outsourcing Pharma that in discussions with FDA, the US regulator has been open to considering review of an NDA based on the totality of evidence, unlike the EMA’s CHMP. The DMD landscape There is no cure for DMD, with current treatments including surgery to correct postural deformities and steroids to improve muscle strength. Translarna is designed to restore the production of dystrophin in muscles by making cells bypass the nonsense mutation when making the protein. The market for DMD treatments was projected to grow from $3.42 billion in 2024 to $8.19 billion by 2029, with a growth of 19.13% per year. The growth is driven by the increasing DMD burden and rising investments in the space. Translarna itself bagged $355.8 million in net revenues in 2023 compared to $288.6 million in 2022. Other treatments on the market for various types of DMD include Sarepta’s AMONDYS 45™ (casimersen) and gene therapy Elevidys. Meanwhile, Pfizer’s gene therapy fordadistrogene movaparvovec hit a setback in a phase 3 trial when it failed to meet its primary efficacy endpoint. “DMD is a highly morbid, complex and rare disorder that will continue to be treated with a combination of therapies, including those that target inflammation (such as deflazacort or other corticosteroids) and genetic targeted therapies, such as Translarna in the case of DMD secondary to nonsense mutations,” Klein said. “Given disease heterogeneity it will be important for there to be multiple therapeutic options.”

This approval is under the accelerated approval pathway, based on the observed increase in dystrophin production in the skeletal muscle of patients treated with VYONDYS 53.
